    As a 30 year gout sufferer (71 YO), I found the following: Gout can be in any joint of the body - however most prevalent in the big toe. I had one attack in neck, many others in various joints. MD treatment and pharma meds on a daily basis did not stop my gout attacks. Gave up this strategy after about 3 years. Avoiding high purine foods and alcohol did not stop my attacks. Exercise and natural remedies helped more than any of the above. My daily cocktail was and still is: Dark cherry juice, turmeric, ACV, celery seed, fresh lemon juice w/pulp, cinnamon, black pepper. Even with the above cocktail the attacks become more frequent with age. Keto diet stopped all pain and swelling in 10 days. After that I switched from Keto to High Fat/Low Carb diet. Now, after 4 months, gout and related symptoms are still gone. I eat many foods that are on the DO NOT EAT if you have gout lists. Asparagus, mushrooms, pork, bacon, red meats, etc. Still drink some beer but in lesser amounts and it's low carb. Weight has decreased by 13 lbs. (now at ideal weight for my height) Blood pressure has decreased from ~150/78 to 120/71. Sleep is much better..... SUGAR is the ENEMY.

    what are your blood uric acid levels? I don't have gout but my UA levels are on the high side 7-8.5ish most days. Been on low carb diet since July of this year.

    @@mattolsson8816 I have no idea of my uric acid or cholesterol readings. I have not been to a medical doctor for over 10 years. What I do know is that BP and weight both came down to ideal. Gout attacks decreased (I have had one gout attack since Dec 2020.)

    I'm not a quitter but I couldn't go through with the keto diet because it made me feel awful. I then tried the Agoge diet and I lost so much fat, and most importantly no more cellulite.

    SO, I had my first carnivore meal, consisting of a Ribeye steak (medium rare, from my local butcher), six scrambled eggs, Kerrygold butter, and water with a teaspoon of Solima MCT oil. Immediately felt super-charged, followed immediately by gout onset symptoms in my ankles, wrists, one of my toes of my right foot, an the bridge of both feet. Needless to say, this scares the crap out of me.

    According to Dr David Perlmutter, it's the consumption of fructose which is primarily responsible for driving uric acid production. So avoid sugars to avoid gout. I went keto a year ago. As Dr Cywes said might happen I had the mother of all gout attacks a month in. Absolutely nothing since.

    1000x yes and describes by experience exactly. From chronic gouty attacks to No Gout. Took about a year but my God my life has improved by eating Keto/ Carnvore. Nice side effect was 95lbs lost and kept off.

    @@AmericanRoads it was still >8 as of 2022. I need to check it again but I believe it will take more time to get say sub 5. The attacks got very rare but when they happened it was so bad….therefore I went on Allo.

    Is this still the case since you posted this? Do you consume high oxalate foods? No problems from oxalates? Do you take potassium or electrolytes? Especially when carnivore? How much salt do you consume? Thanks!

    Thank you so much for posting this Gout info. A dear friend has suffered pure hell the last 4 or 5 months after starting diuretics for leg swelling. Unfortunately none of the doctors or tests know about gout unless it is the big toe. She has unbelievable pain in almost every other joint. Last week I found the diuretic and gout connection on the MayoClinic website and then it all made sense. For some people KZread videos and solutions don't compare to a trusted hands on local doctor and they suffer needlessley. Without youtube videos I'd still be using a walker for my Parkinson's which you would never know I have a year later after watching youtube I started B1 and Carnivore have me aging in reverse. I'm sure others are watching friends and family suffer because they believe what we have been told all out lives. I'd like to see youtube offer support help to those of us who are labeled complete nutcases for finding solutions your doctor doesn't know.

    I HAD gout, kidney stones and was a fat pre-diabetic guy. In my mid forties they all ganged up on me...I have never drank alcohol, but ate a really really high carb diet my whole life. I lost “some” weight (the HARD way) by cutting calories but my gout and kidney stones would still randomly appear without warning...very frustrating. In my early fifties I went “Atkins” low carb and lost a lot more weight and my overall health got better and my gout/stones slowly went away, now I’m 59, cannot remember last time I’ve had a stone or a gout attack (6-7 years at least). My metabolism is now much better, but it was a long journey to realize I “now” cannot tolerant carbs from a lifetime of eating a high carb diet. Basically I eat keto, meats and green veggies and nuts, last year my A1c was 5.3 and about 10 lbs from my target weight...originally went low carb to just to lose weight, health/metabolism was a bonus,

    What greens do you eat? Spinach makes my gout worse. I'm on carnivore right now for about 2 weeks and no flares or attacks yet. Still another 4 weeks til I fully get flushed. But I don't like just eating meat all the time.

