The Importance of Time in Treatment
This question is fundamental because the time it takes to start treatment often defines the final result, i.e. how much vision the person will recover. Retinal vein occlusion is a clogging of the blood vessels, similar to a thrombosis, which prevents the proper circulation of blood in the veins of the retina. This condition can manifest itself in two ways:
1. **When only one branch of the vein is affected.
2. **Central retinal vein occlusion: when the main vein draining all quadrants of the retina is affected.
In the case of central vein occlusion, the condition is generally more serious and can occur in an ischemic (more severe, with a lack of oxygen to the retina) or non-ischemic (with slow and inadequate blood circulation) form.
Consequences of Venous Occlusion
In non-ischemic cases, the main cause of low vision is macula edema, a swelling in the central part of the retina that makes it very difficult for the patient to see. In the past, there were no effective treatments for macular edema secondary to diabetic retinopathy, venous occlusion or other causes. Today, however, intravitreal antiangiogenic injections have revolutionized this scenario.
Treatment with Intravitreal Injections
Intravitreal injections are medications injected directly into the eye, through the white part, to treat macular edema. Studies show that, in the case of venous occlusion, several factors determine the visual prognosis, including:
1. **Whether the occlusion is ischemic or not.
2. *Initial Visual Acuity:* How much vision the patient has lost at the onset of the condition.
3. *Time to Start of Treatment:* How long it has been since the start of treatment.
Why should treatment be started immediately?
Treatment with intravitreal injections should be started as soon as possible. As soon as the diagnosis is confirmed and the treatment plan is defined, it is crucial to start the injections immediately. Studies indicate that the sooner treatment is started, the better the final visual results for the patient.
