@@queencysamuel B/B/B/C+ It went marginally better than IELTS, I still got under the B on writing required by the GMC :( ... weirdly enough, the GMC webpage does not provide an option for "C+" which appears to be a new possible result, so I filled it anyway as B/B/B/B and hope for the best (for me, C+ is not C) given the current need of health providers they might be more lenient with a C+, who knows

how can one arrange these in an Ideal order while writing 1.description 2.explanation 3.recount 4.classification 5.comparison 6.recommendation tell me if its ok to write in this manner.

can i look on the case notes while writing?
He was administered Valium and taken to River Valley Centre. Mr. Smith first presented at emergency two weeks ago. By the time he arrived, he was much calmer and his behavior had normalized. He was apparently suffering a psychotic episode.
is there a themeplate for writing?

Depends on the patient circumstances. For instance, you could use the chronological order for a normal referral where introduction would be followed by past medical and social history and then development of patient case (A to Z). Whereas, in case of an emergency letter, your introduction must immediately be followed by the current situation.
