Chronic Pelvic Pain Syndrome: My Story - Chronic Pain Podcast - Ep. 1

Today's video is specifically for the men out there suffering with CPPS otherwise known as Chronic Pelvic Pain Syndrome or Chronic Prostatitis.
There is still no cure for this condition and it plagues 1000s of men every year, if not more. I know there are a lot of questions that come up off the back of people hearing my story and instead of answering those questions individually I have decided to make a podcast exploring the topics of chronic pain.

    Made me cry man, I've been dealing with this for over a year but it all makes sense. I was working out regularly, eating like shit though, with multiple medical issues. But this gives me hope that I can continue doing the right things and over time and can improve my situation

    @@mahmoudnagy5861 No I’m afraid not. 2 years in. Certain aspects have got better, but the main stuff is still there. I’ve just had to accept that I’m going to probably have this for a long time, and I need to just live my life the best I can until this goes away

    Great feedback. Been struggling with this for over a year and no one has been able to help yet. I have recently ditched sugar and focusing on diet and activity. Will just keep moving forward and hopefully will get easier as time goes.

    Diet and movement is important. I would highly recommend looking into myofascial release of your quads, adductors and lower body. This helped me a lot at the start, on from there checking imbalances and positions of the hip is important 😎💪🏼

    I'm a female with daily CPP, 4 years now. My pain started after long distance running combined with very high level chronic stress. Just recently started working with a sports rehab PT doing dry needling with estem working on building core strength because I have none. I had a personal trainer tell me to stop running so much because I had core issues, but I was stubborn and didn't listen. I'm learning a very hard lesson now, this pain is debilitating. I started my journey flying to California to attend the Wise Anderson protocol in person, and while I grasped the concept, I struggled with accepting it because I still wasn't getting any better. Now 4 years later I realize that I do have biomechanical issues and core issues that need addressing. This was a great video, its good to hear from someone who made it to the other side! We need more videos like this

    You almost perfectly describe my experience. You put into words what I have been unable to put into words. I ran into your video, probably not coincidentally, as Google likely learned about me from my search history. It baffles me - the degree to which you precisely described my journey, the physical part, the psychological, what you were doing for work, and what was going on in your life when it started. I also had started a business, been through a career transition, and was under pressure to get married to my long-term girlfriend partner (now married), etc. Even the timing of when it started - at the early stages of the pandemic. I have been suffering from this ambiguous pelvic pain and going to doctors pleading for help for over 3 years now, and this information is all very new to me. After two and half years of this, I actually had a total hip replacement on my right hip, with an intent to do the left hip after that as well. The right hip surgery has not had the expected impact, and even the hip surgeon is advising to be more cautious. Before surgery, and now, I can hardly walk. Nobody can understand it. I have done a lot of physical therapy, but we don't seem to be focusing on the right things. The therapy either has no impact or makes it worse. Before that, I was athletic and doing monumental feats of strength in the garden. Lying in bed is actually more painful than sitting. At night, underwear can sometimes evoke an excruciatingly uncomfortable feeling. Walking, I'm exhausted and nearly disabled in less than a half mile. Trips to the grocery store are challenging. I have to come home and nap several hours or I'm wrecked. I'm not that old. It has been hard to communicate. I have tried. You get the shrug and "It stinks to get old." But I talk to old people, and they say, "You're not that old." So.... We have gone down almost every avenue EXCEPT the muscles in the pelvis. Up to and including sawing off a hip and replacing it. When that didn't work, I was left with, "What now?" "What am I not describing carefully enough to connect with these doctors correctly?" The experience is a nightmare. In some ways, I feel lucky. I can only imagine it would be worse if I was out of shape, overweight, and on a poor diet - as the doctors would likely blame that for my experience. However, that was not the case when this started. I have a lot of issues with my body mechanics and time at a desk - correct. But I'm simply lucky we couldn't just dismiss my conditions as, well, you need to diet, bro. Nothing against fixing up those things, but sometimes, when these go-to excuses are used, if that's not the real issue, people never get a solution.     Thank you for commenting and taking the time to share so much detail. It is a wild experience trying to explain what it feels like and even more overwhelming when there are no answers anywhere. To preface the Functional Patterns element, my posture and biomechanics was the last thing that I hadn’t really addressed in my life… It was the intention to improve my posture and explore repositioning and building muscles up that never worked… The process of doing that has been very powerful - hopefully our chat on our call can help 🙏🏻

    Been suffering this torture for six years. Just kinda figured this out a couple weeks ago, it started with me when i had a muscle pull in the groin left side which i now suspect psosas muscle which involved the cord and testicle. Ended up with groin mesh both sides after exploritory surgery and took five years to convince Dr's mesh is a problem and it was wrapped around the cord. Now i have major issue's still regarding functional ability and now looking for a therapist. I have had to be my own advocate and literally tell the Dr's what to do, they will treat a symptom not necessarily the problem. Glad to have found someone that has been through same and thanks for sharing.