    @@grimakastatik Sorry to hear about your gout issues. Gout runs deep in my family and it may take a while to turn your metabolism around. Also the joint damage done during the attacks probably will not ever completely go away. I am mainly a green salad eater (and yes spinach is one of my favorites) along with cooked cauliflower, broccoli, and smaller amounts of green beans, zucchini and yellow squash. I eat too way many onions…love fajitas!

    I got rid of mine by going Keto My doc could not believe it but it’s been gone for two years and even the residual pain from big toe joint is now gone too .... no more meds

    I’ve been on keto for a few months now and I’m not doing so well. I still get a bit of inflammation but the dehydration is off the scale. I wake up feeling like complete shit. I’m thinking of going carnivore

    Absolutely exacerbated by hyperinsulinemia! My gout began two years before I was diagnosed with NIDDM. I had profoundly painful, gouty big toes, ankles, & knees. After I began a ketogenic diet, jettisoned carbs my insulin & glucose levels are approaching normal, my gout flares have ceased.

    Good info. I really got hit hard at about the 2 and a half month mark. inflammation and pain in hands, wrists, feet that crippled me. I had not experienced gout before and this felt like a hard stop to continuing. I had seen so many great benefits from carnivore to that point that I was crushed that the anti-inflammatory diet was knocking me on my butt with inflammation. I suspect that, in my enthusiastic cold turkey leap to carnivore I was overdoing it and my body told me in no uncertain terms I needed to slow it down and let the system catch up. I have options in the carnivore diet to back off a bit so no need to stop the progress and will see how things develop. One thing is for sure - the system has a very emphatic way of letting you know that things are not quite right. All in all, after this painful flare-up I wish the system had a better way of doing it - a little red flashing light or even a flatulence-fueled trumpet alert.

    I am prone to gout, hugely improved since adopting a keto woe. I still however get the occasional flare up and seem to get it going into ketosis. I have measured with a home uric acid meter short sharp increases in auric acid when going into ketosis. These sharp increases seem to trigger a short gout attack. I typically am keto but am always low carb so go in and out of measured ketosis. I need to take allopurinol to control it. Dehydration also plays a huge role which also happens to me when I am not keeping fluids up when ketogenic. Still have weight to loose. Nasty stuff this gout is but thankfully the combo of medication and keto are is working for me.

    One thing I discovered after I had a severe gout attack during a 3 day fast shortly after I began Keto, our body gives priority to eliminating ketones over uric acid. Ketone elimination is critical to prevent ketoacidosis so uric acid can build up during high ketone production as in fasting. I found this out from a diabetic study done decades ago. I love researching for answers. My body seems to have adjusted to being in ketosis, I no longer have any arthritis or gout attacks now.

    Hi Richard I am on the ketogenic diet for 4mths + I did a 108 hr fast and end up with my fingers had red flares on them and I do Omad and since saw it flared on my hands not really painful but did not know what cause it .. can you tell me what study you saw it in please.. I really saw my ketones went up to 6.9 also 7 and then to HI on my keto mojo ..thanks for your reply

    Thank you Richard Mathews for shredding light on this subject I search for information on when ketones are high what’s happening in the body or what side effects and there was nothing my ketones went up to 7 to HI and my glucose was 43 mg /dl for 2 days straight and I felt fine .. it was through a comment that I read (that’s why I read comments) that I found out Fasting /Autophagy side effect of long fast is gout .. so thank you

    I’ve been fasting for a while and lost a lot of weight and now I’m getting flare ups all the time now. It’s annoying. Just stopped fasting like 2 days ago because the flare ups got worst.

    @@Havik714 I found that drinking several servings of a high quality electrolyte with a large amount of potassium over several days "flushed: out the joints, uric acid crystals, oxalate crystals cause joint pain and it worked well for me.

    Unfortunately you have just described my entire medical history for the last 10 years. Including my LCHF/keto history for the last 5 months. I have osteo arthritis of the hips with the joints of a 60 year old. I'm 55. If my doc said at the first diagnosis of gout not that "youve got hypertension" but "you've got insulin resistance" this might have been avoided. Nice one Doc.

    And it’s an incredible pain I have high pain threshold but man it’s like a knife in the joint on scale of 1 to 10 it’s a 12 and it’s always starts in the left big toe lol

    My gout now goes to mostly rightside arm, elbow, wrist and knee. Top of left foot. My covid vaccine second shot triggered the worst knee episode ever and lasted three weeks. I cannot tolerate the gout meds, prednisone is what I have to do. I hate how it makes me feel. I had decided to be on a very strict Keto this past two weeks, measuring daily, also measuring glucose. I watch Dr Bosworth also. I think this week I was in 1.5 (5?) ketosis most of time. Thinking maybe ketones are triggering my gout. I have had gout this year, and been detoxing the steroids 50% of the time. Gout in past was coming every two months generally. I am getting hopeless. This discussion helps.

