After the 2023 Grammy Awards, Madonna's strange, pillow face appearance has been a trending conversation. What happened to Madonna's face? Dr. Karam is sharing how YOU can avoid having the overfilled, pillow face look as you embark on your anti-aging journey. I am 70….I still look like me but just older and I’m ok with that. My face matches my hands and my body. I do my hair and make up, dress nicely and I smile when I am talking to someone. No regrets. ❤😊❤

DON'T EVER GET GRUMPY..hell.breaks..lose..madonna was clueless to the challenge and she hooked up with an amateur.. like most people do . especially when doing tattoos and photography..all one of the same . you must incorporate PASSION AND ART.. from the master's hands and eyes..not yours you will f...it up!is how it works

That's why. We all have limitations of money that slow us down. They just do whatever, trying to look 21 forever. We know we're lucky if we can simply achieve 20 years younger than whatever age we actually are. So at 60, we'd be aiming for 40 not 21 and would know we can't possibly afford to look 21 even if it was technically possible for us.

I know Madonna won’t see this comment, but if she did…I would tell her that people weren’t being ageist about her looks. People were shocked by the disfigurement she’s had done to her face. Plain and simple. It’s hard for some people to accept that they’re aging.

I agree. I think ageism would look like if she performed looking closer to her natural age and people were dunking on her for it and complaining that she looked old. Being surprised and a little uncomfortable by her completely different and unnatural look seems normal... I'm not advocating anyone be mean about it, but I get the recoil factor.

I completely disagree with you. People have been calling her old since 1993. I have magazine covers where they called her grandma Madonna when she was 34 years old. Let that sink in. If you were called old while you are still in your mid-30’s you would be traumatized as well.

@@anthonymejia2549 People called her older looking because she was older than she claimed..in 1993 she was 41 not 34 and she was 44 in 1996....I remember when MTV announced her exact age on her birthday in 1996 and let the cat out of the bag. My friend who worked with the Sex Pistols knew her from the London music scene in the 1970s. She is 71 yrs old this year.
I was so shocked to see what Madonna had done to herself. I don’t think she accomplished a “ youthful” look, but rather looks like she’s wearing a mask. It’s too bad.

I had a similar thought: Doctors allways say inflammatory processes in the body are bad for your health. If you have a bad tooth it can affect your heart and so on. But inflamation because of filler? Who cares? Really???!!!

Exactly. It's all very cookie cutter. No one seems to have their own face anymore. They all look alike, and it's not pretty. Especially in younger women, it's that same overfilled lips & cheeks, plastic blow-up doll look. The fake gigantic lips and big plastic breasts are atrocious. I always wonder how much worse they are going to look as they age. There are whole generations of women sporting or wanting the Kardashian/Jenner look, when the Kardashians & Jenners stopped looking like their true selves a decade ago or longer. I don't think there's anything wrong with having plastic surgery, but stop the madness!

Yes. All the equality we've appeared to achieve is to rope men into the same bs. I use skincare and sunscreen and am still waiting for my first wrinkle at 42, I'd be way too chicken to have surgery I think so ageing it is. Have a good day, friend

I'm 55 and naturally look 15 years younger thanks to my family genetics. *praise God* But, I'm not against cosmetic procedures. That said - Madonna has gone beyond! She's turned herself into a monster face. Sad.

You don’t have to, but you also have the option to use available methods to reduce signs of aging. I’ll choose the second option.
Regarding Madonna, I'm surprised you didn't mention the obvious face lift and brow lift which completely changed her hairline, along with the fillers and giant lips. I'd be surprised if she didn't get her jaw shaved, also, which I've seen in many other female media stars do now. Actually, she scared me when I saw her.

She had a facelift like 20 years ago and looked fantastic afterwards. There’s been nothing done to her jaw. And her hairline looks fine. It’s just the puffy cheeks and lips that are the problem and that can go away if she can manage to let go of her addiction.
You explained everything very nicely and truthfully. I am also a doctor, a dermatologist and I completely agree with you. It's amazing how many people don't see the impact of the pharmaceutical companies that make fillers, it's become like taking drugs.

People are most probably not aware of the inflammation processes that occur in their skin and tissue due to fillers, or they think that it is a temporary effect. I can't help but remember the film 'Catwoman' (with Halle Berry and Sharon Stone).

