Dr. Matthew Weiner is an author and bariatric surgeon who specializes in the treatment of obesity. His practice is located in Tucson Arizona.
Dr. Weiner graduated from the University of Michigan medical school in Ann Arbor, Michigan. He completed his general surgery residency at New York University Hospital in New York City. He practiced in suburban Detroit for 12 years before moving to Tucson in 2019 where he, together with Dr. Guillermo Higa, founded Tucson Weight Loss Surgery.
In addition to his surgical practice, Dr. Weiner is the Director of Bariatric Surgery and the Medical Director of Telemedicine at Tucson Medical Center. He is an active member of the American Society of Metabolic and Bariatric Surgeons (ASMBS) and is a past President of the Michigan State chapter a Bariatric Surgeons.
Dr. Weiner has performed more than 2000 bariatric surgeries since joining his first practice in 2006 and frequently does complicated revision surgery. He also performs general surgery with an interest in stomach and small intestine surgery along with complex hernia repair.
In his first book, A Pound of Cure, Dr. Weiner debunks the traditional understanding of calorie balance driving weight loss and offers a non-surgical solution to those suffering from obesity through nutritional stability.
His second book, How Weight Loss Surgery Really Works: And How to Make it Work for You, is now available on Amazon in paperback and kindle. In this book, Dr. Weiner describes how the hormonal changes that alter your brain, fat stores and hunger signals are the most important component of weight loss surgery, rather than the portion control effects that have been emphasized in the past.
A third publication called The Bariatric Diet Guide And Cookbook is a cookbook for bariatric patients that will offer recipes and guidance on the most effective post-surgical diet. It’s set to be published in May 2021 and is available for pre-order at most online bookstores.

    I use erythritol and Stevia! When I was diagnosed with diabetes my blood sugar was 375! Now at 7 pm at night my glucose is in the 80's! I eat low carb and my A1c is well under 5.0!! I went from being over 300 lbs to around 220lbs!

    One thing he forgot to mention is that eating fruit sugar is processed by a different pathway as other sugars and fructose (fruit sugar) is processed directly by the liver and more readily converted to fat. I have changed my diet to use monk fruit, stevia and erythritol and I have lost 40 pounds and fixed my type 2 diabetes. Studies linking weight gain to the use of these items don't have much credibility with me when compared with my experience.

    He didn't mention anything about insulin respones. They don't spike your insulin. Which is why these sweeteners are good for weight loss.

    They will most likely lower your blood sugar a bit. (if you have no carbs in your stomach) They fool your body into thinking sugar came in and your body will respond with a bit of insulin (not much) thus lowering BS. i have noticed this in myself and youtubers have as well. I am more interested in gut microbiome. sacharine and aspartame will kill good bacteria. i want to know about stevia.

    I've been on keto using only stevia for over a year now. I've lost over 50 lbs and dropped my A1C from 6.9 to 5.4. So I don't think that everyone gains weight from artificial sweeteners. It still depends on your diet and eating habits.

    WRONG! These don't count as ARTIFICIAL sweeteners... these are low-calorie NATURAL sweeteners extracted from natural source. But the question really should be... is monkfruit/erythritol/stevia better than regular sugar? EVERYBODY knows sugars don't have any NUTRITIONAL value (except maybe for monkfruit which has antioxidant properties from mogrosides).... so don't compare it to a real fruit. We are not gonna squeeze a fruit in our coffee.The point of the low-calorie sweeteners is as substitute to high calorie regular sugar. Focus on why it's a better option...if it is. If it's not, explain why. This video is really useless coming from a doctor.

    I only use monkfruit or stevia extract. I dont have sugar craving much anymore, and Ive been doing this for extactly 2 months and 1 week. Ive also let go of what i call poison (Carbohydrates, sugar, and starch). My result so far has been, I can go 16hrs a day with absolutely no hunger, i dont crave sugar anymore, Im a type 2 diabetic, and my glucose is now between 90 and 119 at all times. I feel energized and Ive lost 35lbs. I didnt do this for weight loss, I did it because to type 2, which my blood sugar use to be around 245. The weight loss was a added benefit.

