Best explanation I have heard so far about multifocal iol and blended vision. I have comorbidities in the dominant eye and multifocal is not recommended. The other eye has epiretinal membrane and probably a monofocal iol for near vision. The doctor did not tell me what kind he put in. Probably didn't think it was worth the time and effort to educate me. Thanks for the video sir.

Great video! Based on my research I am considering a PanOptix in one eye and a Vivity IOL in the other eye. I have significant astigmatism, ketococonus in the eye I am considering the Vivity IOL besides presbyopia in both eyes. This video explained why the lost in image detail can occur when both lenses are the same.

This video was so helpful. I just had my first eye done last week at the age of 43, hence plenty more working years in front of the laptop to go, so the lens I selected (mono-vision) was really important. Hopefully I can get the enhanced version in 20 years.

I just had cataract surgery on my left eye , the dominate one. It was for distance and will have the other for closer vision. I was confused when you said it was difficult or didn't work well (this monovision) . your video was a bit lengthy and I do prefer when speakers can be succinct, as it makes it easier for a layperson to understand. I did have Lasik years ago and that worked wonderfully, and I believe it was a combination, near and far. I could shoot and nail a bullseye (with recurve bow) and could read quite well very small print, but that went away after a while. My doc said this procedure would be similar. Perhaps I misunderstood your video as I am tired and have to get ready for post op. thanks for your video and enthusiasm. Peter

Thanks. Nice presentation. Presently, for the past 3 years, I have had one monofocal distance lens in one eye and now I am going for a - 1.50 in my other, near, eye. I'm hoping it goes well and that IF it needs tweaking with a laser that the doc can do that.

You just helped me so much! I have 20/10 corrected distance vision and little microscopes for eyes when viewing close without my glasses…but, I have cataracts. I embroider (very fine detail) and sew and bird watch. Close and distance vision necessary for these activities. I’ve been putting off cataract surgery because I’m not willing to lose distance or close vision. Problem now is I really can’t drive at night due to the cataracts. I hope I can find a surgeon who is willing to work with me on blended lenses. The place I went to originally treated patients like they were on conveyer belts. BTW, I’m 60.

This is a very helpful video among a sea of videos that emphasize multifocal IOLs. For a variety of reasons, I suspect multifocal won't be the best option for me, but I thought I would be stuck with a mono-vision approach - which I haven't been able to achieve well via contacts. And so the blended option might be the ticket. Thank you for the great explanation and rationale.

@@keilana6 multifocal lenses show multiple images to your eye at once, so you will have fuzzy vision, halos and glare and worse contrast. The lens you got from medicare has the absolute best image quality even though it has less range. Lenses extending your range ALWAYS have a catch, as the amount of light entering the eye is constant and when a lens starts to split it around, you will lose image quality and tend to have all kinds of side-effects. So you didn't "lose" anything, but likely gained stupendous quality and color in your image, but you need to wear glasses.

@@rhett7716 The only guidance i have is that if you're comfortable with monovision and understand what you're up for, i don't have any opinion one way or another. Many people are happy with it. I've always had emmetropic vision so I prefer having my eyes work in a pair for great depth perception.

So what if I only have one eye with the cataract and the other eye is clear with 20/20 both far and upclose. Would getting a monofocal lense in one eye set for distance create a blend and would that work to allow for no glasses?

Blended is also called micro mono vision correct?

Marketing tends to always tell you what the benefits of non-monofocal lenses are, but they don't explain to people what the COST for that benefit is. Always remember to ask what's the catch because there always is one. Monofocals have the best possible quality, contrast and color but the least range. To get range, you need to give up quality of vision, contrast and color, potentially also suffer from ghosting, glare and halos if you select a multi-focal diffractive lenses.

What do you suggest I do if the Drunken Sailor, AKA, surgeon put the wrong lens in.

