    Brad, thank you so much for this great review. I am taking Finasterid 1mg now for 10 years to prevent aging related hairloss (which works incredibly well with absolutely no side effect of any kind).     Thank you Doc for a scientific and independent report that always shows pros and cons. I would like to ask you for a video discussing the problem of testing the body's reaction to dietary supplements that you recommend to your patients and what tests (and how often) you undergo yourself, to make sure that, for example, the liver does not show symptoms of problems.

    Yes we in naturopathic community use a supplement called saw palmetto, which has the same mechanism of action; alpha reductase inhibitor which prevents the conversion of testosterone to its more dangerous form dihydroxytestosterone. Which is responsible for male patterned baldness, BPH and prostate .

    So from your video there appears to be be another possibility for future research: 1. The inhibition of dihydrotestosterone and prostate shrinkage for better detection. As you mentioned, lower dose finesteride may reduce the aggressiveness of cancer in the finesteride group. This lower dose, whether 1mg or other less than 5mg may be more viable an option if low dose tadalafil is added to the regimen to gain more profound prostate shrinkage. Your comment is welcome.     I am on dutasteride for 2 years now, my hair loss stopped and I have never had any prostate pains or issues ever after taking it. My doctor recommended dutasteride as it is stronger

    Agree, taking Avodart (Dutasteride) is a lot more effective than finasteride for hair loss, reduces prostate as well.

    Finestride decreases PSA, and because it is “ falsely low”, we don’t investigate early prostate cancer. Thus why we catch prostate cancer in a more aggressive state with people taking finestride. This is what I have seen in my patient population as a physician.

    @@bogrunberger that’s because of the rise in testosterone which means more testosterone is converted to oestrogen. You could lower body fat to counteract this, or take a SERM like tamoxifen.

    But isn't that why we're supposed to take into account a person's use of finasteride when measuring PSA and have a higher index of suspicion even with mildly elevated PSA in this population? (Maybe that's easier said than done?)

    I was having a poor reaction to Finasteride (aka PROSCAR), "feeling- not-like-me." My Urologist suggested the PROSCAR was creating a hormonal imbalance. So I am also interested in hearing your thoughts and comparison to prostaphane (sulforaphane) as a suitable substitute. Currently taking 2x 0.4mg Tamsulosin (Flomax). Am also reviewing/taking; Saw Palmeto, Pygeum, Pom-T, and Lycopene for prostate health. Your reply and suggestions greatly appreciated! Cheers.

    I interpret this a bit differently. I don't see detection bias, I see that Fiasteride is just preventing low-grade, non-terminal cancer and has no effect on the deadly cancers.

"Impact: This large confounder-adjusted study supports the view that exposure to finasteride is not associated materially with male breast cancer risk."

    Good news. Alternatively, Saw Palmetto and Stinging Nettle root extracts taken together do the same thing as Finasteride (DHT blocking) according to several studies and personal experience with hair regrowth. Blackmores has a prostate formula that has both herbs in adequate amounts as well as Zinc and Selenium, but any quality extract will do. I highly recommend this herb combo for anyone experiencing hair loss, it works! As a side benefit you'll be protecting your prostate too.

    Finasteride is more selective for 5ar type 2. You don't want to inhibit allopregnanolone conversion with unselective 5ar inhibitors. Rather take less finasteride if you want a milder effect.

    I agree, I do the same; saw palmetto and the same stinging nettle root for like 7 years now, I didn't notice any negative effects. But I also used finasteride 5 mg per week (0,714 p/d) 6 years now --> still a good libido. But I saw a video that it's not really beneficial while using finasteride + saw palmetto together, so I stopped using saw palmetto for a few months now... Don't notice anything.

    That’s totally wrong saw Palmetto and nettle root does practically nothing to lower scalp and serum dht. It doesn’t work for hairloss. Load of rubbish.

