    Thank you Dr. Gillard for sharing your knowledgeable presentations. I'm sure many people are very grateful for the details you share. Although some may not recognize or understand specific terms this information can be vital to making important and well defined decisions upon treatment. Many physicians don't go into detail during appointments and leave out vital information for the patient. I'm a detail oriented individual and must know specifics in order to trust my physician.

    I have listened to a lot of your content and I appreciate the way describe everything so perfectly. As an MA I prefer the medical terminology. It's been most helpful to understand the bulging discs on my L5 and Annular tear in my L4.

    Wow, what a great explanation! And the answer that I wanted to hear regarding MRI machines - wehther a 3T one is better than a 1.5T one. Now I am going to find an imaging facility that has a Philips MR 7700 machine

    What happens to your disks if you don't have the lumbar curve? In my MRI I don't have this curve, my spine looks so straight. I just had a L5-S1 micro discectomy. I believe my L5-S1 disk herniated because of my straight spine. Has anyone done any MRI in those MRI devices where you can stand up on your feet? They said your spine has a more natural curve when standing up as opposed to when you lay down.

    @6:15 omg so then when they are doing a discetomy, is this where the scar tissue will form? Is that why the nerves hurt?

    Sir I have centrol and both paracentral disc bulge & posterolateral osteophytosis at L4 /5 level causing thecal sac indention, mild spinal stenosis and bilateral neural foraminal narrowing

    If all conservative care has failed, the treatment is surgery. What type of surgery depends on the cause of the stenosis, but it's typically something called a decompression.

    My MRI showed 3 herniated discs and they want me to go to physical therapy....if the bulging disc is protruding and pushing on nerves to create my pain and siatica, I don't understand how physical therapy can change that.....the discs are not gonna recede back between vertabraes again, so it seems futile....I know ppl who've had a laminectomy and are still in pain after the surgery

    The $64,000 question is whether those discs are actually pain generators or are they completely innocent have been there for years? It is well known that about 30% of disc herniations are not pain generators. The trick is to figure out if these herniations match your clinical findings (neurological and orthopedic findings to be exact). Big disc herniations can be reabsorbed by the body - sometimes completely reabsorbed. And this resorption is done by mother nature, and has nothing to do with any type of treatment you will receive. Maybe some decompression treatment might help mother nature along (there's no solid research to prove that, but it makes sense), but all in all, its all about mother nature. If the sciatica is severe and if you start suffering motor loss, then you could get forced into a discectomy - that's a whole another discussion. If you think that herniations are symptomatic, then you should be seen by a spine surgeon sooner rather than later so you can be properly worked up. Discectomy is a time sensitive thing for the most part, so you don't want to wait around too long. The rule of thumb is about four months. Note, this information is for educational purposes only.     Hello, I actually just had a MRI done on myself with one series (T2-weighted) of axial and sagittal's done with 2 mm cuts, down the plane of the disc. It took 50 minutes and now I have beautiful crystal clear images from this 3 Tesla tube. Too many MRI centers are rushing people through their machines and getting mediocre to poor quality imaging - I see these every single week, something that I don't see in other countries.
