I am a Civil Engineer myself. I would suggest you to straight go for IELTS but have any job which pays you at least your IELTS fee, and Application fees etc. And start applying everywhere where you see opportunity.

I would suggest you go for second chance if you want to pursue MBBS. MBBS is very expensive worldwide. Some countries like china, Ukraine, and Russia are a bit cheap with partial scholarships. I will make a detailed video. But you can also change your field towards biotechnology