    @@Railman122 I only have 2 moderate balding patches in the front. My brother is a decade younger, doesn't take finasteride, and has 2 very prominent one alone with baling top of the head. My father's side of the family either have no balding or front partches, my mother's side on the other hand has what my brother experiences. But my brother inherited my father's hair color and texture and I inherited mother's, go figure. So I guess I'm not as bold purely because of early use of finasteride

All data I have seen, show finasteride 1mg just as effective as 5mg with respect to shrinking the prostate (perhaps you have a study that shows otherwise?). 1mg reduces PSA less than 5mg so 1mg may not have quite the same degree of prostate cancer reduction, but it's probably a small difference. --Elliott

    I take Proscar 5 mg for the past five years with no side effects and improved BPH. I just wanted to mention that with regard to PSA levels, I have read that you have to double the value of your blood test report because when Proscar lowers your PSA you could miss the beginning of an aggressive cancer possibility… So if your PSA is 0.9, then it should be doubled and interpreted as 1.8, if PSA result is 5 then it should be interpreted as 10, for example.

    The most important thing to do to reduce prostate cancer rates is to eliminate LED lights. The blue light disruption from LED street lighting alone (in Spain) led to a more than doubling of the rate of prostate cancer. Indoor LED use made the rates even higher. Ditto for breast cancer with a ~50% increase. These occur through sleep and hormone disruption caused by the blue light exposure from LED lamps, and the impact then on the hormone systems of the body. This also interferes with many cancer treatments. Some LED lights are better than others in emitting less blue light. Less isn’t zero. A rare few LED lights do not cause this effect. They emit 415 nm light rather than 452 nm. And they have a broader range of phosphor colors. These are very uncommon and not generally available. Instead, only the intensely blue lights dominate.

    52 year old male, have been on finesteride for the past 26 years for male pattern baldness, came off finesteride (5mg 1/4 daily) in March, psa went from 2.6 to 4.5 in 14 weeks, free psa % was 12.6%. Had an MRI 1 week ago, a 5mm lesion was identified with a PI RAD = 4 score. Urologist recommending a biopsy. Could this lesion have been in my prostate for a number of years whilst on finesteride and it has now become more aggressive from ceasing the 5 alpha blocker?

    Makes sense since Finasteride blunts DHT Dehydrotestosterone which cause prostate growth and hair loss. This is me writing this prior to watching video. Since i know a thing or 2 about this area!     There needs to be a comparison of the data between the two Finasteride groups (placebo and dosed) of men with a BMI lower than 25. Did the overweight group suffer the aggressive cancer development more by a factor that is inconsistent with their BMIs? What was the BMI range tested?

    I have been taking Finasteride for over 30 years with varying degrees of success in my fight against hair loss. Some of the side effects of Finasteride are reduced if you regularly consume Epilobium which inhibits the enzymes 5-alpha reductase and aromatase. These 2 enzymes are involved in the etiology of benign prostatic hyperplasia. I notice better erection quality, more sensitivity and no gynecomastia fortunately.

    Thanks Doc. I've used it for hair loss for about 3 years. It works for me. Sex is not a problem at all with it. My worry is though, If i was trying for a.baby, should I stop as I do hear it could mean complications with the birth. The things I read can be conflicting

    Interesting video. But what about less then 1mg of finasteride? I took 0.5 daily for preventing hair loss for about a year now, because at the beggining I had some issue with higher dose. When I split the pill in half all the side effects are gone and hair are still alive. Hope that's gonna work for preventing prostate cancer too.

    Since astaxanthin is also a 5-Alpha reductase inhibitor, I'm wondering if it could be a non-prescription alternative to lowering the risk of getting prostate cancer (and for countering hair loss, too). Someone ought to do a study on that. I take both Finasteride and astaxanthin.

    I've been taking dutasteride to shrink my prostate and it has improved my flow but not all the way so I added brocolli sprouts but no benefit then i swapped it out for Tumeric and in two weeks it was improved and then I upped the tumeric and I think better again. I know of one rat trial that seemed to show that in rats tumeric outperformed dutasteride and even better in combination. What i wonder now though is now that my prostate is smaller can I lower to a kind of maintenance dose or even better just switch to the tumeric. I',m slowly tapering. I know it takes about two months to unwind for me so it will be a slow process. So now I'm down to 5 times per week and tumeric on about 4 times a week. I'll keep going. My urologists don't seem to care if anythign they try to talk me into surgery. I really don't like the side effects that much you only miss your libido when its gone and it seems that I'm more sensitve to others feelings now. It's I guess an advantage but it takes time to get used to. It's nice to have more hair. I'm not a doctor so if there is anything you could add I would be grateful. I'm sure many will be interested in this field. Thanks for you good videos they have changed my life a little for the better.