@@annehedonia156 I can only say personal experience on my own skin. I have been doing microneedling (dermaroller or dermastamp) without any combination with heat for 15 years. I hear various experiences from my colleagues, regarding, for example, RF microneedling, mainly that the device itself is not adjusted properly, so fat reduction occurs. In any case, I very much agree with Dr. Karam, as far as the skin is concerned, microneedling, chemical peeling, certain lasers, fat grafting for volume and later a perfect vertical lifting.
Thank you for telling the truth. In the United States we are obsessed with looking like we did at 25 and it is causing a lot of body dysmorphia and other issues. I am 58 years old and I am learning to love who I see in the mirror because I see people taking chances and doing risky procedures to look 30 years younger and it's unattainable. I am going to gracefully and gleefully turn 59 in July and continue to love who I see in the mirror. Listening to the wisdom that you are sharing has made me confident in my decision.

I think the main problem with fillers, especially in the upper and mid face area, is that they cause the eyes (which already recede with age) to be dwarfed by all that added volume. Madonna is a great example. She never had a heart shaped face to begin with, now she does. But her big, wide-set expressive eyes now look close-set and beady. Like buttons in a pillow - that’s the effect.
It’s a shame that Madonna hadn’t met the current Dr. Karam in her early 50’s or even 40’s. It’s sad that someone or more people did that to her beautiful face. Fillers have always freaked me out 😬 I know they have come a long way but still…

Madonna’s face has been her fortune & this is true for all female performers in the limelight Facelifts can produce fantastic results but there is a limit to the number of procedures before everything goes south & one looks worse than if none had been done but that’s the price of celebrity & wanting to look good & arguably worth it if your livelihood depends on your looks
You should do a whole video about the inflammation and scar tissue caused by fillers because I think people would have second thoughts that they don't know they may need to be adding to the mix in their decision matrix! Wow

Exactly.. because after that injection. It must cause issues. The body isn't used to chemicals..and the nerves, muscles must be affected. Her face is swollen and the facial tissue..might be rejecting all that filler.
I'm 56 years old and I never had anything done to my skin. What I use are 3-4 different moisturizers morning and night. I apply them in an upward direction on my face, neck, and decollette area. I purchase them at WalMart and Amazon only spending no more than $35 - $50. This has worked well for me since the age of 50.

What’s really sad is that so many women (in particular) seem to be the most ageist in western society. As if an aging woman can’t be beautiful unless she looks like she’s less than 40. Of course ironically they are often making themselves look older.
Because of chronic medical issues, I was not supposed to get old. Now when I look at myself in the mirror i see an older person that I always wanted to see. I resemble my mother, my father, and my grandparents... I wouldn't have it any other way. 😁

Very helpful. I am disappointed that cosmetic doctors are still not telling patients that filler doesn’t really dissolve. It feels most unethical. I’m sticking with profilo and facial exercises

I believe the swelling will go down and she’ll look fine by the time she tours this summer. I for one don’t care for plastic surgery but M has earned the right to do whatever the hell she wants, as do all women, really. It’s her body, her choice. @@bereal6590 Helen Mirren is another actress, and one of the best at that, that has let herself age normally. Meryl Streep still looks amazingly young. I'm guessing she must have done something, but she's been very careful to not let it change her look. She doesn't seem to do botox either, and her face is still animated and expressive. There's nothing more unsettling than watching a person try and talk who is all botox frozen. It looks like their eyes are trapped in a frozen face. I have no idea why so many younger women start doing it.

There is NO pressure for people to look a certain way. what bullshit! Vanity is the pressure to look a certain way. And have the courage of your convictions. If you have surgery and its a fuck up, dont blame the fact that you are now an aging celebrity and blame AGEISM CRITICISM. No one forces anyone to have cosmetic surgery. Lots of people dont.

As he clearly states here, “fillers leave behind a footprint.” ( 10:04 ) She’s been getting work done since she was in her early thirties, it was just less evident than it is now. You have to redo your work roughly every 10 years. When you do, something else goes cray and you need to get THAT done. It’s the way of plastic surgery. The lesson here: don’t even start getting plastic surgery or, if you do, wait as long as you can to start because it’s not a “one and done” thing.

