The play is mainly set in the court of the Western Han Dynasty, with Wang Zhengjun, the longest-lived queen in Chinese history, as the main plot. It describes the life of the harem in the feudal society of the Western Han Dynasty, where people were fighting for power. During the reign of Emperor Xuan of the Western Han Dynasty, Lady Wang and Lady Chunyu were both female doctors in the harem. Later, they were exploited, and one committed suicide and the other was beheaded. Lady Chunyu's daughter Chunyu Yao witnessed the tragic deaths of her mother and sister, and thus planted the seeds of hatred for Lady Wang's daughter Wang Zhengjun.