    Most people that develop prostate cancer suffer from BPH. Finasteride is approved for BPH, and if you diagnose it early, it might as well prevent it from happening

    The long term study shows 10.5% got cancer in finasteride group, the placebo group 14.9% got cancer, this is a 4.4% reduction, to say it is 25% is twisting stats deceptively IMO. 4.4% isn't a huge difference, are the possible side effects worth a 4.4% chance reduction in getting prostate cancer? That is the question.

    Look up absolute vs relative risk. Most drug studies use relative risk stats, whereas you are using absolute risk. It's always a good idea to look at both, but neither is inherently more valid than the other.

    I REALLY think caution should be exercised before trying finasteride/ dutasteride. The potential mental health/ long term endocrine side effects make it seem like this one may be worth skipping

    It seems likely that astaxanthin does this. It is a 5-Alpha reductase inhibitor too; and it activates the FOX03 longevity gene, which lies dormant in two-thirds of people. It is said that you are three times more likely to live to be 100 if your FOX03 gene is active.

    Hi Brad, When I took Finasteride I did have side effects. I am now taking Saw Palmetto and Pumpkin Seed Oil - both have been shown in several studies to inhibit the enzyme that converts Testosterone into DHT, although probably less effectively than Finasteride. I have not experienced any side effects whatsoever from these two so far. Any thoughts on these herbal treatments?

    If you look at the data though, some men do not respond to Finasteride. I suspect there is an insulinogenic effect on receptor sites in the prostate that promote both BPH and prostate cancer

    Are there any other risks of finasteride like increasing estrogen levels, as the amount of circulating testosterone not being converted to dht but instead, as a balancing act, it's aromatised and turned into estrogen? Usually this happens when people take exogenous testosterone, like dome bodybuilders, so the excess testoterone gets turned into estrogen, especially when they get off of it. I understand that this is not quite the case, cause the increased testosterone is just by a slight quantity as dht is itself very low in the adult, but nonetheless, how ca we be sure? After all, estrogen itself, is dangeorous abouve a certain limit in men, and it's also dangerous for people that have blood clotting predispositions. I'd be so curious to know your opinion about this possible issue, if it is, at all, an issue or not.

    finasteride is risky. read about "post finasteride syndrome". In some men, blocking DHT conversion messes up their whole system beyond repair

    I took fin for hair but noticed it reliably raised my liver enzymes. I want to take it for the hair and potential prostate benefits but not sure if it's worth the potential liver issues. Once my enzymes are fully in range I may give it one more try

    However, this medication also reduces the PSA (prostate-specific antigen) levels by approximately 50%. That is why, when we are screening for prostate cancer, the PSA number should be doubled in order to get the true reading for the PSA. Anecdotally, this might not be happening in the real-world setting. Now, let’s say the PSA is really 7 ng/mL, but because of the 5-ARI, the lab test will show 3.5 ng/mL. This 3.5 is less than 4.0, and it does not look scary at all, so we would not even suspect prostate cancer. Now, if we saw 7 ng/mL, we would have great concern. So, if we don’t double the number, we could be missing some prostate cancers. This study included over 80,875 men in the Veterans Affair database to see if this is really happening. Indeed, the patients on 5-ARIs were worse off for prostate cancer in every respect compared with those who were not taking the medication. For example, the time to diagnosis from an elevated PSA was 3.60 years (95% CI, 1.79-6.09) for the 5-ARI users versus 1.40 (95% CI, 0.38-3.27) for nonusers.     I have been on Propecia for over 10 years, and unlike all of my other male friends of similar age (48) I do not have to get up to pee overnight. It could be related to prostate I guess.

    Some viewers may think about doing the plasma dilution by donation. We must stop Finasteride at least 30 days before any blood product donation, except direct donations to a male or menopause'd female.

    It's interesting work. The side effects have always scared me, but it's nice to know the rates are low for side effects and they tend to be reversible. Specifically for prostate cancer, I'm much more interested in research into Vitamin D levels. There have been quite a few studies on the link between low vit D and aggressive prostate cancer, and prostate biopsies pre and post Vit D treatment have shown a host of epigenetic changes that show promise in reducing the aggressiveness of prostate cancer and possibly even preventing the expression of markers associated with precancerous states. I don't know if all the studies stand up to scrutiny - there are a lot of "Vitamin D will cure everything" proponents out there, but I think I'd be more comfortable with vitamins and herbs for reducing inflammation and promoting epigenetic changes than something that directly interferes with hormones any day.

