    Great video sir. Our hamilton trainer told us to always do the tightness test first to check for leak, then you would do the flow sensor after. That's to ensure that the flow sensor is not calibrated with a leak in the system. He even demonstrated by creating a small leak and running the flow sensor test and it passed, then he followed it up with a tightness test like you did and tightness failed. So it could be very possible to pass a flow sensor test and then fail a tightness test. Hopefully the clinician will redo the flow sensor test after the leak is found and the tightness test is redone. Thanks for all you do.

    Hello George, I am facing tightness test fail issue, what could be the problem, even I put the whole circuit tightly on all junction's. Please suggest

    Hi: If your using a humidifier, bypass it and hook the circuit up without it. If the tightness test still fails then the issue is probably not with the humidifier and is somewhere in the circuit. If the tightness test passes then its your humidifier. Depending on the type flow sensor used, the older style may have separated a bit in handling it so maybe replace it and its tubing. Ensure those tubing for the sensors are tight where they attach to the vent. Another potential site for trouble is the exhalation valve. Remove it and ensure that its put back together properly and that its snug/tight. Check all connections including the MDI port and temp ports. Check the port close to the humidifier where you place the temp sensor. They can sometimes look like they're in all the way when they aren't and leaks and auto triggering can occur because of it. I've also encountered problems with the circuits themselves. Occasionally you may get a new circuit that was damaged in handling before you put it on the vent. If you try all that and still have a leak, grab some cheap large bore tubing and connect it to a patient Y connector......you'd be creating a basic generic patient circuit. Use that with the flow sensor connector and leak test it. if you still have a leak then it may be internal with your inspiratory valve. Let me know what happens.

    it'll never happen, especially in the post covid 19 era. but i do know of some hospitals where the RT is no longer doing regular floor therapy, or doing regular treatments in ER. They are transitioning them only in critical care units, or intubations and ventilator management in ER.