What about monovision?

is blended bet .50 & .75 difference between eyes. Is 1 diopter stll blended and tolerable if never did mono contact lens. What is most can achieve with -2 in one eye and -1.50 or -1.25 in second eye. Trying to get as much distance without losing all close-up . Moderate myope only-2.50 & -3.25. Don;t want multi focals but torn which way to go. Want to be more glasses free but is it good idea for myope to lose the ability to take off glasses to read if want to. If people go distance with -.50 is it hit or miss is they can see computer at about 24 inches or so and does that mean glasses before 24 to see anything clearly and what is average size font can be read Do you have to hold arm out 24 to read or would it be closer. Don't want to be reader dependent on and off but with near/intermediate is it then glasses on off to see distance? Anyone ever do intermediate. . Is there a combo of -1.25 and 1.50 -what kind of vision do you get more or less with that. thanks

Blended vision seems like it could be experimental as to how to blend the two lens selections?

I had one eye cataract removal / lens implant set for distance (this was required due to a cataract following an eye procedure). Other eye recommended for cataract removal / implant at some point. I have nearsightedness in this eye (wear a contact lens). My optometrist gave me a contact set for a closer range vision to help me try to decide which distance to set that implant for. However, your natural eye still adjusts focus; I would still suggest having your optometrist give you a contact to aid in experimentation before it’s irreversible.

ive been monocular for 27 years of my 42. My question is: Being monocular, do you have an opinion on what is best for me? i am really scared to be "sold" on a designer premium option from people who aren't familiar with my situation. i am leaning towards Vivity based on my limited research (i just started today) Any help would be GREATLY appreciated. thanks.

I just got some mono focal lenses and I was told that I was going to need glasses for reading. But a day after the surgery I realized I can see Distance with no problems but I can also read messages on my cell, write messages, work on the computer, without wearing glasses. For very small print I will need them but I'm in no rush. I thought I wasn't going to be able to work on my computer.

@@Umearrr As a resident of Ontario, Canada, I could get for free. the basic mono-focal lenses on both eyes. I was offered multi-focal lenses for $3,000 which would allow me to never have to wear eye-glasses, unless for very small print. I thought about it and I then decided to go for the basic mono-focal lenses, not so much for the price but because it's a simpler lens to install. All I wanted was to be able to see distance and not have to wear glasses outside, especially during winter. I left the clinic and I could immediately see distance and could read. At my checkup visit the doctor was surprised about my sight and told me that the patients who chose mono-focal (distance) couldn't see as well for reading. I now feel like I have multi-focal but for the price of mono-focal. Even if I needed glasses for reading and for doing computer work I would still be very happy with my choice.

Hey thank you. But I did my cataracts a year ago both eyes. I se well and I am ok except that they say I have early signs of macular degeneration And hopefully we can I hope regulate this to not go further 🙏🌹

@menamarybaldeosingh618: To boost the strength of the macula, take lots of vitamin C - and not the acsorbic acid kind which is just made from corn. Take vitamin C from Camu Canu, or eat loads of vitamin C-rich foods. There are also a few very necessary eye-health vitamins additionally like lutein and zeaxanthin which you can Google and which are very important to take also.

So would the AcrySofIQ Vivity be a blended lens. I am 65 years old and I am seriously considering this option. I understand this would cost about $3000 an eye, but I think it will be money well spent. I am very dependent on my glasses now which have a power of about 9.0. Thank you for all your good information

I am in the same situation as you and need to make a decision in three days so my lenses can be ordered prior to my November 8th cataract surgery. I am considering monovision or the Vivity lenses. I have had monovision Lasik for about 15 years and love it, which is making me lean more toward the monovision. I haven’t worn glasses the whole time since my Lasik. I want to make the best choice, and am looking for reasons why one would be benefit me more than the other. Not clear cut info out there.

Vivity is excellent in a blended vision approach - works well for me. The non dominant eye is just set to be ~0.5 diopter near sighted and the dominant eye was set for distance. But the far eye sees well from intermediate to fan and the other from near so not quite far but the 2 eyes work well together.

