    I like the audio of the split S2 for a fixed split but it would be nice to hear the normal, wide, and paradoxical splits as well rather than just a description of them.

    Decrease in the sense that the intrathoracic pressure becomes more negative. Inspiration -> diaphragm contraction -> the thoracic cavity expands -> thoracic volume increases, thus pressure decreases -> lungs, right atrium and his vessels expand -> more blood in right atrium -> more time for the right ventricule to expel all of the blood -> split S2.     Hi thanks for this great vid! Is there any way of clinically differentiating between fixed widely split S2 (like in ASD) and a paradoxical split S2 (PDA)? Is there any difference in their sounds?

    Hi, I know it's been about 6 months but I'm gonna try to answer the question anyway cause I wanna test out what I've learnt :D So, one way that I know to differentiate these two is based on their quality during both inspiration and expiration. Fixed splitting will produce a split in S2 regardless of inspiration or expiration. It doesn't matter what the patient is doing. That's why it's called "fixed". In the case of paradoxical splitting, there will be a split in S2 during expiration but not inspiration. That's why it's "paradoxical" since normally, you'll only hear the split during inspiration but not expiration.

    @@LorianandLothric Fixed split doesnt change with respiration. Normal physiologic split, P2 comes after A2 on inspiration. Paradoxical split, A2 comes after P2 on expiration and inspiration they are closer together, usually this is due to aortic stenosis which causes the aortic valve to close later than usual

    Taking in a deep breath and filling the lungs with more air than you would normally with a regular inspiration or regular breathing.

    S3 has more of a groove. You can almost dance along to it. S3 has soul, a little funk. A split S2 on the other hand, is more techno.

    @@ayshasarmad1240 S4 happens shortly before S1 and waaaaaay after S3, so you won't mix it up with S3 or split. If this split is lub-dudub, then S4 is lulub-dub.
