Hip Bursitis, Piriformis Syndrome, Sciatica, Herniated Disc, Or Something Else?

Many people with pain in the buttock, hip, and leg are told they have hip bursitis.
But, many people who are told they have “hip bursitis” feel pain in areas other than the greater teochanteric bursa. It’s not just tender right behind the trochanter/out hip, they also have low back discomfort, a history of herniated discs, SI Joint pains, pain in the center of the buttock (Piriformis syndrome?) and even pain, numbness, or tingling that shoots down their leg (Sciatica?).
In this video I chat briefly about what Hip Bursitis IS and what it is NOT.
True Hip bursitis is pretty clear cut. Pain and tenderness (makes you “jump” when you press on it "jump sign") to the outer hip just behind the greater trochanter (point of the femur bone). Hence why the official medical diagnosis is greater trochanteric bursitis. It will be bothered by positions of pressure or compression- sitting with legs crossed, standing with hip dropped/weight shifted, running, prolonged walking, or lying on that side. That’s it. No central butt pain, no piriformis pain, no numbness tingling going down the leg.
The presence of anything else makes the diagnosis highly suspicious. Pain in the back, history of sciatica in that side, central buttock pain/Piriformis Syndrome, groin pain, SI joint area pain, numbness or tingling that goes down the leg, or “hip bursitis” pain on both sides suggests something else is going on.
It’s not uncommon to be told you have hip bursitis as well as Piriformis Syndrome, Sciatica, or a herniated disc. Basically, you’re diagnosed with everything because no know can understand what’s actually going on. So they give a catch all explanation and list of diagnosis for your condition.
The back can refer pain (send pain signals) To the outer portion of the hip.
The sciatic nerve travels just behind the greater trochanter end near the bursa… If it is inflamed it can give you what feels like hip her bursitis.
The piriformis muscle attaches directly to the greater trochanter- right near the area of the bursa.
So, keep in mind that while hip bursitis is fairly easy to diagnose alone- Many other problems - including sciatica, herniated discs, SI joint problems, piriformis syndrome, and gluteal tendinopathy can cause feelings of “hip bursitis” and pain the area.
So, rather than relying on a specific diagnosis- your best bet is to establish a movement pain relationship.
Meaning, if you have a specific movement or position that reproduces your outer hip pain -study that movement pain relationship-and figure out how to break it up.

    Hi there , I got right sided bursitis about a year ago , had injection and was doing well . Christmas morning I hurt my back , CT scan showed herniated disc . The pain was off the scale with sciatica , doctor gave me Lyrica which was a blessing , specialist suggested surgery . I couldn’t tell if the pain I still have when standing is from the hip or back . I don’t want to do surgery if it’s the hip causing the sciatica. The pain is in the butt and down to the knee . And maybe the lyrics is stopping the sciatic pain that goes to my toes. Your video has helped immensely. Thank you ☺️

    I think you're forgetting about co-morbidity. As far as I understand it, tight piriformis can cause bursitis. I personally believe I have both. Started with bursitis-like pain (alone), then migrated to butt pain.

    Same thing here. I had an acute injury to my adductor. I believe that "caused" the hip bursitis because I started walking in a different way.

    My pain is worse after figure 4 stretch. Often in the glutes, but also in iliac crest in the back. Knees sometimes hurt. Trochanter area hurts. Overall very stiff in hips and back. I got into trouble by sitting too much, lunges, squats, stair climbing etc. One day i cleaned under the furniture, worked out very hard, inverted the water jug, and now im suffering ever since. I'd do anything to start to feel better.

    What would deform your hip it's sticking freakishly out and locks up at night and causes extreme pain running up my back and down my leg, hope it's not out of joint.

    My painful side often switches and sometimes it's both sides. Right now I feel like there's a knife in my left upper/outter glute especially if I extend my leg in front. Usually most painful in morning even after spending thousands on a good bed. I don't think stretching works too much. I had FAI surgery on my opposite side 5 years prior.

    Hey, awesome explanation, different from all doctors in Brazil, and if you are equal(both sides) of flexibility, which exercise should someone do? Strech or tighten?

    Hey Gabi! Thanks for reaching out...if both are equal and if you have one sided pain, then it tells me that motion is most likely not the issue and that you should NOT be stretching. Again, I havent evaluated you, but if flexibility is good and equal then head down the path of strength first. (tighten) Make sense? What are you currently doing? Dr C

    Thank you Dr Charlie! I have L 3,4,5 bulging. Also have bilateral hip bursitis. All I’ve heard is how to test for it and what NOT TO DO! So what should we do to relieve the pain? Including exercise. Pls. So frustrating.

    When I do toe touches to stretch, particularly on my achey side, I get some relief. I do this periodically throughout the day.

