How does a plate and screws help a broken bone heal?

Putting a broken bone back in place and holding it with plates and screws is one of the most common types of surgery we do as orthopedic surgeons. Why do we use plates and screws, though? In this Ask Dr. Geier column, I discuss that question from a reader concerned about what the hardware does to the bone.

Jorge asks:
I don’t understand how drilling your bone to put a plate and screws helps a fracture heal. Are you not messing it up more by doing this?
When we perform open reduction and internal fixation (the formal term for the surgery) of a fracture, we are essentially opening the skin to get down to the bone, putting it back in the proper position, and using hardware to keep it in that position. Yes, doing so requires drilling holes in the bone that the screws fill up. But we do that because often just putting the bone back in place won’t hold it in good position until your body heals the fracture.
In this video, I explain why orthopedic surgeons use metal plates and screws instead of other options as well as how a fracture actually heals in the first place. You will better understand why some fractures need surgery and if and when plates and screws are removed.
Please remember, while I appreciate your questions, I cannot and will not offer specific medical advice by email, online, on my show, or in the comments at the end of these posts. I broke my left Clavicle about four years ago and had surgery almost straight away due to it being out the skin, they didn't put a plate on it they put some kind of ring through it? I wasn't sure, anyway weeks later during physio therapy it snapped again.. I then got a titanium plate with screws put In, I found the plate to be very uncomfortable for the first couple of years and literally had to pull the skin over the edge of the plate sometimes.. (gross eh). I'm not lying in bed having broken my right collar bone now, I had surgery 7 days ago now, and they put a plate in. I am extremely nervous this time around and don't even want to sit up out of bed sometimes worrying some kind of movement will break it, breaking your arm is so much easier as you can be a lot more mobile. I did find this video has helped my understanding more and I shall subscribe to your channel! Cheers

Dr. David Geier My boyfriend shattered both of his legs in a crash over a year ago and he had a total of 23 screws and I believe two plates put in the heal his bones. The nurses told him it was a miracle the doctor didn’t just amputate his legs due to the crazy amount of damage. His right leg is nearly healed but his left leg where his shin is there is a space in the bone. Is there any reason for that and will that heal? And the screws on his right leg are on his foot and he wants to know if they can be taken out to alleviate his pain of walking on them

I’ve broke my ankle about 1 month and a half ago getting hit by a car on the pedestrian crossing,I’ve had my surgery that contains 3 screws and one plate...and now I’ve just started walking in my walking boot, this being my second day.Unexpectedly breaking this 2 bones I’ve realised who are my real friends, what’s really going on in the hospitals and how to appreciate your life even more and who to let in or which to be kicked out.I can only say this sucks as this happened so young,I’m only 18,and at this point I should of been starting many stuff but I put them on pause, but I’m keeping positive and whoever is going through any kind of pain right now,it’s going to get better, never give up,trust the time and trust yourself because if you not going to get yourself a good recovery,no one will do it for you.ITS GOING TO BE FINE,YOUR SCARS ARE SHOWING HOW STRONG YOU ARE,NEVER BE ASHMED OF THEM.And if you can,try hesitate it as much as you can.Stay strong.

On May 5th this year I had a total ankle dislocation with a trimalleolar break including my fibula and tibula. I missed a step on the deck! The surgical team initially reduced it and 5 days later I had surgery. I have 2 plates and a lot of screws. They did an excellent job so thanks to our NHS. I wore a hard cast for 10 days and then an aircast boot for 7 weeks which I took off when sleeping. I now attend physio once a week and I am just learning to walk with one crutch which will hopefully progress to none by mid August. It so important to do exercises with your foot even when resting. Taking painkillers is also important because it hurts!! I was always active but I'm older and I'm realistic about recovery. I hope this time next year I will have good functional use of my ankle and will be able to enjoy walking and cycling again.

Your explanation of why plates and screws are used has calmed my apprehension. Just the fact that you say this hardware in my leg does not mean I will suffer with on going pain is reassuring. Oh my gosh, I just ran across this video as I had a TPF and had one plate, six screws and a cadaver bone graft. This happened in Nov 2021, falling off a ladder, I was 68 yrs then. I was nearly 12 weeks NWB and was in PT for 5 months. It was a very long, emotional, painful injury. There is not enough on the internet about this horrific injury. I am blessed to be able to walk again thanks to my ortho surgeon Dr. Pomponi. I am still limited to certain things, like kneeling and squatting. The doctor says he'll remove the hardware if I choose to. Right now, I'm pretty much pain free...except when I bump the little piece of the plate protruding. He said eventually I'll need a TKR but probably not for some years yet. My quads and glutes never did come back 100%. For anyone reading this who has or had a TPF, there are a few support groups on FB to help get through this. Anyway, just wanted to mention my situation and thank you and all Ortho surgeons for giving us back our life❤

