This was the most perfect ending to an amazing show. It changed television, forever. We began with Dr. Greene,and we ended with Dr. Green. Carter, the student, was now the teacher. Absolutely perfect!

Dr. Green the teacher I believe said the very same thing to Carter when he first started so I thought it was completely appropriate to finish the series with Dr. Carter saying the same thing to the new Dr. Green in homage of Anthony Edwards who was the OG.

"Dr. Green ? You coming ? " That's the best way to end the show, which connects to the start of it.

Masterful end connecting the first episodes to the end scene

This is one of the best endings ever. The camera slowly backs away, while the ambulances arrive. It make you think that the series might end here, but they're still there, doing their jobs.

Even though shows end, just like our lives, the world keeps going after it's ended. I like these types of endings the best. Some people are selfish and want "closure", but a lot of times in life, you don't get that.

This is how I view my nursing jobs. The days of wings and units being named after hard working nurses and doctors are over... The ER and Facility will continue to run long after your gone.

I always thought it was so genius to wait until the very last scene for the viewers to finally see a full shot of the hospital after 15 years. Fantastic ending!

I remember that making me oddly emotional the first time I saw this.

When Carter calls Rachel, "Dr. Greene," I smiled. Mark and Peter were his mentors, and I have a feeling he'll be Rachel's. Totally full-circle.

John Hinchliff even Carter told Morris to set the tone in season 13 in Noah Wyle’ s final full season on the show. Greene told Carter to set the tone at the beginning of the show and on Greene’ s final episode as an ER Doctor.

I was so happy they brought back the original theme music at the end. I hated the cheap intro they use for the last 3 Seasons.

When a series finale comes full circle that is what I consider to be a great finale.

I love how they did the entire last season actually, catching up on characters that had left(Ray, Doug, Carol, etc) flashback episodes with characters that had died(Mark,Romano, Pratt) then the ending came full circle.

Still my favourite series finale if all time. Bringing back Rachel and having her continue her father's legacy is just brilliant, and then finally seeing the hospital in full for the first time in 15 years. I'm still in awe. Plus they brought the opening credits back for the first time in years too.

To end it with Carter, little Greene and the whole staff doing what they do was just awesome and I cry every time I see this scene. And they fact that he called Mark's daughter "Dr. Greene" brought tears, even though Mark is dead, he lives on through his family and his friends.

I love how they're all standing waiting for the ambulances to arrive, a subtle nod to "Blizzard" (Season 1 Episode 10), after the og team prepares for the patients from the Kennedy Expressway and waits for them. And of course "Dr. Greene, you coming?", followed by the music kicking in. Amazing series. No other medical show will ever live up to E.R.

I watched ER from the first episode to the last. This finale was an amazing finish to this series. The crisis situations covered on ER always brought out the best in the entire group, and I don't think they could have ended this series any better. Dr. Carter inviting the (not yet) Dr. Greene along just brings it all home. THANK YOU, Mr. Crichton, Mr. Wells and all involved for an amazing 15 year run.

@@hcs4life21 ... Two reasons. First...money. By cutting the opening credits it made room for more commercials. That's why many shows don't have openings anymore and just go straight into it. The second reason was the opening sequence itself. With so many cast changes and main characters coming and going in the final 3 years they didn't want to have to make a new opening credits scene every 3 months.

Yeah, if you ever had a chance to see the actual set, you would really understand what it must have taken to create this scene. The actual set wasn’t much bigger then what you saw through the series. Kind of cool they went all out for that last shot.

Hate to tell you, but that's not the real Cook County Hospital. I was born and raised in Chicago and went to John Stroger Hospital for 5 years which is technically Cook County Hospital renamed. The true Cook County Hospital is just N of Stroger at 1835 W Harrison St, Chicago, IL 60612

"General" was not in the real hospital's name and ER dropped "Cook" from signage during the first season to make it clearer that it wasn't meant to be a specific depictiion. In the ER universe Chicago has County General Hospital instead of Cook County Hospital.

This scene final is so good, especially when Carter asks Mark Green's daughter Rachele if she wants to join!! What a beautiful ending!

