Found this one really straight forward for fire scenario.

Why didn't they take her out the exterior door that was *right* next to the room?

Is that the only extinguisher in the building? There should already be one in the corridor next to the room. If the TV is on fire, put it out!

What about bed-bound people with later stages of dementia and physical deterioration?

Because it was an electrical fire which the care staff are not allowed to deal with they have to wait for the fire brigade to isolate all the electrics in the building.