    Actually, this was so helpful to me. I was so desperate to get my body in a better place because of my failing health that I decided to give carnivore diet a shot. After 2 weeks of egg white bread and meats I ended up in the hospital unable to walk on my right foot. The pain was worse than my nerve pain from neuropathy. They didn't do any tests other than an x-ray and went by the uric acid from lab tests from 15 months ago. I was so discouraged that I stopped the diet. I will give it another go, I bought Milk Thistle for my non alcholic fatty liver and R-ala for my neuropathy and berberine for my blood sugar. Getting back on the horse.

    I'm living a Keto-Carnivour lifestyle now for 15 weeks...lost 34lbs, just had a mild flare up after TUNA consumption...usually eat Cow...however, I have been off of all gout meds, and most of my Hypertension meds too! Well on my way, I need to be more patient I guess!

    I had gouty symptoms when I began keto, and I've been given to believe that's not uncommon. It was only temporary, but the uric acid thing one of those things a non-keto-friendly doctor will possibly freak out about, as mine did. Mine attributed it to the red meat. Other issues came along, and I ended up switching to Denver's Diet Doctor Jeffry Gerber.

    ive been on a carnivore heavy diet for a few months, had a relapse over the weekend, several beers, and capped off the wknd with some carb heavy snacks, and suffered a gout attack. feels to me like purines in foods are the fuel and the sugar and carbohydrate heavy binge session are the match. my gout always comes back when i fall off the wagon and indulge in heavy carbohydrate intake.

    Wonderful video! I have a couple of questions. I am a 41-year-old male, approximately 170 lb, I exercise and eat healthy. I keep getting gout flare-ups, and I think it's related to a lower carb diet. I do not follow a strict keto diet because I have cortisol issues that I've never been able to resolve related to a nr3c1 gene, not as important, however I now believe that after watching this video, the gout flare-ups could be a combination of things, but more importantly that I take propranolol. Could this be the cause of my gout? Because I've been confused how I can be getting gout, if I'm not even in ketosis, I.e eating a severely low carb diet or consuming tons of fat/protein. I have added whey protein to my nutrition, however I still don't think that could be causing it by itself. I do have type 2 diabetes in the family, so it could be a perfect storm kind of situation. Are there any supplements or things I could add to help remove the uric acid? I read somewhere that exogenous ketones might help. Your thoughts? Thank you for reading this.

    Great info Dr.C...I'm mostly ketovore, but my triglycerides are close to 1000 at times and uric acid levels are 10.5, my question is is it possible that I could have a gene problem where my liver is dysfunctional in all the mechanisms of processing fat, purine etc ?...oh and my A1Cs are around 5.2

    Re "broken liver": What blood tests should be taken to assess liver function, either to eliminate or confirm its involvement in causing high uric acid levels? May we presume the lower end of the range of "normal" values is preferred?

    Is the medication you refer to called Colchicine? Closed captions show the name as culture scene. Just want to make sure. Suffering from gout in my big toe, as I write this; first time I ever had gout and certainly don't want to have it again. Pain is excruciating however Advil every 6 hours plus an ice water bath for foot is helping. It started yesterday and hope it goes away soon.

    How long did yours last? My first attack and I’m going on one week. I was prescribed colchicine. Putting on ice, elevating, drinking a gallon of water a day, tart cherry juice, cautious of what I’m eating. Did you see a rheumatologist?

    Dr. Pete says heat is more helpful for gout. Currently having my 1st episode. Day 3-tried ice 1st day then doing warm soaks, shower and heating pad. Using IBU 1-2 X/day. Still inflammed but can bear weight. LCHF/KETO/CARN 3 yrs and this is my 1st. Had 2 meals that included pepperoni. Processed meat-not good per Dr Pete. He has YT CHANNEL.

    Thank you so much for doing this video! I've been hoping you would do one. I had my first gout attack a year ago and went low carb when my PA suggested I reduce my fructose intake as a possible remedy. I had another significant gout attack this past March when I went full-blown keto (exactly as you said). I haven't had much luck with colchicine (and didn't want to go on allopurinol), BUT I have had amazing results with alpha lipoic acid. In your opinion, is this a safe alternative (at 1800 mg per day)? I'm guessing I'm not fat adapted yet, as I can feel the tingle in my toe when I get lax on my ALA.

