Even starting locations and throne positions will really help balance multiplayer games. I've finished ~6 mp games of dom 6, some mapnuke and some generic maps. Every map has been wildly different, mostly due to various cave settings. Personally I think caves should be treated similar to UW in MP. Cave nations often need them, and it is overall an advantage to have, just like a pond. So make them small, around 9 provinces, and make sure the overall # of provinces for each player is balanced with caves included.
