Connect positive and negative terminals as it is and take another wire connect to the ground screw of DVR and to the negative of power supply .. that's fine it worked for me and cross check with tester to the metal body for the current pass if off now .

If u use multiple power supply connecting with camera and DVR. U facing ground loop problem. Video quality is so bad because noise level are increasing. So I highly recommend u all off the power supply ground(Negative) connect with each other.

Check harddrive as it get dead easily after few years and you see loose connection of hdd with DVR inside it
