A 65-year-old woman presents with a fever and malaise which she had noticed to be progressively worsening over the past few months. Her vitals signs on arrival to your clinic are 39.2°C, HR 130, RR 32, and BP 88/50 mm Hg. She has scleral icterus on exam, along abdominal tenderness and appears disoriented and confused. What is the likely diagnosis?
