Discharge planning is a complicated business! Welcome to part TWO of my discharge planning series. In part one, I go over the long list of factors you need to consider to discharge a patient safely and efficiently. In THIS video, I give a few tips that will help you keep track of everything during the hospitalization so that discharge isn't delayed due to factors within your control.
