How severe is President Trump’s COVID illness?
Does he have COVID Pneumonia?
As a pulmonary & critical care doctor, I’m watching President Trump's medical team, including Dr. Sean Conley, give a press conference. His medical team includes 3 pulmonary & critical care doctors, 2 infectious disease doctors, & more. Unfortunately, Dr. Conley is choosing to withhold certain medical details.
So in this video, I will take what he is saying, with what information is being given, & do my best to read between the lines so that people can have a real understanding of what is going on here with President Trump.
We want to know, does he have COVID pneumonia?
And I will also try to classify the severity of his COVID illness. Then, at the end of this video, I will discuss what this means for President Trump’s health moving forward.
What constitutes mild illness, vs. moderate illness, vs. severe illness, vs. critical illness is still very much a grey area. Generally speaking, someone who has symptoms but is not short of breath, & does not have low oxygen levels, is mild COVID. Moderate illness is characterized by shortness of breath and/or lower oxygen levels. Evidence of viral pneumonia is seen on chest x-ray or CT scan of the lungs or even ultrasound of the lungs. Severe illness is characterized by someone who is having trouble breathing & consistently requires extra oxygen & or ventilatory support.
Then there is a critical illness when patients have either ARDS or hypotension & require either high-flow oxygen or mechanical ventilation. Defining the severity of illness is hard to do. And this also makes it tricky in terms of what medicine to give. For example, these studies show dexamethasone, a steroid type, reduces mortality in moderate and severe diseases but not mild diseases. But, what defines mild vs. moderate vs. severe disease? The rationale behind giving dexamethasone is that it can suppress inflammation within the body, so the hope is that it will suppress the inflammation within the lungs.
For COVID, we give dexamethasone in either IV or pill form, and it's 6 mg once a day, and we give it for a total duration of 10 days. One of the side effects of steroids is that patient’s blood sugar levels become higher, so hyperglycemia. And that is why we usually have to counteract that by giving patients insulin. And when you give insulin, sometimes the patient’s blood sugar, meaning blood glucose, drops too low, known as hypoglycemia. So when we give dexamethasone, we check blood sugar levels by fingerstick every 4 or 6 hours. At the press conference, they mentioned nothing about this. But if his blood sugars are out of whack, they would not be talking about discharge planning.
But stepping back and looking at this from a bird’s eye view, where does Trump lie on this spectrum of COVID illness. Right now, he is not requiring supplemental oxygen. What would the progression of his illness look like if he were to have clinical deterioration? Well, typically, this entails stepping up his oxygen therapy, so 2 L NC, maybe 3, 4, or 5 L NC. If that is still not enough oxygen, maybe an oxygen face mask or something called a high-flow nasal cannula. These devices provide a lot more oxygen than a nasal cannula.
If these are still not enough, that’s when you start talking about mechanical ventilation. Either non-invasive mechanical ventilation, meaning a Bipap mask, or a bi-level mask. Or the patient might need invasive mechanical ventilation, meaning a breathing tube connected to a breathing machine. Often, these patients require vasopressor support, meaning IV medications are given when the blood pressure is too low. So that would be the sickest of the sick. It’s the patients who are on mechanical ventilation, a breathing tube requiring 100% oxygen. Sometimes though, even that is not enough, so some patients get ECMO, which is extreme. And sometimes, despite us doing all of these things, patients still die. That is just the reality of this disease.
Currently, President Trump has to saturate 98% on room air, which is good. And this is to be believed because you see him in that the video of him; he is not in respiratory distress, not requiring supplemental oxygen, he is not cyanotic. But the important thing to keep in mind is that patients with COVID can look like they are doing fine. They can have an improvement in their symptoms. They can improve their oxygenation. But, within hours, they clinically deteriorate to the point of requiring mechanical ventilation, especially when the illness's inflammatory phase becomes very prominent, meaning the cytokine storm that develops. Typically around 7-10 days.

