    Ver well done. used time effectively. excellent use of pauses. explained Down's succintly but appropriately. 0.12 - "found out I'm pregnant." - Dr's response Congratulations was instantaneous. What if this was an unwanted pregnancy? Rather " How do you feel about it ?" may have been appropriate.

    Quite a tricky consultation, I probably wouldn't know these numbers off the top of my head or explaining the condition in simple terms - well done. But the ending was a bit strange - so she is worried about Down's syndrome, but no actual risks identified, no tests or scans done yet. The conclusion is that she will discuss with husband and read about Down's syndrome before making a decision how to proceed. Just not sure what decision exactly, she is not expressing any doubts about pregnancy and procedures like amniocentesis are not on patient's request as far as I know? But yeah, difficult case for management plan/follow up as outside of actual GP scope of practice, this is all managed by midwifes nowadays and obs if needed.

    So the Doctor should not congratulate initially till he finds out what is the patient's main concern. It is a good point.

    What is the blood pressure and abdominal exam is doing in this scenario?? She just found out that she is pregnant. Talking about work was irrelevant.

    they offer amniocentesis when risk of Down syndrome is 1/250 or more

    Amnioscentesis miscarriage risk is 1 in 1000 not 1 in 100

    he said 1/1000 for risk of DS, 1/100 for risk of miscarriage from amniocentesis.
