I used to be a graphic designer and it has been a few year that I haven't used inDesign

how do you add the cover to your indesign book file?

If you were to get a freelance project from a publisher (penguin, simon and schuster, etc) would the art director provide you with a template for the book cover/ book cover jacket. So you woulden't have to manually set this up? and just focus on the design part?

Templates usually come from the printer, not the publisher, however, the publisher may have some available. I usually rely on my own resources to set this up though because each book has a different page count, paper and set up making it impossible to use one template for every book.