I think I will never be able to process that ER ended.In my small room somewhere in Hungary,I had been watching ER for 15 long years.I grew up watching this series,and I totally identified with the characters' life.I loved them all,I cried with them,I laughed with them-call me sentimental or stupid,I don't mind.ER was the only series which I couldn't get bored of.I miss its atmosphere,the actors,and I'll miss them in my whole life.Thank you for the wonderful 15 years!R.I.P. I always get chills when I see this especially when Archie asks if Carters working. This show is missed.

This show was unboubtably the best medical drama ever. I've watched every episode multiple times and never get tired of any of it. It got me through life at point, knowing on Thursday at 10-I could unwind, realax, and be along the side of staff at County General Hospital.

GeorgeGraves I got the same feeling when our younger daughter had to have heart surgery at 18 months of age and we got our first visit to Johns Hopkins (her cardiologist saw her at the monthly clinics in our town up till then). It still blows my mind to think that the hospital alone covers more than an entire city block, and I am no less impressed by their knowledge and skills. If we ever make decent money, you can bet we’ll be donating some to this place as a way to say thanks!

no matter how many times I watch this "Dr.Greene, are you coming?" gets me every time.

To think ER ended just 11 years ago, adding to this perfect ending Carter tells Dr. Greene his mentor's daughter if she's coming everything coming full circle the student is now the teacher.

The train for me was another character more of ER, great to watch here his last scene🥲👏

One of the few shows whose final episode happened to be directed by the same man who also did the very first episode. Kudos to director Rod Holcomb! This was a deserved Emmy win for best directing on a drama series.

When this ended, I started bawling because it was like a part of my childhood that ended. But I also felt extremely satisfied with this fitting ending to a wonderful show! And I was more than thrilled that Carter was there!

Right. I was 5 when the show started and I watched it up to the last season with my sister. This show was well ahead of its time and I became a better medical provider watching this show!!!

It would've been TOTALLY amazing to see John Carter as the Chief of Emergency Medicine, that would've been the final detail to the last season (in my opinion)

I recall over ten years ago me and my parents went down to California to visit my aunt, and during our last day we went to the Warner Bros. studio lot. One of the sets we saw was part of the sets of ER (where a patient is taken into the Emergency Room), and as we were passing by sets and such, we saw Laura Innes (who played Dr. Kerry Weaver in ER).

Even seeing this now, I still get teary, now that ER is gone. My mom was a huge fan up until season 8. She stopped watching right after Mark Greene died (he was her FAV character) and I didn't care for the show. By season 10, I was right into it, and as huge a fan right to the very end, as my mom had been. So, I can say....SHE turned me on to the show and I'll always have her to thank for that. RIP Mom.

When Carter says that to Rachel, I tear up. Echoes of Mark and all of the great work he did. And then the theme music comes on. It makes me smile.

I feel like Rachel was standing there representing her Dad in a way, even though she IS a med student. Still cool.

"Dr Greene, you come'n? " Instant goose bumps and teary eyes.

R.I.P ER Thursday September 22nd, 1994-Thursday April 2nd, 2009.

I have to admit, when he said "Dr Green" I teared up a bit. Full circle.

My old man was an emergenciologist and he was head of the ER of our local hospital. Now gladly enjoying his well deserved retirement. I’m truly proud of him.

My all-time favorite medical drama.

I want E.R. back :'(

Truly the best series finale.

Not embarrassed to admit I cried like a baby. So well done.

It was so cool how Rachel Greene was gonna become the next Dr. Greene. Carter was gonna be a mentor to her, just as her father was to him.

It was so touching that it ended with Dr. Carter teaching Dr. Rachel Greene the daughter of Mark Greene. It was so fitting

I was seven when the 1st season of ER aired.. I loved Dr. Greene and the scene where he died was probably one of the saddest moments of TV history. It's so befitting that Rachel comes in for the final episode... "Dr. Greene, you coming?" - perfection. A fantastic show, a contender for one of the best ever.

Would've loved a season to see how Carter acted as Mark Greene to the these younger doctors. Now he's the old calm mentor

I honestly had tears running down my cheeks when I watched this. Felt like the end of a long journey.

I just what to say er is and was way better then greys anatomy there will never be another show like it how my people cried watching the last episode i did.