I’m so glad I didn’t take the advice of a plastic surgeon in my area who recommended cheek filler when I was 49 to lift my cheeks ! It seemed like bad advice and this reaffirms for me that I made a good choice. No question that your posts and videos are extremely informative and provide excellent guidance for prospective vertical restore patients 😊

Its not actually vanity per se. People always think this but the impact a sagging, unattractive face can have on confidence and mental health is profound and extremely distressing for many people (just ask any plastic surgeon.) I think vanity is more about putting yourself on a pedestal and thinking youre better than everyone else because of how you look, or ONLY caring about how you look. But vanity and confidence are totally different things-confidence is just liking yourself without having to compare yourself to others. And for many many people, this is linked to looks, no matter how much people want to deny it

Madonna is highly intelligent. It’s probably emotion (fear) about her aging appearance that is overriding her intelligence and motivated her to choose going this far with the filler. It’s human to allow feelings to supersede logic (common sense) when making decisions. Intelligent people do it all the time. She’s found someone to help her do it with this.
It's so sad. How can women not see that they look completely fake when they do that?? I'm asking an honest question.

No, its usually a very gradual thing and many times we are being told that fillers will lift and botox helps to remove the wrinkles but really, that's not what we really need to make us look how we feel (like in my 30s, that's how I feel)

Fat transfer is the most dangerous filler you can get, the one with the higher risk of disfiguration or asymmetric results and you can gain weight in differently in differ areas of the face as fat is not from the same body part. It's awful, fat cells are very delicate and better be left alone.
This video is so enlightening regarding face shape. I was worried that my weight loss surgery would make me look old (I mean, it is a common side effect) but I am just 33 and my weight loss so far has actually brought my jawline back out and I have an actual face shape again. I think I look younger now ^.^

Genetics is really more important than anything in my opinion. A lot of people don;t take that into account.

The thing is I don’t recognize Madonna anymore. She was very lucky being born with a beautiful face. Her fans are concerned. The hairstyle was very different as well.

I had some minor work several one years ago. I also have used Botox and Restalyn in the past. You have to be very specific with your doctor and take their advice. Many celebrities do too much too often because they don't want to spend even 5 minutes looking older. I've always believed for us regular folks, it should be on an "as needed" basis.

Im glad I didn't get hooked on fillers. I look pretty good for my age . Ive tried 3xs to add fat to my boney hands. My hands look like theyre 80. Ive worked so hard all my life sewing, cooking, cleaning gardening, painting ( houses) etc. My hands cannot be fixed to match my pretty good skin. If & when I get the courage Id go to Dr Karam. Your FACE is NOT THE PLACE TO SAVE $$$ CUT back on eating out, clothing , new car but when you've got to look your best dont cut corners on your FACE!

Thank you for such a sensitive discussion on the subject of aging, and why fillers have a really limited shelf life in that process. I find it mystifying that the average person, like me, can get such superb advice for free, and yet so many with unlimited resources end up with strange, unattractive outcomes. Frederique Bros, Utuber, recently underwent full face eyes lips neck lift with great results, and she begged anyone considering procedures to watch your videos before making a decision. I wonder how many plastic surgeons hear the words " but Dr. Karam says...." !!

This is the best way of explaining the basics of the influences of fillers, etc. For me this doctor explained what that person wanted, and how these fillers can be used in the wrong way. I think the more these procedures become more available people do need to understand the does and don't. This doctor is a natural lecturer on the subject.

I think limiting the effectiveness of fillers to just the 30s and 40s is misleading. Many people in their 50s look well and refreshed with a little strategic filler and botox. It all depends on genetics and how well someone has taken care of their skin. However, Dr. Karam makes an excellent point. There are some things that can only be fixed surgically, if someone wants a more youthful and rested look. Finding the right doctor is a huge challenge though. Getting a more rested look that will make you still recognizable isn't easy. I'd like to see a video on finding a good plastic surgeon no matter where you live. There are many in my area. The way I understood is that in your 30s and 40s filler and Botox can "fix" what you wanna fix. But later on they can help but alongside of face lift, not alone. Basically they won't make someone look any younger. And I have noticed that lately. Many people I know get botox and fillers and the lines are gone maybe but they really don't look any younger and not really any better and I'm not trying to be mean at all. And I'm not judging either, we all have different goals, I'm just thinking that these procedures like fillers/botox will not really solve what I would like to solve (which IS like he said, sagging in the lower face). I did them in the past and they helped but I don't think it's the solution anymore.

@@TheSalonLife Depending on the skill and experience of your dermatologist, and upon your level of sagging, some people get amazing results for jawline with Ulthera or Thermage. In addition, some neck aging signs like cords can be relaxed with Botox. I know. I've had them all and looked significantly better than my peers into my 60's. Restrained use of fillers in marionette lines have been aces. And Botox in my forehead, neck and around my eyes has continued to prevent deeper wrinkles from forming. I have reached the age where I will need a facelift to manage my jawline and neck and for a refresh. I'm 71. My picture, unfiltered, is online. I don't look like I'm in my 40's. But those treatments have helped me preserve some of what I had, without surgery. And I didn't have any of them until I was 50.