Just found out 6 weeks later at Fracture Clinic that they had done a bone graft and removed all metal. However, I was so shocked to see new X-ray which showed much bigger plate and more screws in ankle. Nobody told me and Inhave been so stressed at seeing all this metal in my ankle for a second time! I’m normally a fit person attending my gym classes 3 times a week for past 10 years! I feel Ill never get back to how I was - all because I slipped whilst walking my dogs back on Good Friday!

Eat lots of veggies drink lots of water. Fight anguish pain frustration or boredom try to keep ur mind occupied reading writing or learning a new hobby. Shift from bed to sofa if U can all day.but most of all rest well and be patient.

I broke my right ankle five years ago very badly both bones so I got to do the surgery with plates and screws. So to those who might afraid of it, it will be all fine and helpful. For me it was not possible to have the ankle back to normal again without the surgery. The only part that still bothered me while walking after the surgery and physiotherapy were screws on the left side which I could feel them from my skin, but after removing them (9 month later) no more problem.

I had a trimalleolar fracture 3 weeks ago from falling while ice skating. I had surgery to stabilize the bones and a plate with 12 screws and pins were placed. I'm in a hard cast now for the next month. Getting a lot of numbing, shocking, lancing, burning sensations on the side with the plate and the top of the foot. Not sure if it's the plate itself pressing against the bone and cast or the surgical incisions pressing on the cast itself along with cutaneous nerve damage. It's difficult to find a comfortable position to sleep..so just trying to think positive. How is your healing process going??

After I was cleared I started use a resistance band to break up the scar tissue so I could have full mobility again. It was hard to go through that but I made it and I know you can too.

I got an ankle fracture due to fall from our stairs 2 months ago and I had undergone surgery last month. It took 2-3 weeks before I got my surgery to wait to reduce the swelling. I got a spiral fracture on my Lateral Right complete oblique displaced lateral malleolus and it was fixed with screws and plates, I also got my medial portion opened as well to get my ruptured deltoid repaired and also got screws to further support my medial bone. I got an excellent Surgeon.☺I'm on my 4th week post surgery and just got my cast removed last Monday and my doctor advised me that I could practice walking and stepping my toes of course with the support of crutches. I'm on Calcium (with VitD) Medication as well for stronger bones... and also, on my 3rd week post surgery, I was advised as well to practice massaging/rehab my foot inward outward motion only so my muscles stretch little by little. I also got to step my foot on the ground at a 90 degree (by sitting of course..) That's where I am on my recovery stage for now. 😊

I've fractured both wrists and had plates and screws implanted. A little over a month ago I ruptured a tendon in my thumb. When surgery was done to repair the tendon the hardware from the wrist fracture was removed. My orthopedist is really good and spent time talking to me about it, but I was shocked at first when he mentioned the possibility of removing the plate and screws because I truly didn't understand that they were there to support healing. It just never occured to me that they could be removed. I never had a problem with my plates/screws and had no idea that occasionally the hardware can contribute to tendon wear.

Great explanation. I managed to slip on a slope at home whilst feeding Kangeroos at home. In hindsight, perhaps wearing flipflops wasn't the best footwear to wear on a 40° slope covered with dead grass and I twisted my foot as I slipped. I ended up with a spiral fracture in my Fibia and two small fractures in my Tibia and add to that I got a syndesmosis injury too. The Tibia has basically been left to heal as they were only small, however the Fibia had open reduction and a plate with 12 screws holding it all together. The surgeon was going to do a "tightrope" but decided on two long screws to hold it all together. (Don't know why he changed his mind) Two weeks later it doesn't feel too bad, however I am sick of the moon boot I have to wear. I have been told non-weight baring until Christmas, however I have started to wiggle my foot to keep the blood flowing and I have ordered some resistance bands to get some strength back very gently. I wonder if I will bee at the airport metal detectors?