    ​@@rawmotionfitness I've implemented some weight loss strategies, such as minimizing prolonged sitting, committing to a daily walk of at least 2.5 hours, and engaging my mind with lectures on my phone during walks. I prioritize early meals and, crucially, focus on staying hydrated by consuming plenty of water before 5 pm. Additionally, I recently sold my electric bike as I suspect and sitting for so long might be the contributing factor.

    @@christset I think that the bike was a big no no for me too. I bought a Peloton in lockdown with my wife and thought it would help me stay active as I wasn't able to run... For the 20 minutes that I was on it I was fine but it felt like my prostate took a massive hammering afterwards. Weight-loss will help a lot with the core and I would recommend improving the pressure system within your core and ribcage to help take some pressure off the pelvis. If you need to know what that means more, please let me know and I can give you some advice here.

    Thank you for this video. I believe you have pointed me in the right direction for resolving an issue related to my problem. After spending over a year dealing with prostate, constipation, urinary urgency with little output, and all the pains associated with each, I have gotten no help from doctors other than offering scary medications that don't help. In June 2022 I bought a fancier office chair (reclining) because I spend too many hours on the internet at a small foldable table. Being foldable means its legs don't allow the chair to center with the table and I very often recline with my legs offset to the left. I'm always leaning to the left even when not reclining. I have often wondered if this chair had "something" to do with my situation because my problems began 5 months later and have gotten worse with the passing time. It makes perfect sense that I have probably developed an imbalance in everything related to my hips & pelvic floor due to the long hours spent in this chair. Bredan, at the very least you have provided information that makes more sense than the useless doctor visits I experienced. 👍

    This is helpful. I will dig deeper into functional patterns. I feel like I've thrown almost everything at this with very little success. I had to deal with this in college, and at the start of the pandemic, many of the symptoms came back after overdoing it with my training. I'd be interested in connecting if you are open to it

    Hi there, I am sorry to hear that you have been struggling with this condition for so long. I hear what you are saying about the issues you are facing day to day. There are probably a number of reasons for this. I have recently recorded a podcast for the channel with a lady called Dr Potts who will was able to share some more medical background on this topic.     Ok, I understand. The Body is one of the Truth. My CPPS came after a really stressful Time in my Life...i had never so much emotional Stress in my Life before. I can say the nervous system is a big part of the Problem...i was to Long in the Fight or Flight Mode...the my Muscles in my Pelvis goes Spasm. Its a normal function of the Body. For me the root causes is Stress. I had never Problems with my mechanical Body...never. Good Luck to all...💪🏻

    Great video and advice. Did you have any split urination when you suffered from cpps? And if so now that you are free of it has it returned to normal? I read an inflammed prostate can squeeze on your urethra too..

    I haven't ever had that experience. I do believe that my prostate is enlarged but I have no way to prove this as I never had it checked when i was younger. The only issue I have had with urination is related to the feeling of increased urgency. Now that I have improved my posture and pelvic positions this has greatly changed.

    In UK i had 12 sessions of acoustic shock wave treatment to the perinnium. It mainly worked along side goving up spicy food, caffine abd alcohol along with pelvic floor stretches and internal pressure point work. Got to 95% ok.

