An 82 year old male presented with dyspnea on exertion for last one year. Try to spot the findings in these echocardiography clips and make a diagnosis. Do calculations with the provided data. Answer will be explained in latter half of the video.
Case 101: FINDINGS:
PLAX and PSAX view showing thickened and calcified aortic valve leaflets with restricted opening. CW Doppler showing high gradients and velocity across Aortic valve suggestive of severe Aortic stenosis. PW doppler with LVOT measurements all put together in continuity equation to calculate valve area which came out to be 0.8 cm2 in this case (Severe AS). There is an additional finding of plaque in the abdominal aorta.
AVA calculation by Continuity Equation:
AVA = CSA LVOT * VTI LVOT / VTI AV
CSA = πr2 = 3.14 * (1.22)2 = 4.6
AVA = 4.6 * 21.1/110
AVA= 0.8 cm2
