You need to revise this video about insulin receptors and diabetes. I would draw a small artery in red, small vein in blue, and a met arteriole that connects the two. Then show a capillary bed that branches off of the met arteriole. Insulin binds with an insulin receptor which causes the cell to signal the release of glucose transporters from cytoplasmic vesicles. Glucose transporter type 4 in skeletal muscle is the most numerous. The GLUTs and not the Insulin receptor allow glucose.

Don't forget that the brain, red blood cells and exercising muscle don't need insulin to internalize glucose. Therefore in patients with Type 2 DM, exercise can be a very effective took to manage blood glucose levels. Of course it isn't perfect, but if the name of the game is to control blood glucose (so that it doesn't damage end-arteries, for instance), then every little bit helps. Great video though, and nicely illustrates the difference between to two type of diabetes mellitus. Note also that there is a "diabetes insipidus" that has nothing really to do with blood glucose.

It's actually not at the level of the receptors but at the downstream signaling pathways that the defect in insulin sensitivity in type 2 diabetes is located... Although patients with inherited mutations in the insulin receptor gene that lead to reduced levels of the insulin receptor or with defects in the insulin receptor do exists (Rabson-Mendenhall syndrome and Donohue syndrome).

It is important to revise the video to show the capillary beds which provide nutrients and take away waste products from many cells. The disease process of diabetes and the damage to capillaries is very important to understand. With diabetes and high levels of sugar in the blood stream, sugar non enzymatically ( not in the body's recipe book ) binds to the inside of the blood vessels. Mature red blood cells have lost most of their organelles but must fold to pass through the capillary beds

You might also mention that the pancreas is really two organs. Exocrine ( digestive enzymes released into the duodenum ) and endocrine ( Islet of Langerhans cells Alpha, Beta, Delta, PP, Epsilon that secrete glucagon, insulin, amylin 2, somatostatin, etc ). The Beta Langerhan cells contain GLUT 2 that sense the level of sugar and lead to the release of insulin.

You may need to consult a 1. cell biology text 2. biochemistry review text. Younger students will probably see a side note in their anatomy text that discusses diabetes. Diabetes and obesity are a major cause of health problems in the United States. It would be very beneficial for young people to understand a better model of the disease. I often use an analogy of tree branches and leaves to explain capillary beds and also lung bifurcations for COPD.

It increases vascular osmolarity, for example. This promotes osmotic flow (of water) from the tissues to the veins/arteries. Of course, this would end up lowering vascular osmolarity but increasing tissues', leading to a equilibrium. The thing is that this equilibrium is leaving tissue cells dehydrated. That's not cool. There may be more reasons, of course; but that's the one I can think about right now.

Glucose molecules are “big and clunky.” When they get pushed through vessels and capillaries that are 1 cell thick, the molecules tend to shred the really small vessels, causing diabetic neuropathy. This is also linked to damage in the feet, hands, eyes, gums, etc. That’s how it was explained to me, anyway, so take it with a grain of salt. ¯\_(ツ)_/¯

This is the complete opposite of what macrophage is saying. What macrophage is saying, Glucose enters the cells "to be stored" and not to be used as energy. When the body needs energy, cells release the stored energy back into the bloodstream.

  • @Pen2AndyP

Reducing the amount of carbohydrates you eat can reduce the amount of insulin your body will produce, effectively helping the body become more sensitized to insulin again. It's a much better solution than to inject more insulin to your body, which would just make your body more greedy for it in the long run, but of course, this would mean a high-fat, protein and low carb diet. It'd make us have less glucose in our body overall, especially since all carbs turn to glucose starting in the mouth.

Cells need a specific amount of glucose to to gain ATP (energy currency of the cells). If they don't get that, their metabolic properties and respiration (oxygen) becomes inefficient causing ischemia, which can cause a heart attack (angine pectoris). So if you don't wanna risk a heart attack better take insulin. If you have hypertension ACE inhibitors might help with both conditions (diabetes and hypertension).