    Network Spinal Analysis- chiropractor releases tension throughout your spine with gentle touches. Something additional to consider to what was spoken about here. This is all stress/tension related, for me atleast. peace. :)

    Mine is somewhat similar. During lockdown I was sitting on a hard plastic chair doing some computer work for a whole fortnight. I felt some discomfort but put it down to sitting for so long. Since that day it came on I have been in constant pain BUT only when I sit. To drive I have to wedge 2 blocks of cushioned wood underneath the top of my thighs to keep my perineal area from touching the seat. I have tried everything but the only thing that helps is a strong dose of Gabapentin and opioids. I can’t join the rest of the family in anything anymore and can only sit on the toilet to gain any relief. It’s like sitting on a golf ball! This is 4 years now, day in day out. I don’t think I will ever be pain free 😢

    Hey there, sorry to read about your pain and issues. I hear what you are saying... It will seem rather overwhelming to deal with this issue of sitting and participating in family events. I remember having to drive for an hour to visit family and in 'smart' clothes... It was horrendous! I would ache whilst driving and then I would have to go to a quiet room for about 10 minutes every hour to stretch my pelvic floor. I hated the feeling and nothing helped... Though I didn't try the pain killers that you have access to. Ultimately there are several things that have changed with my core pressure and how my body works that makes sitting really easy for me now.     I’m another of the unlucky guys that has this demon condition. I couldn’t walk more than three or four steps without viscous pain. I’ve been stretching like a madman but still can’t sit without experiencing severe burning pain. I hate that the people/companies who can help, charge an amount that is out of my reach.

    This was a consideration when I was initially seeking treatment but working on my lifestyle, biomechanics and managing stress has essentially fixed my issues. So a varicocele is unlikely to be the cause. Is it possible to have symptoms from a varicocele, I would imagine yes.

    ive been taking antibiotics for 6 months on and off. I find it really difficult to accept its muscular though i do work as a heavy duty mechanic lifting heavy objects. The antibiotics help everytime. Though no matter how long im on them the pain comes back each and everytime. The doctor can not find infection. So im thinking about taking your advice. But im so nervous taking this route because the antibiotics are the only thing that have kept the pain gone. My experience seems different from yours. But if you have any advice at all from someone with a similar experience to mine it would be so helpful.

    Hey, thanks for taking the time to comment and also to ask questions. It is important to note that without knowing your lifestyle and work life balance it is hard to advise specifically. The way that I looked at it was that *NOT ONE* doctor would even entertain how my training, posture and job had affected the situation. When I asked if heavy lifting could be a cause they said no. Yet, all the literature around CPPS that I have read points to myofascial issues in the pelvic region. _ (Also worth noting that your pelvic region is pretty much from your lower ribs down the the middle of your thighs, so if you are imbalanced in any of that region it could be a contributing factor. I have issues with my obliques and hip hikes.)_ Heavy lifting with incorrect pressure system in my midsection was the root cause. *HOWEVER* , contributing factors that compounded the pelvic pain were, stress, anxiety and anchoring into my pelvic floor when feeling anxious. In other words I sometimes unconsciously clench it and it takes a while to relax it. Antibiotics have been shown to help reduce inflammation, but mostly in cases where there is infection. That being said, if your pelvic floor is too tonic then the prostate can get 'grumpy'. Just remember, you can still have antibiotics at the ready if you choose to work on your mechanics... It's not a life sentence with pelvic pain but it certainly requires a lot of changes, being persistent and consistent with the process, which I think is why a lot of men struggle. If you are lifting heavy objects for work then I would suggest exploring how you are lifting and how your muscles are working together to lift things. I hope that helps, please come back if you have further questions.

    Possibly. I think it came up in many of the threads that I caught online… That being said, unless there is an infection, which is hard to definitively prove with any prostate infection, then it’s more likely an issue with the fascia and the muscles in that region of the deep front line. The only way to rule out infection is to run a long term course of antibiotics which most men I speak to have done with no pain relief.

    Is rectal pain also a part of this? Because I have been dealing with pelvic pain for years now and I have not been able to find not one solution and I do not want to depend on NSAIDs or any other medication, so I'm not sure what to do at this point. I feel so depressed and bummed out 😢 I think this is what I have cuz I cannot think of any other name for this pain that I experience every day of my life.

    Hiya, sorry to hear that. Yes, any pain below the kidneys and up to the mid thigh is technically pelvic pain. Medication made me depressed too. I felt horrendous and the effects were minimal. Maybe if I had given them more time it would have made the difference but I nearly lost my business because of the drugs. Rectal pain can also be down to other things... What has your Dr said?

    I had 2 per week but the frequency is less important than choosing a reputable practitioner and someone with experience with pelvic floor issues and Functional Patterns. :-) Also, rehab with any issue like this is often about taking things away from your routine that create stress rather than adding more on top...

    Brendan did you ever have a sensation of feeling the need to walk hunched over? a feeling of tugging or pulling on your testicle or pubic area? or the discomfort moving from side to side at all?