    Great video. What about CPPD (pseudo gout) ? I am young and developed CPPD in my hands. First flare happened right before I was diagnosed with Normocalcemic Primary Hyperparathyroidism. Second flare, a few months after Parathyroidectomy. (2 adenomas were removed). Would Keto help? It’s a very rare and painful condition. I have limited range of motion

    ANOTHER EXCELLENT VIDEO! I found that HYDRATION is key. But how does one get rid of old crystals (non-surgically) that are still in the body after being very low-carb keto/almost carnivore for many months? Those old crystals can flare up sometimes and it is grueling.

    Alkalise your blood. You are on the right track with the ACV. Add some quality lemon juice to that. Alkalising your blood is the only way that i have learned helps rid your joints of uric acid crystals. I also take magnesium citrate and potassium citrate for the same effect. Good health to you!

    Great video, Dr. Cywes. Can you tell me what causes cramps throughout the body. I'm doing carnivore since Aug 24th and just lately I've been having severe cramps throughout my body! I started taking Keto Chow electrolytes wondering if that's it. I try and drink a lot of water during the day. Also, I'm a type 1 diabetic. Is there something I'm doing wrong? I also have an appt with you on Dec 1st via phone. I'm in California. Thank you and all the best. . . . .

    I was given coltrazine a few months ago and had the worst diarrhea I have ever had in my life and was very sick. I told my doctor he should be horsewhipped for prescribing it.

    I need to know this too. Been on strict carnivore for 3 months and the latest blood test shows a rise in my uric acid even tho I'm on 300 mg Allopurinol a day.

    I've had gout my entire adult life (30+ years). I've tried for many years to control it with diet (including keto) with no appreciable results. Unfortunately, after 3 decades, I had to throw in the towel - the only thing that helps keep my uric acid in check is allopurinol. I hate having to take it, but the alternative is damaged joints, as it was progressing from my toes to my knees and elbows. I think that maybe (?) there is a genetic predisposition to gout in some people? FWIW, I've lost 35 lbs on keto, and I don't drink alcohol or eat high purine foods (shellfish, seafood, organ meats, beans, etc.). I even avoid foods that have organ meats in them, like sausages.

    I suspect that to be the case. I remember my grandfather having gout, too. Keto has been a great success for me (lost 35 lbs, triglycerides at 57, glucose at 86, etc.), but it just wasn't any help for my gout. Before I went on allopurinol, my uric acid was consistently around 8, no matter what I did.

    There is a genetic gout and gout brought on by diet. My doctor made this clear to me. No matter what I do I have to take allo. It's genetic for me. Diet and exercise helps keep it at bay but I still have it.

    I experience a pounding sensation on my 3rd toe, left leg! Had an x-ray and its called idiopathic toe neuroma, I am getting injections to reduce the inflammation, do you think a gout diet, as you're suggesting, would help me!

    YES, an otherwise healthy person can get debilitating sudden gout in the foot and elsewhere. Unintentional autophagy. Get sick -- TOO sick to eat for almost two weeks -- while also NOT drinking enough water. Severe dehydration combined with unintentional extended fasting.

    Interesting discussion on another YT re notion of dehydration, interview with marathon runner doctor....there is such a thing as too much water because we pee out important minerals like potassium....and it is the loss of minerals can causing more harm...I believe his advice was drink when thirsty, and not the standard 8+++ daily glasses.....athletes are bit different, as are diabetics who feel thirsty from insulin response

    Thank goodness I do not get gout. Not before keto, and also not since keto. My brothers not that lucky! The one brother actually gets gout from eating venison meat. (and also obviously from red wine).

    I never had gout or a flare of uric acid I am on a keto /carnivore diet for 4 mths went on a 4day fast and came out of it with red fares on my fingers near the joints and on top of my hands .. can you recommend something please Dr

    Stay the course. Ketones and uric acid compete. With times ketones get used better allowing more uric to leave the system thru urine

    Fruits for gout👇 Eat fruits high in vitamin C such as oranges, tangerines, papaya and cherries. Apples, pears, pineapples, avocados are low-purine fruits and therefore can be eaten in moderation. Drink at least 8 glasses of water every day unless you are advised to restrict your fluid intake due to medical reasons.👍

    That's nonsense. The sugar in the fruit makes the vitamin C ineffective. Avocados are good, because they don't have a lot of sugars. Otherwise, vegetables are where you want to get your vitamin C.

    I completely disagree. I'm pretty sure that the fructose I got from the fruit I was consuming was what caused my first true gout flare. I'd rather give birth than go through another one, so I'll pass on the fruit and keep going with keto.
