Yes, you should get the authorization using the imm5476e form.

If someone have only given name ...but in application it's optional so what i have to fill in surname which is mandatory.

You need to be authorized as a representative to apply for someone else’s visa (parents', siblings', in laws' etc.) using the published IRCC protocol. You get the authorization using the imm5476e form.

What should we upload in employment letter document? It says a copy of a letter from the employer in canada. This letter must state that the company will hire you to work in canada . So who’s letter should be uploadeed ?
