Hi Michelle! Great to see you this week! Thanks for the update on your progress! Getting off the cane is a "catch 22". "Quality walking" might require a little support from an AD. So I say, keep working on normalizing your walking. I hope that helps!

  • @FJRMarty

"...quality of movement...no, just keep walking..." So what 'your' profession as a whole is recommending, and in fact what I experienced, is to learn to walk poorly, and do it over and over again until you get really good at walking poorly. Ran into this with my old therapy group, both arm and leg have a flexoral synergy patterns. They worked on my strength, not my technique, and long after being dismissed for 'plateauing' I still struggle with my gait - lead with my toe, cannot get a heel strike. Arm is still not much good either. So, Dr. Tara, I must continue to watch your videos, it's the only way I can help myself improve. 😁

Unfortunately you are experience the "implementation" of this new practice guideline. I am hearing similar experiences from so many people and I will admit it is extremely disheartening. But I will do my best to keep educating giving you the tools to practice "quality movement".

Well those statistics are a load of crap 100% agree! I am so disassociated from how my muscles should work after a brain stem tumour ( benign and operated on 4 weeks ago) so I was told to do normal things. Well I have just tried walking to recalibrate my balance , and my brain has reverted to the balance issues before I had surgery as it is what it remembers. Definitely in need of some specialised neurological rehab because just walking does not work. I am in Australia I wish you were here to help me! I love your videos. I spend all day trying to understand things. So please done ever stop explaining the why and how everything works ! Thnkyou so much for your expertise delivered to us who are struggling at home alone to make our bodies work again!

Tara you're asking a lot I had a stroke in 2018 unable to walk etc etc just now beginning to recover using a cane the idea is it's hard to comprehend some of the complicated movements that are advocated so just saying take a walk it's a simple way of understanding with aphasia which keeps your body moving all you have to think about is move the juryy is still out and I get what you the chief difficulty is that when your helpers are there doing their best to support you are encouraging you to keep walking thinking they're doing it what's good for you so it's hard to tell them to forget about it very hard

thank you once again please keep up the super videos. and I am super pleased to see in your latest uploads you have endorsed my belief in intermittent fasting. I have been trying to get endorsement on this from my rehab doctor and neurologist but they have been reticent and I have been frustrated. my argument has been that if I loose weight and get back to somewhere near my comfortable weight I used to be before my stroke. then everything will be easier and mobility and walking will improve as well but no one has tackled the problem of how to loose weight when ones activity level has dropped dramatically. intermittent fasting has been good for me I am able to drop weight but maintain good nutrition and make sure I get a proper balanced diet with all the nutrients and vitamins my doctors have been worried that a traditional diet would restrict. and yes I find the 18/6 diet works best as it is very easy to fit into normal daily routine and therefore keeping it up is easy the 18 hour fast gives you a decent amount of time in the fat burning and autophagy zone so the benefits really kick in. if I go longer lets say over20 hours and reach a state of fasted ketosis I have found that I am much better at working at my computer when previously my concentration levels had dropped and I had experienced brain fog while in ketosis I am much sharper with no brain fog or confusion I guess my brain likes to run on acetone not glucose I have commented before with you and you helped me with my 'Soas' flexibility issues. but this communication is mainly about saying thank you and please keep it up you are touching and helping a great deal of people such as I and you deserve all the thanks and rewards due to you you are a beautiful soul.thanks again xx

I don't have such problems anymore, in the earlier phase due to fatigue it happened to me, after climbing the stairs it would stop. I didn’t understand why, but the brain had obviously and subconsciously solved the problem. I realized it helps, and now I know why. Thanks for the explanation, have a nice day.

I have been rehabbing since June 2020 and making a lot of progress. But my walk isn't perfect yet. My wife is a physiotherapist assistant who work with stroke rehabilitation. Intensive rehab for 2 or 3 years and less acute now because unable to deal with acute strokes since my incident. She keep telling me the samething you always say. Quality movement over reps. If you keep repeating a compensatory patent. You will have harder time to get rid of it to have a normal walking patent again . She had watered me very early on in rehab that walking using a cane to make sure I kept proper walk was important. But what does she knows??? Well I have to use the cane now to regain a normal walk. So I am a lucky man but I need to listen more lol. Thank you for all your good work we are using soo many of your exercises and I just keep improving very well. Despite the Dr's prognosis or disability insurance opinion. I keep proving everyone wrong . They keep telling me that an advance cares paramedic cannot return to work in full duty after a bilateral stroke caused by a bilateral internal carotide dissection within few hrs. But like you said it before in multiple video. Mental part of brain injury rehabilitation is just as important. Fortunately for me I just never surrendered to the idea that I wasn't gonna return to work in something I have been doing for 20 years. The only thing I can't be sure of just yet is how much longer before I am ready to return.but I developed an amazing resiliency to stuff I have no control on like time. But if this is my worse issue. I am golden. Thanks and keep posting videos. Unbelievable how many peoples you help.

Dear Dr. Tara, 💜💜 Super video. I agree totally with your call for quality. 💜💜 I had this in the beginning and overcame it with your exercises from earlier videos like stepping over a small yoga block with left and right. My PT also let me walk a lot backwards, that trained a lot. Now I do muscle rebuildup through leg lifting in bed back, side, front, straight, bent each 10 times, twice a day. It is really hard but the walking practice gets so much easier now. So many greetings over the pond. We never met or spoke, but you are such an angel for my recovery 🙏🙏

I have worked on my recovery for six years; 5 days a week, 3 plus hours a day. It's a slow recovery.my hyperextension of knee is sideways perpendicular to the direction of travel.

I have neuromuscular condition that causes spastity (tightness) in the back of my legs from hamstring to ankles. This tightness that at times can be real bad where I do that step to which does make me very tired.

Hi doctor Tara.. I'd like here your suggestions about visual defects like peripheral vision loss or in my case it seems slow to come in mostly. I have only sight in my right eye and its the left side peripheral area that's my problem

Thanks for another helpful video! I love the exercises, but I have a question about the first one (sit backs in kneeling position). I have not been on my knees too much since the stroke, but I’ve noticed that when I am, it can be uncomfortable/painful in one/both knees. Is that just a matter of developing strength after non-use? Thanks!

Hi Randy! It is hard for me to say without doing an assessment. Sometimes pain is ok and sometimes it isn't.

Walking Improvement Walking forward requires swinging the affected leg and foot past the good foot. Walking backward forces the affected leg to carry the entire body weight through that same range of travel. A leg thus strengthened will swing forward easier as it moves only its weight when walking forward. Walking backward also aids balance. I use shopping cart racks and handicap ramps as parallel bars walking forward, backward, stepping sideways and for stationary leg and balance exercises. Handicap ramps usually have stairs nearby as added challenges.

I have difficulty in walking. No accident. It developed.I was with my massage therapist and asked me to bend my legs and he noticed that the signal from nerve to muscle is slow. I feel that there’s pulling me backward when I’m standing. What do you think is my problem?
