Fixing Big Hernias: Component Separation
This video review the basics of abdominal wall reconstruction and component separation. We review the basics of abdominal wall anatomy, then dive into the few key pieces that most learners are missing that prevent them from understanding how to perform component separations. We then discuss the fundamental problems related to large hernias, and how component separation techniques have been developed to address these issues (large fascial defects, weak tissue, risks of intraperitoneal mesh). Finally, we provide a basic description of the techniques involved in posterior/transversus abdominis releases and anterior/external oblique releases with helpful diagrams. We discuss the relative advantages of TAR vs EOR.