Back in2017 while weight lifting I felt a small flutter / pop down in my right pubic area over the course of a week or two I developed a constant dull right testicle pain, ache in the hip and lower back tension. Eventually testicle burning and felt like it was being pulled on or tugged on. Eventually between the testicle burning and the hip and low back ache and tension I could barely walk, eventually it then started to occur on my left side too but not to the same extent and oddly it would switch sides. After 14 imaging tests of my pelvis, lower back and hips they never found anything, same with seeing urologist. Eventually I was diagnosed with a sports hernia by a sports med doc via an ultrasound this was late 2018 towards the end of my pain, I saw a PT and finally got better. So I dealt with it for two years. Flash forward to September of last year, I decided I'd make a return to weight lifting but stop the heavy stuff etc, low and behold upond doing some adductor work I felt the same small twinge or pop right down next to my pubic symphysis and immediately developed all of the same symptoms, right testicle tugging and pulling and tugging and pulling in my pubic and right groin area, testicle burning, hip and lower back tension and ache, then it went to my left side after taking a funky step and the left is very similar minus the tugging and burning, and it goes from my right to left side. Right is worse when standing and walking left is worse when sitting and laying. I had another MRI and the ruled out the sports hernia but found a small right side inguinal hernia in the exact location of the sports hernia. Saw a hernia surgeon and he told me there's no way all of this is from a tiny hernia and wasn't sure if my scans showed a hernia or not and wouldn't know until her cut me open so I denied. I am now with a pelvic floor PT, whom thinks my issues or pelvic floor and girdle related, my left leg consistently it 2 inches longer than my right, and she Believe I lack lots of stability in my pelvis causing it to shift around etc. I've had two PTs tell me now they don't think the sports hernia was a proper diagnosis and maybe it was mistaken for the inguinal hernia or vice versa so mentally I'm trying to let that diagnosis go. I seem to be getting better slowly but every now and then over a stretch a bit and get and odd sensation or flutter right up my penis and immediately can feel things on my right side tighten up, like my inner groin lower abdominals and testicle begins to tugg and sensitive when sitting and walking. Thank you so much for your response it's comforting to hear someone acknowledging the testicle tugging feeling as every doctor and specialist I've spoke to looks at me like I am dumb, and they also don't get why I get this overstretched, scratch prickly feeling in my groin. And under my buttcheek but I'd assume it's nerve related. Anyway thanks for your confirmation and if any of this other stuff sounds familiar which to me about of it did after watching your video that's be great to know. And if anyone else feels similar to my symptoms above let me know, I feel lost in this just like you Brendan especially when doctors and specialist have no idea and when I just get told it's my back or an infection causing my symptoms, or the sports hernia diagnosis, I thought I had a final answer with that until my recent imaging said other wise and while two PTs totally disagreed with it.

    Thanks for your response Brendan! That's reassuring to hear. My initial run in with CPPS to my knowledge was back in 2017 all the way until nearly the end of 2018, I was weight lifting and felt a small pop or twinge in my right pubic area and within days developed burning in my right testicle, tugging and pulling feelings in right testicle and inner thigh abdominal tension and a limp to where I could put a whole lot of weight on my right side, eventually after about 8 months of this it moved to my left side and present with similar but slightly different symptoms. Over the first run in with this I had nearly 8 mris and 5 CTs scans, ruled out hip and labrum tears, hernia, back issues etc. nobody had a clue, 3 rounds of PT and eventually via an ultrasound I was told a sports hernia towards the end of full recovery. Flash forward to September of last year again I was lifting weights very lightly and felt the same little pop / pull feeling, I went right back into PT assuming it was the sports hernia only to be told confidently by my PT that he highly doubted it, identical symptoms only this time it took only about 1 months until I had the left side too and it goes back and forth. Had another MRI done and they said no sports hernia, but inguinal hernia, off to a surgeon I went, he looked at my imaging and said he doesn't think I even have a hernia or if I do it's tiny wouldn't cause this plethora of symptoms. I think got referred to a pelvic floor PT. Where I seem to be making progress. She believes I like didn't injure something in my lower pelvic area or low abdominals that led to my entire pelvic area becoming dysfunctional including my hips. She does a lot of hip gliding with me and realignment as it seems like my left leg is consistently 2-3 inches longer than my right. I'm hopeful at this rate that the two hernia diagnosis were just moot diagnosis and that it felt is my pelvic area as I want to about surgery at the end of the day and every doctor seems to think it's not a sports hernia or regular hernia. I've been through the ringer with this. Right side testicle burn and pull, inner right adductor tension, lower abdominal tension right side and small crackles in my pubic symphysis area. The pain sometimes literally just moves over to my left and presents with tons of hip and leg tension and lower back tension with pressure in my pubic area and left testicle sensitivity but not burn. It truly is kind boggling. Sorry for the massive write up, but hearing you acknowledge the tugging was great reassuring to me