    The doc missed the mark big time in this presentation. He isn’t alone most bariatric surgeons are still recommending fruit in the diet. So sad to see this bad science.

    I don`t have diabetes but using Stevia and Erythritol instead of normal sugar. After a few months, I started to do not crave sugar as much as before. I felt like my brain is starting to change the connection between sugar and high calories. Your body wants calories, if it does not get from a type of food it`s not going to crave for it maximum for another nutrition. This is my experience.

    The problem with these types of things is that artificial sweeteners probably aren't good for you...but they are much less bad for you than what you could be guzzling in high corn fructose syrup and empty calories, etc. If people give up high fructose added sugar goods by eating a few tablespoons of any artificial sweetener it is probably the right choice for them.

    Fasting can lead to blood sugar rebound. If you are doing keto your body is already mickiming if while you sleep. no need to put the additional stress on the body.

    funny how he never actually explains HOW your body is supposed to know it has just eaten something sweet-tasting. and the reason people who use artificial sweeteners are heavier because people who are already overweight are more likely to use them BECAUSE they are ALREADY overweight.

    Base line is that If you consume ANY type of sugar whether natural or artificial it will prove to be harmful, so just Have like Two teaspoons a day, and you’ll be fine.

    I think the take away from this video is that if someone truly wants to get healthy, they probably need to limit sweeteners, whether "artificial" or "natural". And most people don't like that idea. Because a lot of folks (like myself) got heavy over many years by eating sweet things. I would have liked at least a reference about the safety of artificial sweeteners. Because there is no EVIDENCE that they are not safe. And the weight gain, doesn't come from consuming them. Weight gain comes from potentially eating other foods that do have calories that you may crave after consuming the artificial sweeteners.

    I am a sugarholic. I only use monk fruit and stevia and a bit of erythritol. The problem is I start having more and more.... and my cravimgs for a sweet taste and carbs increases more and more. So what I have to do is go cold turkey on anything sweet at all for a few days and then start back very slowly. I have to do this every several months. So no, they are not the greatest thing but I think genetically it is extremely difficult for some of us to give up sweetness.

    I learned from another expert that Erythritol is actually neurotoxic. It affects your hormones and makes you crave more for sugar. If anything, get rid of the Erythritol as sugar sub.. and stick with stevia and monk fruit as they are actually the best sugar alternative cause they don’t affect the brain, they have lower GI, dont cause insulin resistance, etc. But ofcourse, care and moderation is key.

    I don't get your argument? Natural = good? That's a fallacious statement; even the fruits (apples, bananas, berries) are the products of thousands of years of artificial selection and propagation erring towards the side of sweeter products. The berries that the cavemen ate are for sure a hundred times less sweet than the supermarket berries we eat today.

    Last month I bought grapes. Nobody would eat them They were sooo sickenly sweet. Just terrible. I threw them out. I also bought Herring in a jar but it has tons of seetness. Sweet fish? Really.

    We have wild blueberries, raspberries, blackberries and strawberries around here. These are totally wild plants, not propagated, fertilized, watered or anything. They are sweet, especially the raspberries. Even if these plants came from seeds of domesticated plants, they would revert back to their "mother" plant's characteristics once left to grow wild.

    When I was a kid in Louisiana, we would grab sugar cane when we could find it and chew on the pith. If my mom caught us, she would make us immediately go brush our teeth.

    Does the fact that they’re heavily processed and derived make a difference though? This is what worries me, and of course not all natural things are good for us either... I’ve tried stevia and erithritol here and there but only in very limited quantities If you know anything please lmk though!

    The research simply says that if you eat alot of sugar substitutes then you will get used to sweets. Then, when presented with sweets you'll tend to overeat them. If, however, you plan your carb intake, then none of this will apply to you, and there's no downside to sugar substitutes.

    I do not have more than 4 tbsp. of sweetener a week, and I try to sneak in inulin or ground flax seed in the recipe. Hope that is ok. Type 2.

    No they are all bad, stevia, monk fruit, coconut sugar, white sugar, brown sugar, agave, dextrose etc all bad. Use organic unheated honey or organic syrup, best one is date.

