    When I was a kid, my Dad was the doctor...lol! He would have gone for the wire cutters first thing while lecturing me on the importance of not putting washers on my finger! Actually it wasn't a washer but rather a grommet! I thought it was pretty! :)

    Brave little guy. When our son was two, he chewed up an eraser, and shoved all the little pieces into his ears. It took four adults to hold him down to get it out. Not a fun day.

    The ER is overkill for this issue imo. I would have gone to the garage or tool kit in my truck and gotten some snips and lubricant and this would have been over on a few minutes. Parents need stay calm and then adapt, improvise, and overcome.

    It's possible they tried for a while at home but couldn't find anything that was working and the finger started to swell a bit or something.

    You are correct, a pair of wire snips to start cutting the plastic towards the finger then the last little bit, go down the other side if necessary, done. Versed and a trip to the ER unnecessary. Probably a $8,000 bill came home with this family.

    They gave him intranasal Versed at the start of the video. It’ll dramatically ease his anxiety and awareness, and depending on the dose, may prevent him from remembering the procedure. But to your point, it’s certainly true that Versed doesn’t have any pain-relieving properties.

    There could be vascular damage it’s better to be safe than sorry. Medically speaking; she could’ve caused a devolving of the finger if she tried to just pull it off. I’ve met nurses with all the knowledge in the world freak the fuck out when their kid becomes sick or injured MALE OR FEMALE. This kids visit to the ER has nothing to do with family dynamics. Also, what the fuck do you care what she pays for an ER bill?!

    💗😊💕The same thing happened to my little guy when he was a toddler….I was so surprised that this toy from a birthday party goody bag got stuck on his finger….the ER doctor was able to break apart the kaleidoscope, into many pieces with a pair of pliers. We are so lucky that he didn’t need any needles.💕😊💗

    Why didn't they use a cast saw? It would have cut the plastic without harming the finger and they wouldn't have had to inject into the finger which probably swelled it up more.

    I remember as a child in UK having plaster casts for bilteral talipes (I am myelomeningeocele) and was terrified of the noise that the saw made when the casts were being changed or removed! Reassurances that I was not having my leg ampuated were to no avail!

    That would work for sure in some children and probably an older child. We usually ask the parents about the anxiety level of their child. If they have any concern we usually do intranasal versed.

    @@lmellick Gochya. Versed in my area, hospital considers it a moderate sedation. We get to do all the monitoring/paperwork to go along with that. Its not the end of the world.

    Good job, all, and MOM, for keeping the little dude calm to allow the docs to do their 'deconstruction' work. Good thing to get him there quickly, before damage was done. :) Also, I was wondering, could the swelling (and obstruction by the 'ring' of the item) prevented good perfusion of the local numbing injection?

    If the metacarpal block had worked, it should have gotten the digital nerves proximally. Metacarpal blocks are not as effective as my favorite the tumescent digital block. Good question, though.

    Floss rope? Try to tie it firm/short from the end of the finger to the hand. It often helps.

    I was watching the championship ring removal just wanted to add in that the string technique works much better if you use an ice pack and keep the hand above the head for 5 to 10 mins first :-) Oh and using the cast openers (cast/splint cracker I dont know what you call then there but they look a little like scissors to open a cast that has been cut down one side) is easier than pliers etc as the tip is already flat and can use more controlled force to open a ring And for removing anything that is plastic if you can put a thin metal guard between skin and the item if you heat scalpel or a needle you can melt the plastic instead of cutting it which reduces the pressure on the finger and also helps prevent accidental pinching or skin tearing as it's much easier to control (I had an autistic foster child for 8 years so we had many many incidences of fingers where they weren't meant to be as well as items in orifices they disnt belong in etc) 😊 love watching your videos and your bedside manner is impeccable!

    Cold water will help the finger to stop swelling. The boy with the class ring you worked on. Why not try cold water or ice to reduce swelling?

    If they were to cut it off it would probably do more harm than good, it would be very difficult and they probably wouldn’t even have a tool to cut thru thick plastic like that

    Medicine is often not a pretty as we would like. We try very hard to minimize pain. However, even the IV lines placed for procedural sedation can cause pain especially if multiple attempts are required. If our digital block (metacarpal block) helped with a sprayed pain dampening vapocoolant along with the nasal Versed had worked, we would have caused minimal discomfort.

    I’ve used nasal versed before for various procedures like IV placement, PICC line placement, and Incision and Drainage. It does work pretty well but you really have to administer it correctly or else it doesn’t really work

    In my opinion, pulling out some of these plastic like stuffs are harder as their friction may cause more problem, if possible cutting them is safer.

    Very well done, but a trip to any fire station and a fire firefighter would of had that off in a few seconds for free. Plus the child would have probably gotten to see the trucks too.

    One night I got a CD stuck around my finger. It was there for about eight hours, I spent the entire night trying to figure out how to get it to come off. Fortunately, that morning I was able to get it off without having to seek emergency assistance.

    Yes, I like Versed IN, but if the child is hyped up and anxious it sometimes barely touches them. Also, if the nurse doesn't push hard on the atomizer it just leaks out as fluid and not mist. When that happens, it seems less effective. I may start using IN ketamine more.

    The child could have been given an initial anesthesia by breathing in order to relax... Then an anesthetic injection is given to the finger I don't know why the rush 😬

    Have you ever tried to block the child’s view of the injury (or the work being done to the area) with some sort of drape? Maybe this would further reduce anxiety in some patients? Just a thought… absolutely not a criticism :)

    You are correct and good suggestion. When possible we will turn so we block their view, but that wasn't done for this patient.

    Actually, part of the video not included was my spraying a special oil for taking off stuck rings. The finger was too edematous for it to work. Thanks for your suggestion, though.