    @@squidgaming5094 this is horrible to read and I am sorry that you have had to deal with that. There are a number of things to consider here though, from what you are saying and based on my experience with my clients and in my pelvic pain my pressure system was not working properly. I would often try something like a renegade row and have a click or a big pop in my left hip when doing it. My whole left leg and left side of my pelvis is hiked as well... When I talk about the pressure system you have to imagine a balloon filled with air and the top of the balloon (ribcage) is compressed and pushing pressure down the bottom of the balloon (pelvis). When I started to address that system with Functional Patterns it helped to alleviate my symptoms. What kind of postural assessments and corrections have the people you have seen done with you?

    Yes, I believe that if you look up the clinical area of pelvic pain it in a very broad area including the hips, abdomen and more than halfway down the femurs.

    I've had left testicle pain since 2008. Orchiectomy no help. Many other surprises. A pudendal nerve block is the only relief I've had. Looking into pudendal nerve decompression surgery

    I'm also having blood in my after, and pain after intercourse etc. Urologists over many years always throw antibiotics at me and nothing else, it's super frustrating. You had that issue and everything came back clean in terms of testing?

    I no longer have the blood or any issues after intercourse. I had multiple blood tests over a 12 weeks and a lot of antibiotics but nothing showed up and it felt like they were throwing spaghetti to the wall to see what would stick. It was concluded at that point that I had CPPS… As opposed to one of the other 3 forms of prostatitis… When I learned more about how to use my pressure system in my upper body it was more beneficial.     Yes, I gave up drinking when I lost weight 10 years ago… managed to give up caffeine last year. Caffeine was a hard one for me…

I sometimes few like my life dont make sense any more i have been having recurrent utis pain bellow the scrotum, painful evjaculation during and after, erectile disfunction..i have been on different medications, gone for dviu(dilation of the urethral structure) am currently on herbs medications.. this has affected my relationship with my partner i feel depressed worry all the time.. but i do exercise evryday ..i work normal good erection ofcourse with pain below the penis left side of the scrotum above.     No. My PSA readings were all good. The second test I had they were elevated because the nurse did my bloods after the doctor checked my prostate. That can skew the readings, if your doctor does a PSA blood work up after a prostate exam then your readings will be elevated...

    I’ve had “Levator ani spasm syndrome” or as the mayo clinic called it, “pelvic floor tension myalgia” for nearly 40 years. My pelvic floor went into chronic spasm after a prostate infection. The only thing that ever helped was knocking the spasm out using a TENs Unit across my pelvic floor.

    What symptoms are you having. Testicle pain can come from the pudendal nerve. Figuring out why the nerve won't stop firing has been challenge.

    I believe that, if someone has CPPS, and not the other 3 versions of Prostatitis, then they will likely battle with issues for the rest of their lives. It may lie dormant but when stress comes back there will be a flare up. Equally the pain can shift to areas in the pelvic and hips or even into the legs. In other words, the body will deal with pain again unless the person in question can learn and understand how to manage their life and their biomechanics so that they don’t end up back in the place where they were…

I went from pain and hardly able to walk to being pain free... I find it hard not to see that as a journey.

    Hi B, Thanks for watching the video and it’s a great question. I don’t think it helps but it certainly isn’t the main cause. I was fairly frequent before and up to the point of the pain kicking in and then I stopped for over a year. The only time I would do anything would be with my wife and even that caused me problems. It slowly improved and now there is no issue but I find that if I am more frequent with any form of ejaculation my pain can increase. The prostate is ultimately is sandwiched between the bladder and the pelvic floor. So sitting on soft seats and with a rounded back and a full bladder can either cause pain, or weirdly cause arousal as there is pressure on the gland itself. I am happy to make a video on the sex function aspect of CPPS but I’m not sure how KZread will respond to that in terms of their policies… 🙈 If you are on instagram then feel free to reach out, I can’t always reply straight away but can certainly help as much as possible…