    So what exact process is "triggered" in the body because the taste bugs can sense sweetness without calories? I was thinking perhaps these sweeteners would cause an insulin response thinking that you're eating sugar, but many tests have been done and they do not trigger an insulin response. So what is he saying the process is that eating these would cause weight gain? Incomplete

    Being a doctor doesn't mean anything. They do what everyone could do, read the data provided by the industry. They do no actual trial and mostly talk from a virtual point of view. It doesn't take a genius to understand that introducing foreign nonorganic chemicals to your organism isn't good. But since it is marketable who cares about common sense.

    You can’t gain weight from artificial sweeteners they do not have calories. You can’t defy the laws of thermodynamics. Maybe a side effect of consuming them is overconsumption of other foods but to say they cause weight gain is dishonest.

    @@Smil7001 artificial sweeteners like sucralose mimic sugars and are just slightly altered sugar molecules. The fact that it is so similar to sugar still triggers insulin production. Anything you eat that the body can convert to sugars will be turned into fat. The body can convert protein into sugar. The sad fact is that diet drinks make you fatter.

    The refining process introduces a lot of chemicals that nuke our gut microbiome. Aspartame in particular destroys your good gut bugs creating an imbalance. When there is dysbiosis that is what influences weight gain.

    Are you certain of this in regards to stevia or monk fruit? I am fasting regularly and reducing sugar during days I do eat. This is in part due to my emerging awareness of microbiome in the gut. I accidentally cleaned up my gut a decade ago by going whole foods no sugar diet and ADHD was relieved a year into this new way of eating. Sadly I let life run me over sorta of speak and went back to old eating habits ADHD back in full force and energy low. Any help would be appreciated.

    @@michaelsager5688 Great job on getting back on the wagon!! It is so great that you had cleaned up your eating and reversed your ADHD!!! Hold on to that proof that you did it once, because you can (AND WILL) do it again!! 💪🏼👏🏼 I am certain that it is dysbiosis that causes disease and weight gain. The effects of aspartame are already well known. I suspect we will see the same effects across any CHEMICALLY REFINED sweetener because of the chemicals that are added. They can disrupt the biome and COULD cause a dysbiosis. I was on Keto for 8 months and then started having insomnia. Going strict carnivore for a few months helped, and now I think I've isolated it to be the zero-carb sweeteners. I was having a lot of Keto treats at that time and was using Truvia and then Swerve (erythritol) . (I cannot speak directly to monk fruit.) This is not to say that Stevia in its raw leaf form would cause the same issues as a product like Truvia. (Of course unless it was not organic raw stevia, then it's still risking the same problem because it still has chemicals.) Again it's the refining process with chemicals that worries me. So if you're worried about leveraging zero-carb sweeteners to help as you clean up again this is what I would say: start with where you're at. Focus on coming off refined sugar with these products if you need an intermediate step. If you can keep using them without seeing adverse effects, great. But if you develop insomnia or other things like I did, don't assume these refined zero carb sweeteners are as completely innocent as we're led to believe. Their time to be swapped out may too come. Keep at it, you're gonna do great! This may be tough, but so are you! What else do you specifically feel you need help with?

    Maybe artificial sweeteners aren't causing weight gain. Maybe heavier people rely on them more, which accounts for the link between the two.

    I know of many people that add monk fruit and/or Stevia to protein drinks and smoothies to simply enhance the flavor of an already nutritional item. The argument about “confusing your body” based on the assumption that those sweeteners are only added to non-nutritional foods/drinks doesn’t seem to address that situation.

    This is bull shit, not the information. Stevia has 0 impact to blood sugar and 100% natural. Will not increase your weight and recommended by every keto nutritionist. I personally eat it every day and check my blood sugar - can confirm that.

    That's a great question! I have mixed connective tissue disease and fibromyalgia and really wonder! I read people had gastrointestinal and metabolism problems with these in the comments but it would be great if someone explained the inflammation!

    I use erythritol in my coffee, and I've never had any stomach upset. I know the effects of sugar alcohols, like Xylitol and Sorbitol, and they DO cause me stomach upset. But erythritol doesn't cause that. I only use a small amount though. The only time I had even mild problems with erythritol was when I baked cookies at Christmas with it and ate three of the cookies. There was a little bloating, but nothing near the magnitude of stomach upset from the other sugar alcohols. Another note, stevia in larger amounts can cause bloating. I discovered Zevia drinks cause me to feel bloated.