I appreciate your video and the simple explanation with multiple x-ray images to show examples. My story is this: I broke my left 5th metatarsal midshaft on 7/24/17 when I landed on the front left side of my foot in an adult beginner jazz class. "Acute minimally displaced oblique diaphyseal fracture of the fifth metatarsal." I was given compression bandages and the orthopedic shoe and told to limit activity to low impact. On subsequent visit 8/10/17 I was told that the fracture had displaced somewhat since the first visit, but there were no means to correct it (e.g., physical therapy), other than surgery to install a plate and screws. I was given the option to have the surgery now, or allow the bone to continue to mend and see how much it bothers me down the road, with surgery remaining an option for the future. I opted against surgery at this time and got the air cast and a knee cart. I will return on 8/31/17 to see what progress I have made.

I think even complete bone fractures could be externally glued together using the same resin dentists use to rebuild teeth. The amount of force placed on rebuilt teeth is enormous and I think the same resin could stabilize bone long enough to heal. You couldn't apply it inside bones because you'd be inviting infection - but it might work externally. This is something that could be experimented upon in a lab.

I do have considerable stiffness in my ankle that ive begun to work through with resistance bands but otherwise everything feels fine. Perhaps the biggest challenge I've faced so far is anxiety from wondering if everything will be "right" at some point and I can do all the things I did before the accident. I had a very bad motorcycle accident that resulted in multiple broken bones in my shoulder. This was in 2006 and required 6 plates and 20 screws to put me all together again. They were supposed to be permanent but after a year they were unscrewing and causing more damage. Most of my metal was removed at that point with the last plate and 5 screws removed 5 years later when I had my total shoulder replacement. The pain that the metal caused was a deep throbbing that became worse with changes in the weather, like arthritis. When the last of the metal came out of my collarbone, the relief of pain was incredible. Now I have broken my ankle pretty badly and the surgeon put in a titanium plate and 6 screws. I can already feel the deep pain of the metal. The surgical pain is one thing but this pain is different. I am thinking I may need the metal removed in a couple of years even if it doesn't try to remove itself. Do others experience this or am I just metal sensitive?

I also had a trimalleolar ankle fracture and I fractured my tibia, fibula and talus. I needed 13 screws and a metal plate in total. I am 4 weeks post-op, still in a cast and I still have 4 weeks to go before I can start physiotherapy. Even though I have no pains I am very worried if I will manage to walk properly again and if I will have long-term pains…My surgeon is very optimistic though so I try to stay positive!

I have had Trimalleolar comminuted ankle fracture 3 years ago. Silly fall. two large screws In distal tibia and a plate with 6 small screws on lateral distal fibula. Took 6 months. Lots of rehab This past fall 2020 fractures of radius and ulna mid shaft fall cleaning up after hurricane Zeta. First surgery the screws pulled out of plates into bone in two weeks. Bones separated b Second surgery much longer plates in radius / ulna. : 6 + inch plate in radius and 4.5 inch plate in ulna. 20 screws to hold the plates into bones. Second surgery successful. A very painful experience. At 74 years old. But I am healing and doing some at home stretching and strengthening exercises.

I broke my leg. Specifically my knee cap or meniscus. It got essentially pulverized or shattered. The doctor put a metal plate along my tibia with around like 7 screws drilled in to the tibia. I have a daily crackle and I can’t run or do much hard impact exercises like jogging but it beats the hell out of having no leg. I have had pelvic or hip misalignment since. He did tell me you won’t be back to normal ever but it was a reconstructive surgery to help me live a better life. I was 27 when I broke it. Now I am 32 and have to take painkillers to get by the day. Dr. Tenny saved my leg and I would have not had it any other way. I just have to keep on pushing and keep my weight at a normal level to avoid more stress on it. I just wish I had Insurance when it happened so that I could have afforded physical therapy after the surgery. Love your informative video explaining the reason doctors use plates and screws. It just made me more at ease after years of thinking it was a mistake.

I think , if I have the bone names correct , a corner of the top of the tibular broke away and something else pinged off , I don't know. My husband said that basically the bottom of the leg detached itself at the knee. I have a plate and screws. Sometimes it aches at but still , like a bruising but on the whole I healed well. It's still numb on one side and the knee in general & calf is fatter than my other leg by an inch but I think this what to expect to be left with.

Around two years ago, I was in a very serious accident and have had multiple surgeries since then with the insertion of plates and screws. It has been two years now and I am still having difficulty in all areas to really manage the pain that plates and screws have around the muscles and ligaments within my body. If there is a way to manage that pain with using full range of motion; fore arm (radius and ulna close fracture) and right leg (femur, hip and knee joint) that would help change everything. I have tried and tried having appointments with my surgeon, hand therapists, pain management people and councillors to be able to handle the pain that it causes. What else would you recommend? Cheers