Doctor, question please: I have had cataract surgery in my R eye for distance and it has worked giving me better distance vision. Next, I am doing a contact lens trail for short distance in my L eye to see if Monovision will work. If I then go ahead with cataract surgery for near distance in the L eye, (which has a more minor cataract compared to the right eye but I prefer to get this done now rather than later in life) but if after surgery I find I can't focus and see the detail on my computer screen for my work, will glasses help in this situation? Also, can I get glasses that will enhance distance vision for when I am driving?

I elected for blended vision, with a focal distance of 700mm in the near eye. I only require reading glasses when reading at night in poor light. I thought I was being clever by ordering reading glasses that are +2D for the distance eye, and +1D for the near eye. I find this to be very weird, possibly because glasses not only change the focal point, but also the image size. Over time the brain will adjust for this, but if you only wear the glasses for less than an hour a day, it is not a good idea - no warning about this phenomenon anywhere. I am far happier with just cheap +2D lenses for both eyes when reading in poor light.

My doctor doesn’t recommend me in the multifocal lens he wants to put me in a toric lens so I’ll still have to get reading glasses

you can get toric versions of multifocal lenses as well. Also EDOF lenses like vivity have toric versions.

I could NOT do blended because I had an OLD retinal tear/detachment... AND a ctaract that actually looked like a ROCK..

@@Itsme-ni9jk It's been over 9 months and my right (good) eye with the panoptix lens is STiLL not clear.. left eye no change-- aafter 5 surgeries.. and 2 more to go.. although doc says it looks good- there ia "film" like a secondary cataract over it-tht will need to be lasered out- (which is fine- I'm getting use to it now).. so- won't know for several MORE months yet probably..

@@Itsme-ni9jk NO- the cataract came out EASILY. He had to do it the old fashioned way and made a bigger slit- but there was no scraping or anything. THAT was easy-- t's the detached retina that causes the issues-- the cataract was NOTHINg.. NEVER ever felt a thing. The panoptix eye is finally beginning to adjust- the lens has several "rings" that each is a different distance vision- and the BRAIN has to 'get use to " seeing whichever it needs. that's what's slowing me down--because of other issues. The retina worked gret- but he has to go in to remove scar tissue and "change the oil" ( I have silicon oil in mine due to NOT being able to lie in ONE position long enough. ) There has been NO pain with ANY of the procedures- before, during or after- the healing is slow because the retina detachment was 5 years old... sort of experimental. if you're having cataract surgery- that's a piece of cake- and if the cataract is ALL you need- the healing would be like 7 DAYS-- mine was a lot more complicated causing the slow healing.'

Does it perhaps take the BRAIN a while to LEARN how to see through BOTH???? I've had mine for 3 weeks and my car dashboard and stuff are perfect- but anything past the hood of the car is blurred... and it varies.. sometimes- clear-- sometimes NOT.. so maybe that's brain adjustment??

@@anassyria5176 It's getting BETTER every day- and I found out that if I RELAX and don't "try" to see-- it's better. Old habits die hard, and I find myself squinting and "trying" to focus- and when i stop that and relax- it WORKS.

@@anassyria5176 I don't know for sure, but I watched videos on here and listend to tons of testimonies- and they are all saying the new VIVITY lens is WAY better than even the panoptix-- the actual testing shows a 40% improvement- with MUCH less "learning curve"... but it wasn't available where I went.. OH well- Maybe later I'll switch to it- you CAN have the lens changed ONCE..

Another thing I'm still looking into is the Crystalens, which may be able to utilize my natural healthy focusing ability since I'm 31 and don't need to wear any glasses.. Is blended and mini-mono the same?

How is a human supposed to be OK with having a human lens replaced with a piece of plastic sitting in your eye that sits in your head? This surgery is Concerning. Why don't we have an alternative option to treat cataracts? That is a question no one seems to answer.

There's no way to clear up a cloudy natural lens, so there's no other option other than to remove and replace it. And in many patients, even if their lens isn't cloudy yet, they may still opt ro remove it in order to get great vision without needing glasses or contacts.