    Sorry doc but you are thinking of Maltitol found in candy. Erythritol doesn’t behave quite the same unless you eat cups of it.

    I'll disagree with you 100%! Both erythritol and maltodextrin, as well as any of the "olols" cause my GI tract so much distress, I've stopped using all sweeteners except pure maple syrup, and that only a tablespoon about once a month.

    Yes, maltitol is horrendous for gastric distress and it raises blood sugar & insulin. Maltodextrin I believe is not so harsh on your guts but has a GI of something like 110!

    Just based on the idea that your body revs up when it tastes sweetness, it seems to me that low calorie sweeteners would have the opposite effect that you're describing. If your metabolism revs up and there's no fat to store, it sounds like you lose weight that way.

    Hello Dr Weiner. Very detailed info on these sweeteners. I have one question, how about date sugar? My husband is a diabetic (but not overweight, never was...strange?!?). He has a strong sweet tooth, so I am always coming up with treats for him but always used either date sugar, but mostly just plain Medjol dates. Your thoughts? Thank you. I just discovered you in this media and really enjoying all the info you provide.

    Twice I bought a product that contained stevia plus erythritol. I had terrible gastrointestinal pain and diarrhoea. I googled it and found out the erythritol was to blame.Will never have it again.

    I commented on a previous video about this - but I am a super taster, I have the gene that means I taste bitterness much more strongly than other people. Water is bitter to me - until I began researching my own genes with the genographic study almost 20 years ago now. I can live without sugar, but if I don’t have some sweetness (and more than most people would need in my drinks) I will avoid water based beverages. We are all so different, so the recommendation to avoid artificial sweeteners as a general statement ignores that there are people like me where the taste issues are a very real obstacle. Note that I was quite heavy while avoiding artificial sweeteners (saccharin is extremely bitter for me) and was able to achieve a 100 lb weight loss using Splenda. I will need some middle ground and feel sure among the obese population the percentage of super tasters is greater than in other segments (would be a great study if not done)

    You are presenting this information as science and use terms like ´confuses our body?´ What do you mean by that? It is nothing but a metaphor. Please, I´m not in the market for storytelling here, I want to understand how this works. Please describe the mechanism. My body is not confused, never has been. But my mind sure is. The scientific evidence for this negative effect of zero-calorie sweeteners seems to me to ultimately take a leap of faith to reach their conclusions. They don´t really know how this stuff works. At least not yet.

    You say that artificial sweeteners predispose you to weight gain, but you don't reference any studies to support this claim. On the contrary, I see countless people on keto or low carb diets utilizing artificial sweeteners and not suffering from weight gain. Artificial sweeteners are a tool that can be used to battle unhealthy cravings, they are not gateway drug to more sugar. Please don't discourage people from using these useful tools without providing evidence that they are dangerous.

    Crockett83 he’s not says that artificial sweetener is a gateway to more sugar, he’s saying that when you consume artificial sweetener it turns on your sugar taste buds and that tells your body that it’s going to have sugar in your system so your body gets ready to absorb the sugar that it thinks you’re having, but there’s no calories or nutrients. Your body is like a machine it says OK this is the system we need to run and then it runs that system it won’t stop in the middle of the system to change the system so your taste buds are telling your brain run the system so your brain runs the system and encourages fat production because sugar makes you gain weight so what he’s saying is that you are gaining weight because of a system that your brain is running not because of what you’re consuming. You are tricking your system is what he saying. This is actually what I have been wondering about.

    That’s pretty awesome the bit about fruit and our metabolism. I gotta say I subscribe to this wonderful woman who makes easy to make sweet treats she uses Monk fruit in some things. So are we concluding that it’s still had even tho it has no calories just for the simple fact it doesn’t come a lot with any nutrition? What if it’s made into a protein cake or something?

    No disrespect, but at around 2:20 you said of the the erythritol "the alcohol prevents it from being absorbed" then (again) "the alcohol prevents it from being absorbed in our G.I. tract", THEN you said "it is actually about 90% absorbed", which is it?

