If I remember correctly, I believe guard trails are not suppose to have all the bolts installed. There should be some type of "break" to lesson the impact. Without that break, the guard rail becomes more dangerous from the rapid stop of vehicles do to inertia. I'm sure an installation video could solve this unexplained problem.

They should be carraige bolts, mounted in the same direction. The rounded head of the carriage bolt is safer, with no sharp edges. Just look up images of highway guardrails.