    JAD I totally agree. That’s why I don’t want to see him discharged no matter how much he wants to be. However, he’s surrounded by yes people unfortunately so I won’t be surprised if he gets out. But given the nature of Covid (yo-yo, wax and wane symptoms and that it tends to really rear it ugly head and kick into high gear in the second week), despite these amazing treatments, he has no business to be discharged at this time. They need to chill and monitor him in hospital for the next week and a half. Besides that it will keep him “behaving” and “resting” somewhat in an isolation fashion which I’m sure if he gets to the Whitehouse he’ll start to back peddle and risk spreading the virus willy nilly throughout the Whitehouse.

    Thanks for the update. I’m not a doctor but I’ve had covid and that’s exactly what Happened to me.first week I was fine but on the 12th day my lips turned purple. This virus would give me false hopes as I sometimes feel that I’ve improved then at night I would feel really bad. I only had fever on day 1 and felt good during the day until the 5th day. So when people are saying Trump is better already I’ll be like I’ll give it 2 weeks it’s either he gets better or his health deteriorates but for now, we really can’t tell

    Difference is that he has received experimental antibodies, remdesivir and now dexa within 72 hrs of his diagnosis from what they have told us. Many people wouldn’t have received all these by the day 1-10/12 of diagnosis if they had mild symptoms

    It doesn't seem like President Trump is totally out of the woods just yet. I do believe that being fever free in the last 24 hours and being off oxygen is a step in the right direction, however.

    As usual a wonderful video Dr. Hansen. I am a cardiac arrest survivor (July 2016) and my oxygen levels hangs around 85 to 87% (tested with the pulse oximeter) in the past few months. I don't have breathing troubles and I feel perfectly normal. I am single living in India (US naturalized citizen) and strictly following the rules of Corona lock downs since March and till date infection free. Should I be concerned about my Oxygen levels?

    Doctors don’t know what our bodies are capable of and try and push drugs. I have the most little respect for doctors. I had doctors push blood pressure meds on me for having elevated blood pressure do to being extremely stressed out. Keep in mind I have no history of heart issues in my family. I would never trust a doctor for my life ever

    IKR, he is the president weather they like it or not. He has full medical team monitoring him unlike regular citizens... It came with the job... But even without the Job, he was rich and already had a private medical staff.. The virus affects people with different levels of symptoms. So we will have to wait and see.. Knowing Trump he might go back to campaigning surrounded in a huge plexiglass box on the stage with a mini-split AC installed...🤣🤣🤣

And note the date and time at the start of your remarks? I'm trying to determine the order of your videos (this is the first I came across and now see you've posted several, but don't know if this is the first, last, or what), and KZread isn't providing the date, let alone the time. In fact, if you can go back and begin numbering your videos in chronological order it'd be appreciated. I'd suggest numbering them at the start of each Title, as YT allows one to edit their Titles.     Being let outside the hospital to take a ride for his fans. I know a doctor would not let that happen especially with his secret service people have to be very close. If his oxygen keeps falling could make them wear oxygen for a while. Your video was great but Trump is not acting sick by leaving the hospital like he did.

    Ok, why were my patients not allowed out of their rooms, except for necessary medical exams, etc. and HE leaves his room and gets in a tightly sealed car with other people, wearing only regular masks ( not N95’s). Their families were not allowed to visit them, btw. Why was he allowed to leave isolation? I think someone needs to be fired.

    First thing he was riding inside the beast the driver and passenger compartment are separate. The information about that vehicle is highly classified also none of us know much about it. Additionally your institutions guidelines were created by functionary with a clipboard who with some other people sit around the table and decided what the guidelines are going to be and everything waiting on the extreme side of caution because your hospital doesn’t want to be sued. And to be perfectly honest you sound a little bit like you hate the president but you don’t really want to tell anybody.

    Not informative from this doctor. He speculates to much about someone he has never examined. Military doctors are so much better, more informed, and compassionate.. skills this doctor needs to learn. Stop speculating & let our President get the rest & treatment he so much deserves. You haven't even considered his very hectic schedule he has done for 3 1/2 + years. Not every symptom is COVID-19 related. Especially when the CDC downgraded this to no worse then the flu or common cold. So stop with the false propaganda.