    Good question Richard… many things to consider and I wish it was a straight forward answer. First- optimize the healing environment: Do detective work and ID things that are aggravating problem structurally, and also consider how certain thoughts/beliefs (mentally) might be contributing. Fix these things. Next, find what you need to improve (self assess). 3. Use mind and movement as medicine: Follow a process for identifying specifically which motion(s) will be most valuable for your condition… give you relief. finally 4: Get back to life: Learn how to manage the ups and downs- and build a “bridge” which allows you to get back to life safely. I teach folks worldwide how to do these things inside my Glute Relief Accelerator Program. Hope this helps.     So what if your hip hurts, your lower back hurts and it’s all on one side , but it’s not numbness or tingly and it’s not all the way down the leg to the foot. It’s a stiff feeling pain that sometimes feels like it starts in the lower back above the left buttock and other times feels like it starts in the side of the hip. It’s mostly a dull pain, but it gets sharper at night and when sitting , bending or stretching . The figure 4 stretch test made it feel like sharp pain and it stretched fine. It wasn’t even tighter than the other side it just hurt sharply to stretch. So is that sciatica or bursitis or something else ?

    I cant even do those tests, tight either way and then it hurts in the front top middle of my tendon or muscle of my hip.

    I have chronic hip bursitis in both hips. In 2020, is did several weeks of Physical therapy and at the end, I did get some relief from pain. But since then, I have been doing some of the stretches and exercises I was given plus yoga stretches. I have had the shots in both hips. They no longer give relief. I have trouble sleeping at night because I am a side sleeper. I now take an Advil at night so I can sleep until it wears off. If not stretches or exercise, what is recommended?

    I am having the same issues. Pain in the buttocks and legs more in my upper thigh. Did you find a solution? I am currently taking a muscle relaxant, doing soft tissue massage and acupuncture.

    My doctor said i have ischael bursitis but i have back pain and tingling, but you are saying that i shouldn't have back pain and tingling! I'm so confused

    Hey! Because the bursa is not a nerve- it cannot cause nubmness.tingling... and will NOT refer pain up to spine.     On that fig 4 test, both legs have same free movement. On my sore side, I’m unable to cross my leg without pulling it up with my hands. In walking, the pain is in the quad muscle just above the kneecap and sometimes the pain travels down to the outside part of my calf. Thanks for your tips in these videos.

    Hey Katie! Yes, you can- as you are potentially losing or compressing the outside of the hip/bursa area. But pain is localized to bursa region and does NOT refer elsewhere. Make sense?

I'm trying to figure out if pain is from the bursa or labrum... this hip stuff is complex! Im doing some glut bridges and wall sits right now...I used to do ballet and barr.. this is what I get for stopping! Thanks agaun

    Hey Hazel! good question... hard to give a blanket statement for this... but, in general, one that does NOT compress or irritate the bursa (think stretches, massage, etc)... What type of treatment have you tried?

I've just been diagnosed with this by my GP and he suggested I go on KZread to find out about what exercises, but I found them confusing until I found yours.     Step 1- calm muddy waters and stop poking the angry bear. Step 2- learn creative ways to modify/tweak things that aggravate it.     Hi YES! I found out that I was over stretching and instead of stretching I needed to tighten. How you do it is: lay on your side with your knees bent. Keep your ankles touching and lift top knee up and repeat on both sides. My pain went away in two night and I felt relief in just one night. I had pain for a yea. If that doesn’t make sense look up how to tighten it. (Unless yours is too tight then you need to stretch like this video) BEWARE OVERSTRETCHING. ❤️

    Also putting one hand on the hip while doing this stretch can help you make sure your doing it right. It should burn.

    I need help I've been trying to get help in my town my hips legs back everything hurts and is inflamed I can't feel anything from time to time , can hardly walk and I'm numb in my butt right hip thigh nd foot everything changes from time to time I wake up ok sometimes I take medicine and get worst idkkkkkk I need help so bad and evrrytime I call the hospital they drug me up and send me home. I can hardly sit and I want to cry all the time this is making me depressed I've had all this for almost 2 weeks now . It's hard to shower . I haven't been outside . I need someone..... please .... the way all this started was my hip was tight and for almost a month then I stretched and my hip cracked supppperrrr crazy very loudly then right after that I had to go to the hospital and they said I have a herniated disk but so much has happened. I have a knot in my the middle of my chest now . I can't smoke. Sometimes I can't breath , my hip and pelvis feels inflamed, my groin area went numb at least 3 times . I had temporary paralysis at least 4 times I been to the hospital 4 times..... ughhhh....... please someone!

    As far as i know, if you dont have back hip its probably piriforms( like my case pain just in the butt), and bursites you should analyze by this video

    "diagnostically muddy". 🤣 Sciatica is the headache of the hips....they just slap on the sciatica diagnosis (like taking Motrin for headache) . Sure it can help, but until you get the root (stress/sinus allergies/brain tumor) only symptoms get (short term) relief.