    This drug destroyed sex life of so many men! Even if stopped it leads to side effects! There are many other natural substances that reduce prostate cancer risk that are safer. Finesteride is well known by men losing hair its side effect

    @@theancientsancients1769 No it hasn't. Finasteride is one of the most popular drugs, used by MILLIONS of men each year, with extremely low reports of mild side effects. We have 10 year long studies that followed large sample sizes of patients and we saw continued improvement and still, the vast majority of the men had no side effects. The no placebo effect is real for men who have preexisting health or sexual health problems and then take the drug and think it gave them side effects. If it was destroying so many men's lives, it WOULD be reflected in not only the high quality studies we have on it, but also the follow-ups doctors have with their patients. But it is not eve reflected in the patients who take it for prostate or hair loss. Every year the group of hair loss doctors meet at an annual conference for those who are members of the hair loss medical board, and they go over the percentage of their patients each year who had side effects and it is always in line with the small 2-$ number you see in the studies on the drug. If it wasn't, it would show up in those records of the patients. But many men see the fearmongering pushed by natural hair loss companies selling snake oil products claiming to 'cure' hair loss but being a scam. They have a big financial interest in pushing fear about Finasteride so more young men can buy their products. Finasteride is very safe and has been used for a very long time now.

    @@siemensohm That’s not an argument. All hair loss doctors are. And secondly, a large majority of hair doctors who prescribe Finasteride never end up doubt hair transplants. In fact most of them don’t. And Finasteride is now widely used as a cheap generic, so there isn’t much money to be made there, so you’re wrong on that as well. That said, even if there was a financial conflict of interest, mathematically, that could not stop the fact that you see the same small percentage of side effects in the patients who use it as you do in the studies. There is no way you can fake that.

    I started taking finasteride for the health effects a while ago, with the nice side effect keeping hair. Regarding the negative side effects, I haven't experienced anything aside from maybe a slight increase in anxiety. Most of the side effects are indistinguishable from placebo, based on studies I've read.

    Many of the side effects are due to nocebo. There is a hate campaign against finasteride on the internet, which is scary, these people even invented a medical condition called "post finasteride syndrome" THAT DOESN'T EXIST! All these so-called side effects can be caused by our thoughts, also erectile dysfunction, if 1 of 100 people get side effects chances are you will overcome them once the body find a new balance, but they are really rare, I wonder why there isn't the same hate campaign for all the medications people take DAILY, if they would read the side effects they would stop immediately, and they are way more dangerous than stupid self-created things like "depression" "brain fog" and "anxiey"....

    Is it true that finasteride throws off your PSA count? My husband's dr missed this and assumed his count was normal, when in fact it should have been doubled for an accurate assessment. He saw an urologist for frequent night urination and she discovered the PSA was actually alarming and sent him for biopsy. He had cancer and was spread outside the prostate. He had proctectomy and is now having radiation. We are quite disappointed that this could have been caught sooner and treated differently.

    Do you think that Finasteride will be improved and eventually show up in Walmart? I often wonder why there are bald millionaire men. I have read that in the hair pill trials, some of the placebo group regrew hair. Could the FDA stop Placebo hair trials using sugar pills ?

    @@seanbardot2822 My dad was diagnosed with 3+3. They said wait and watch.. He died from metastatic prostate cancer. I don't think his age was the cause of this medical failure. Finasteride is commonly prescribed by urologists who do no understand prostate cancer. It only obfuscated the psa reading. Does nothing to lower the risk of prostate cancer developing. My dad is one example but also plenty of good information out there too. Prostate cancer research institute for example.

    ​@@seanbardot2822 Best way to monitor PC, IMO is with specialized mpMRI scans on a 3T. Make sure nothing is growing in there. If that is too expensive, then second best sounds like a transrectal 3d color doppler ultrasound (something like that).. it's a fairly new technique and but apparently more widespread. If a urologist prescribed finasteride as a way to avoid PC, I would find a new urologist. And do watch the PCRI videos, they are very good.

    It is a concern for many men as they age...I have had a few scares but they only monitor the PSA...physical exams are not being done in America at least by physicians I have seen.

