How Frozen shoulder looks on MRI

In this week's video I show you how a frozen shoulder, a painful shoulder syndrome with reduced range of motion, can look on MRI and which findings are consistent with the clinical diagnosis of a frozen shoulder. Frozen shoulder is also known as adhesive caspulitis. Both terms were introduced way before MRI (frozen shoulder 1934 by Codman, adhesive capsulitis 1945 by Neviaser). It is important to realize that Frozen Shoulder is a clinical diagnosis and not an imaging diagnosis.

Great summary of these findings! It's always interesting to learn the history of nomenclature for these things and how not-so-accurate clinical descriptors which arose before some of the more modern imaging and pathology findings tend to persist. I appreciate your resistance to the Latin terminology (periarthritis humeroscapularis) which is quite a mouthful and probably unnecessary in most situations.

Hi doc, I am a huge fan of yours. I love your work. I have a few questions for you. i see a lot of patients with a frozen shoulder. I have heard that GHJ capsule contracture is hard to identify at early stage unless it is severe. Is it true? As it is a clinical diagnosis and if I see someone with limited AROM and PROM of the affected shoulder and normal-looking images, is it more likely an idiopathic frozen shoulder? but we suspect that there may be a joint capsule tightness although it is not seen on images. ?? please let me know. Thanks

We can see signs of capsulitis on MRI also early on. Might be subtle and not specific. Ultimately it's a clinical diagnosis in my opinion. Direct arthrography can reveal reduced capsule volume, which can be helpful

@@DrChristophAgten thanks for your reply. 1. Can u explain what the reduced volume of capsule? What does it have to do with frozen shoulder? 2. How do u clinically diagnose a frozen shoulder apart from images?

Great video. Thank you! My shoulder has been killing me for the past two months and I suspect frozen shoulder. Just had an MRI this morning in Berlin. Do you perform MRI evaluations if a patient can provide you with the imaging performed somewhere else?

Dr. Idriss- I am suffering from hormonal acne in jaw lines since I stopped birth control pills and trying to get pregnant. So dr prescribed 15% Azeleic acid AM and PM for last 7 weeks- some improvements but not fully cleared. I just learned from your other videos to not avoid moisturizer and got Aveeno calm+restore. My skin now has lots of scars and dull and I wonder If I can introduce AHA serums?

I've never heard the term "indirect arthrogram". That's just referring to an IV contrast injection vs. an intra-articular contrast injection, correct?

You administer gado before the exam, then move the joint to facilitate the gado to "sweat" into the joint. Only then your go into the MR. So your have gado already injected on all sequences. Instead of only gado before the last t1fs sequence for example. It's not my favourite technique though. Probably controversial. It's been done also in the hip.