If the docs gave out more complete details this doc would not need to do his video series! He's doing us a favor. He gives us background info, both the possible mild and severe outcomes. Because he hasn't seen the details he's giving us the multiple possibilities. There's no propaganda here. It's all medical. This video series is not interfering with the "rest & treatment he so much deserves"! COVID has multiple symptoms, including fever, low blood ox, fatigue, and so on, plus a positive lab test. All of these combined result in a COVID diagnosis. Which has resulted in treatment for COVID, such as the experimental stuff he's getting, steroids, supplemental oxygen, and other treatments common to COVID patients. If he didn't have COVID he wouldn't be getting these treatments. Here's another example. Lupus has 14 major symptoms, symptoms common to other diseases or ailments. If someone presents with 4 of them they get a diagnosis of Lupus! (The 4 of 14 was true when my sister was diagnosed years ago; a more specific way to diagnose Lupus may have developed over the years--I haven't followed the research since she died of it.)

    Wow this doctor is very clearly up to speed with all he needs to know in the Intensive Care aspects of medicine...and of Covid 19. I would say without doubt that his patients are most fortunate to have him looking after them. Speaking as fluently and brilliantly as he does, I wonder if he is a teacher to medical students or perhaps a Professor of Medicine. His communication skills are very good.

    You can't get straight answers from Trump's doctors because there is such a thing as patient confidentiality. You wouldn't like it if your doctor told everyone in detail what was wrong for you, it is unethical, so why should Trump's doctors tell you everything? This doctor does not have any actual clinical information on Trump's case, therefore he is only surmising and putting his 'opinion' forward.

    Awesome explanation! This is what needs to be on tv on shows, and he should be allowed to explain it without interruption or misinterpretation. Really like how he explained the expected findings of the CXR or CT of chest. Most of the media outlets had many doctors on that did not address this and went on to say that the medical team left out important information or didn’t answer the lung imaging. They actually did, just without explaining it. Kept waiting for a doctor to say likely found signs of viral pneumonia based on what the medical team responded, but none that I saw did. Keep up the good work!

    I had COVID in June. First week I had no significant symptoms, mostly just tired. Second & third weeks were difficult. It was almost 8 weeks before I recovered completely. Except for taste & smell, I still haven’t gotten that back

    The reporters tried to ask Dr. Conley for clarification on the imaging during questioning. Covid pneumonia characteristically has bilateral infiltrates on xray. I would be more worried if he had this finding.

    When I see COVID-19 patients, and they ask (or if family asks) if a Chest x-ray or CT scan is normal, or unremarkable, all I have to say is yes, it's normal or unremarkable (if it is). If it's not, then I say we see an infiltrate/pneumonia/whatever on there.

    If he was sick enough to be helicoptered to Walter Reed on Friday and was given experimental non FDA approved treatment and if he has pneumonia, is 74 years old, considered overweight, even a layperson knows he should not be discharged as soon as tomorrow, so things aren't making sense , everyone knows Covid-19 patient's conditions can quickly deteriorate, the current administration is losing all credibility. I think the doctors are trying to do a balancing act of providing information about the president's health to the public without providing details to our enemies who might wish to take advantage of the situation.     If you make another video, I hope you comment on how his breathing looked when he got back to the White House. Six of us nurses thought his SOB was beyond what someone obese would have from simply climbing the stairs. We assessed rapid respiratory rate, gasping complete with open mouthed jaw thrusting, using accessory muscles to breathe (his shoulders were really doing up and down) and lots of fidgeting. There is an NBC news video on KZread we watched to make the assessment.

    Even though he is our current President, and we are use to more transparency in the past from his predecessors, we also need to keep in mind HIPPA laws, which means, if the patient says "you can't tell them this or that", they are not going to tell us. We can't have double standards on this...where we want him to obey the law, but we want the Drs to break it...that is not fair to the Drs.

    They also can’t share information they haven’t been given specific permission to share. Notice they shared his oxygen levels in the 2nd briefing. They probably got his permission between briefings. It could be that they had not discussed it prior to the dr being asked. Patient privacy is huge. President or not the doctors open themselves up to a law suit if they share something without permission

    Can i ask if you contacted Covid early in the pandemic? Have noticed a pattern in people who have long Covid catching it around Jan, feb, march, April.