    Wouldn't it be nephroprotective aswell to reduce androgenic signaling? Men are much more susceptible to chronic kidney disease than women. It might also lower hematocrit a little bit, not sure if that is beneficial for the cardiovascular system.

    We’ve been giving this to male patients with BPH, male patients that are s/p Tx for prostate ca. and even male type baldness in the US for more than two decades now. We also give it to women that have balding hair loss, excessive facial hair, acne and other issues arising due to excess testosterone production stimulated by tumors causing PCOS. We give it to transgenders too.

    Men are scared to use finasteride because of the erectile dysfunction side effect - I certainly am, ive got high risk for prostate cancer and baldness genetically, Id love to start taking it if it wasnt for that side effect

    Thanks, a very useful video. However, I cannot agree with your conclusions related to mortality (the way I understood them). You are saying that the result showing no difference in mortality is due to early detection of PC in patients on Fenesteride? This cannot be the case, as earlier detection should lead to better outcomes, i.e. lower mortality. Again, the puzzle is this: why Fenesteride provides no mortality benefit although it reduces incidence of PC? Let me know if I misunderstood your explanation. Thanks

    There may be an alternate explaination as to why Finasteride does not lower the risk of more aggressive cases of prostate cancer. It is possible that Finasteride only stops the more mild cases of prostate cancer but not the agressive forms. This could be because there are some other factors causing the aggressive cases which are not addressed by Finasteride. As a result, the group using finesteride would show an overall reduction in prostate cancer, but not the aggressive cases. If this is a case, there should not be a concern that Finasteride is elevating the risk of aggressive forms of prostate cancer and it would provide a benefit to the more mild cases. That is to say, if your going to get an aggressive case of prostate cancer, it will happen whether or not you take Finasteride.

    You know what also greatly reduce the cances of hormonal cancers? Cutting out galatose from the diet, cutting out growth hormones from the diet and introducing phytoestrogens to the diet. Next to that a high fiber diet lets the body get rid of any excessive estrogen we make through to much fatty tissue on the body or from ingesting it through our diet (milk much?). Doing this will reduce the chance of hormonal cancer like breast cancer by 25%. Also a complete plant based diet was found to reduce it by 15% in an analysis from 2017. For prostate cancer? 19% lower chance of doing from it for prostate cancer. But also a protective roll from cutting out eggs, dairy and processed meat. Some studies even showing a prevention rate of 35%. Also people who cut out diary, inclusing butter and eggs reduced the amount of prostate enlargement as well. Where a plant based diet was even shown to be protective against prostate enlargement. Bottom line, eating this that trigger the incentive "grow" in your body lead to bad outcomes when it comes to hormonal tissue.

    I have hair loss (alopecia androgenetica) since I'm 20. I take Minoxidil since I'm 20. (now I'm 29) And tried Finasteride (small dosage) for 2 months a few years ago. It looked like I was getting more body fat. Since then, in terms of nutrition, I can experience how a woman feels that they gain weight faster. When jogging I lacked an inner sense of aggression/adrenaline to spurt. Suddenly 2 months later I also got strange stitches in my breasts. My breasts were a bit bigger. That is one of the reasons I stopped taking it. A half year later everything was ok again. Now I take saw palmetto.

    Be mindful the negative side effects were generally seen in older men as studies were done in the older cohert and probably not a concern for younger dudes.

    This can be beneficial and helpful for some people. But there are also a lot of side effects for some. (I had also side effects, reason why I stopped taking it). Further research is needed.

    It was well known 20 years ago that you could take a much smaller dose and still get the hairless benefits than the drug companies suggested. However even then the side effects were unacceptable for any male who wanted an optimal sex life .

    Still waiting for an informed response about the risk of increasing estrogen after blocking the 5a-reductase, which will furthermore affect males and why is it that we can't think of another way of eliminating cancer susceptibility, like, let's say, blocking DHT receptors in the prostate itself? Or finding an immunotherapy or anything that targets the problem itself without messing up other things in the body.

    According to wikipedia, DHT is important for maintenance of the prostate gland and seminal vesicles. Also, the post-finasteride syndrome foundation says that there are way more side effects to finasteride than erectile dysfunction and gynecomastia, even after stopping Finasteride.