    Stephanie Chavez Fresh stevia leaf doesn’t release the sweetness unless it’s ground up. The raw leaf is just as bitter as the powder.

    If u use stevia, find a pure product without fillers like maltodextrin! Pyure is a great brand, I find it at Walmart for a good price👍

    *Pls look into FRUCTOSE. It may not spike blood sugar levels like other types of sugar, but it does lead to diabetes, insulin resistance / pre-diabetes, NAFLD & metabolic disease.* *Every cell in the body can use glucose, but the liver has to convert most or all of the fructose you consume to glucose as the other cells can't do this - and this places a HUGE demand on the liver.* *Food that contain fructose includes: HFCS (high fructose corn syrup) Agave Nectar, table sugar, honey, fruit juice, fruit obviously & more.* *Pls note that Agave nectar is no longer recommended for diabetics or as healthy alternative to sugar.*

    Got off sugar 3 yrs ago. Stevia is my primary sweetener, erythritol & monk fruit are secondary. At 41 yrs old I’m leaner than I’ve ever been at 9-11% body fat. These sweeteners don’t confuse the body, our metabolism does get jacked up by eating excess sugar. And even healthy foods are turned bad in combination w/ sugary foods. Site your research doc. Please, enlighten us.

    Personally, I use ripe bananas to sweeten my morning smoothie. At my local supermarket they often sell off their aging, browning bananas really cheaply, so I buy a whole bunch and freeze them. I also often snack on grapes to help satisfy my sweet craving

    Erythritol makes me so sick!!! An Endocrinologyst told me that erythritol makes fluids gather and stay in the stomach for ages. It stopped my stomach emptying and caused me to have projectile vomiting. I’m fine with stevia and birch xylitol though. But omg not erythritol. 🤮 Plus I don’t like corn anyway.

    I guess you were being sarcastic but I plan to do exactly that: Crush the stevia leaves in coffee, and with the addition of your amazing idea, maybe one piece of sweet fruit, maybe dried, to go with it! Seasonal idea: Fig. I'm Turkish and we eat a small piece of Turkish delight with our coffee. Why not fruit instead?! Thanks!

    zay zay He briefly mentioned Every sweetener that is in the question as he pointed out how the sweetener is refined it’s witch is an answer to the people who claim that The sweeteners that he mentioned is natural and to make it clear to those people who believe in natural=healthy which is false. Secondly He points out the relation between the natural and the byproduct in which he made a good example that if chewed maybe let’s say three sugarcanes it is not equal to 3 teaspoon, three do you spoon so much more! therefore technically speaking if sugar canes are not healthy sugar is worse. Thirdly he Mention that in the long run people who consume artificial sweeteners tend to be heavier the reasoning Is there body make a connection between sweet and low nutritious food which is bad for the regulation of the body it sends confusing signals. Then finally he talks about fruit and how Sweet and healthy it is for the body it’s not confusing and does not make false connections between Low nutrition and sugar. It’s pretty clear.

    I drink one cup of coffee a day with 3 teaspoons of monkfruit. Eat and apple in the morning. Along with triphala, manjishta, and ashwagandha. 20 minutes later I take a fulvic supplement and workout. I'm 5'10 165lbs. What feeds yeast in the body more, monk fruit or sugar? What feeds cancers more? Which one is worse for you in small amounts? Which one is best for fasting? I ask about monk fruit because it seems to be the lesser of the evils...although I read that the flavinoids in monk fruit are whats extracted and contain the antioxidants when refined at low heat. I look at all sciences behind products before I consume them. And I have yet to see a over weight person consuming monkfruit.

    Pretty much the only point I got out of this is if it's "Processed" NO ...... Type 2 Diabetic been using a blend of Splenda & Stevia I like both but like the mix call me weird.... and my levels have been great and I have not gained weight nor lost I am using them for my numbers for my Diabetes

    Honestly, I just want to know what sweetener is not bad for me and doesnt cause inflammation! I generally use honey but I'm trying to learn about the other sweeteners. I dont care about the calorie count at all.

    I knew it. It seems at first I can fool my body but it adapts and eventually my blood sugar soars. Blast it!