That brings back memories like you wouldn’t believe. I watched every episode from the beginning to the end, and I still can’t believe that it’s not on anymore all these years later. I knew this would be the final episode, and I hoped beyond belief that Noah, Wylie, George Clooney, and Julianna Margulies would come back for the final. And they did and when I saw them I lost it my three favourite characters also Dr. Green but obviously he couldn’t be there for the finale but he was really in the form of his lovely daughter. I ER was the greatest medical show ever to be on TV. St Elsewhere was also a really good one.

Admit it, when the theme played at the end all of you smiled 😊

I don't think they could have ended this any better. What a great way to wrap up such an awesome series. It really showed the essence of the show. I miss ER, although I like the early seasons better than the recent ones.

Wow.... A flood of great memories flashed back. Truly loved the series.

Amazing music. I worked in hospitals for many years, and that music was perfect. Will always be identified with this great show.

It's August 2022....just hearing the soundtrack as the camera fades out...nostalgic.... what an amazing series. And seeing Noah Wyle return to the series and be one of the original stars in the final... great full circle... awesome ending reflecting what the show was about... the doctors....the patients... the emergency room with all hands on deck!

It's probably the only series I watched from the begin to its end. In Italy it arrived six months late, and when Internet started to spread I had to avoid its official website to prevent the plot from being spoilt. I found it an appropriate finale, "the show is over but they still work to save your lives, we only have showed you 15 years of their life which doesn't end with the final scene".

I spent years working in an ER. This scene was perfect! The "show" never ends.

The show ended about 14 years ago...and it's still better than any Polish TV show nowadays.. They did an INCREDIBLE work!

And it’s sad that Michael Crichton never saw this air before he died on November 4th 2008. Series finale aired April 2nd 2009..

First time Rachel is called "Dr. Greene", and it was by Dr. Carter. Love the symmetry.

It also echoes back to the very first episode where the nurse pokes her head in the room where Mark is napping and says "Dr. Greene, you have a patient".

Hearing the music again....goosebumps. Best groundbreaking medical drama bar none.

This scene chokes me up every time when they start playing the old theme. I hated seeing my favorite show end, but at least ER went out in grand fashion.

I hadn't seen this finale completely but seeing the ending gave me feels listening to the music. I used to watch this show on TNT. I remember there being distinctively a ER like series preimiering afterwards on TNT. When I hear the music I know it is for that show.

I love how the show went full circle. Dr. Greene teaches Dr. Carter ("you set the tone").... then Dr Carter teaches Dr. Greene. Beautiful!!! Absolute Genius!!

With the theme playing and the camera zooming out one thought came to my mind and thats everyone will always will be in good hands at this hospital. Awesome way to end this series. This show had fantastic characters from start to finish.

My favorite finales are the ones that aren't "happy endings" but ones where life goes on. Yes the series may be over but this world is still living and breathing.

The hospital finally gets the recognition it deserves for being as much a character of the show as any of the actors...

I just noticed on rewatching this clip that Carter's patient is the one with burns to over 90% of his body. This is the same type of patient Carter had in the premiere that made him feel sick and have to go outside to catch his breath. That's when Dr. Greene had walked out to offer encouragement and tell him that there are 2 types of doctors - those who get rid of their emotions and those who work through them. He also told Carter that "treating them is more important than how we feel."

Still one of the best shows ever on Television, great writing and the acting ensemble terrific. It barely got made in the beginning but went on to be a classic.

I have been working in hospitals since 1994, My day is always filled with drama so I was hesitant to watch ER but the great acting and storytelling always sucked me in. Being here in Hawaii it was a poignant moment when Dr Green passed. As a kid I watched MASH, I think I was lucky to watch 2 of the greatest TV dramas in history.

Did anyone else come here looking for the "I am the chosen one. I am Lugia." line?

This was the best medical show ever on tv. All the others, as good as they were pale in comparison. All the actors stood out in their own way.

Sure miss this fantastic show. Such emotion. Miss the Wonderful amazing actors. The characters were awesome. Such talent

I loved this show. My mother was an emergency room nurse. Many of her stories were much like those shown in this wonderful series.

I miss ER...This has and always will be my favorite show ever...I feel teary eyed right now LOL....:[