@@TheSalonLife My dermatologist is Elizabeth Tanzi, located in Chevy Chase, MD. She errs on the side of being conservative. So no freakish results. I highly recommend her. Until I can afford the downtime and bucks for surgery, I'll keep seeing her. Even now, some of those treatments are effective. But everyone is different. These are medical procedures. They are best performed by a doctor trained in dermatology of cosmetic surgery. Everyone gets different levels of benefit. But the odds of a beautiful result ate much better with a great doctor.
Agree! Great analysis, thank you. A very smart way to work. I love the fact that you keep the face looking natural. I live in Canada and I want a deep plane facelift. The surgeons I have met never explained to me that priority, "keeping our personal features" the way you explain it is brilliant. This is exactly what I want, and still be myself.

She has a great family and a wonderful life. I don’t think she would want you to feel sorry for her. Madonna is the queen of change just for the attention change brings. She’s marketing herself, still relevant at 60. Pretty awesome really, as she sails away on her private yacht to the Mediterranean.

Good info. Thanks, now I know why some people look so odd after years of filler and thank goodness I haven't let someone put filler in my face since I am 58 and do exactly what you said with lifting my face and saying wow. Now I know with my age and state of my skin fillers are not my answer. I think I will continue to take care of my skin and "age gracefully"
It's sad seeing so many beautiful women destroying their faces.
I believe one of the biggest problems is that these (mostly)women want to be the weight they were when in their twenties and thirties - which NEVER looks healthy as we age. This fanatical dieting to be sticklike, affects the aging face in quite a negative way - hence the haggard expression (besides that from simply aging) The fillers replace the 'baby fat' look to make the face look plump and youthful...so they THINK. It really just leaves them with a weird, 'lollipop' sort of shape. Keep an extra ten pounds or so as you age, and your face might be better off!
This is super informative. I was thinking it was going to be a total breakdown of Madonna’s face and kinda mean , but it wasn’t at all. Again, very informative, and thanks for the tips! I’ve done fillers a few times but very conservatively and now I’m thinking of a different approach. I’m at the end of my 40’s and it’s time!

I really like how honestly you explain everything and don't give people any false hope, Dr. Karam! An honest plastic surgeon - how refreshing!!! I just subscribed to your channel. I am 60 yrs old now and never had any fillers in the past, but I notice slight sagging starting to happen on my lower face, jaw area. Where are you located??? I wish you could perform a vertical restore and fat transfer on me and make me look they way I feel. I hope I get a reply.

I would have never imagined that Madonna would become yet another fading, insecure starlet that would go to such extremes desperately trying to look young again and of course to no avail. A 60-year-old is a 60-year-old BUT with the right surgery and the right doctor like yourself, anybody can look amazingly refreshed. How people with money that can afford the best end up like Madonna it's just a complete mistery to me.

My position has always been that I would rather have a full fledged face lift than ever use fillers. A great plastic surgeon, like Dr. Karem, is the only way to successfully reverse the signs of aging. Period.

I’ve been enjoying watching you and the information you’ve given regarding the vertical facelift. And it also explaining the different types of faceless that are out there. I have no idea to ask those questions. I also appreciate your sensitivity and professionalism regarding the way you addressed Madonna I also grow up with Madonna and appreciate her as an artist, but honestly that look at the Grammys scared me, it was actually frightening but you were very kind. I would love to have you do some work for me. I’m ready for some rejuvenation.
This was very helpful because I have thought about getting fillers. I had a little lip filler, but I wasn’t that impressed with it. Thank goodness it was a small about. My eyes looked tired when I was in my 20’s, and I have always been self conscious about it. I would be out in public, and even strangers, (grocery clerks, waiters, etc.) would say, “are you tired?” I’ve wanted blepharoplasty for years, but couldn’t afford it. Then I got my uppers done due to an eye appointment, where my eye doctor thought my eyesight may be impaired. My insurance paid for that. I’ll be 58, so definitely would love to get my old face back, but not to look like some of those celebrities. Thanks again for this video.

Madonna was always criticised about or almost accused of aging at least for the last 20 years even when she looked very good. So she could never be sure if she is aging good or bad I think. And this is the result.
