High-Yield Emergency Medicine Shelf/Step 2 CK Review

In this video, we cover a high-yield review of emergency medicine for the shelf exams and Step 2 CK. The first section starts with HY practice questions, followed by important differential diagnoses, and then conclude with some rapid review concepts. Keep in mind differential diagnoses are extremely high yield in emergency medicine and are often very broad.

    I didn’t chose A in Q1 becos I believe troponin would not show up in the first 2 hours in an MI or ACS.

    That's a reasonable thought, it is pretty early in the timeline for troponin to be significantly elevated. Because it is such a highly sensitive marker though, it can be useful early on in management to see what the "baseline" value is and see if that changes over time. So even if it was normal or only mildly elevated, being able to trend it over the next several hours will be very useful in management. Great point though!

    Do you know of a resource that has the diagnostic and management algorithms/steps for these cases? I can identify most of these things but need more concise next best step in management or treatment in each case or scenario. Thanks

    Case Files - Internal Medicine fourth edition by Toy and Patlan is what a lot of us are using. Maybe not specifically for emergency medicine but it has a lot of examples that you're describing. Maybe you can buy it but I know people just airdropped PDFs of it so it's probably around

    It will depend on the clinical context they give you! But correct, with a presentation like this, you can assume if the troponin is high, they want you to understand that you're trying to prevent further myocardial injury at that point. Great question!

    These videos would benefit from a machine gun style information dump. First 1/3 of the video speaks only on classic ACS, ankle injuries and just starts speaking about fevers. As someone not looking for an introduction, and more so looking for a fast review, this didn’t assist me much.

    Have to agree with this comment. Didn’t seem quite at the med student level. We know chest pain differentials. Dr. High yield does a great job but didn’t have an EM review. Would recommend looking at those.
