The Day 3 fertility testing process with Dr. Johnson - What an infertility workup entails and what the results mean for suggesting an infertility treatment plan.

If you've been struggling to get pregnant and gotten fertility testing at a fertility clinic, you may have found out your results and given an infertility diagnosis. But what do these test results mean as far as next steps?
We sat down with Dr. Matrika Johnson, a fertility doctor at Reproductive Specialists of the Carolinas to explain how a woman's fertility is tested and what that means as far as treatment to help her get pregnant.
A lot of different tests are done when you go to a fertility clinic and these all tell your doctor how your body will likely respond to infertility medications and what treatment makes sense for you.
The most important testing your doctor does is looking at your ovarian reserve. Ovarian reserve testing gives an estimation of how many eggs you have. Ovarian reserve is tested by measuring your AMH, FSH, and doing an antral follicle count. The most important measurement of ovarian reserve is your AMH level, as this has been shown to be the most accurate measure of how many eggs you have. It also can be tested at. any point in your cycle. Most fertility hormones need to be tested early in the menstrual cycle when your hormone levels are lowest, which is why day 3 fertility testing is common.
The ideal AMH level for women to have is a level greater than 1ng/mL. Women with an AMH of less than 1 are classified as diminished ovarian reserve. Younger women with diminished ovarian reserve may experience early menopause. Ovarian reserve declines with age so older women are. expected to have a lower AMH level than younger women. However, your fertility doctor still prefers an AMH of 1 or better for ideal outcomes with IVF.
Your Day 3 FSH level is also an important marker when estimating your ovarian reserve. If your FSH is high, it can also signal that you have diminished ovarian reserve and that you may get a lower number of eggs from an IVF cycle.
An HSG is an x-ray test done to check that your fallopian tubes and your uterus are normal. An HSG is normally done between Day 5 and 12 of the menstrual cycle to get a clear view of your uterus and tubes.
Some doctors also pay close attention to how thyroid function can affect your ability to get pregnant. A TSH of less than 2.5 is ideal for fertility.
After you have done your fertility testing and have your results, a fertility doctor then suggest which sort of fertility treatment would be best to help you conceive. The most common options are IUI or IVF.

    This was so helpful and informative! We have had one successful IUI and are trying for a second child via IVF. because our 1st doctor was swamped, we switched doctors and our IVF failed. Only after watching this video did I realize how little our second doctor (who oversaw our failed IVF) explained each step to us. We would get blood tests constantly but never have a breakdown of the doctor saying "we're looking for A, B, and C with this test, and here's where you should fall within parameters." Never, before or after blood tests. He would simply say " ok, we're close but not yet" or "next week we'll extract". I'm rambling but I just wish all doctors could explain in layman's terms and have the grace and patience to do so, I feel I would've had to opportunity to decide to wait, and try when the test results were more favorable.

    @fertilityspace so my afc on my initial ultrasound was only 6. Just had my cd3 bloodwork done. My fsh was 20 and my amh is still pending. I'm 29 and my doctor is leaning toward DOR and we're going right to IVF- which we are fine with I'm just worried my body won't respond well and I won't get any eggs😭

    Do those fertility at home testing kits work like the fsh strips ? For the last couple of months I have been wanting to get day 3 testing done but my period keeps arriving on a Thursday and labs in my area are closed on day 3 Sundays I'm 40 years old ttc for over 10 months

    Hello I’m 45 years old I have six kids oldest is 26 youngest is 11 years old. I tested my fsh and it’s 5.64 can I still get pregnant naturally with normal periods? I had a miscarriage 5 months ago I was 11 weeks pregnant. I didn’t want anymore babies but after the miscarriage I want another baby. Does my fsh sound good I tested 6 days before my period?

    @@Pabloinandout great question! You can actually test AMH at any time in your menstrual cycle! You don’t have to wait for a specific day to test AMH because it doesn’t fluctuate throughout the cycle like some of the other hormones do.
