Chronic diarrhea & Malabsorption syndromes are very broad topics,explained clearly in this video. Learning objective of this video is to give you a step wise approach to chronic diarrhea. chronic diarrhea causes, symptoms and difference between secretory and osmotic diarrhea is explained in this video which is a frequently tested topic of USMLE STEP 1 & USMLE STEP 2 CK.Celiac disease,irritable bowel syndrome, bloody diarrhea is also explained. in my previous video i talked about acute diarrhea workup and acute diarrhea treatment and management if you havent watched that link is given below.
Chronic diarrhea, defined as a decrease in stool consistency for more than four weeks, is a common but challenging clinical scenario. It can be divided into three basic categories: watery, fatty (malabsorption), and inflammatory. Watery diarrhea may be subdivided into osmotic, secretory, and functional types. Watery diarrhea includes irritable bowel syndrome, which is the most common cause of functional diarrhea. Another example of watery diarrhea is microscopic colitis, which is a secretory diarrhea affecting older persons. Laxative-induced diarrhea is often osmotic. Malabsorptive diarrhea is characterized by excess gas, steatorrhea, or weight loss; giardiasis is a classic infectious example. Celiac disease (gluten-sensitive enteropathy) is also malabsorptive, and typically results in weight loss and iron deficiency anemia. Inflammatory diarrhea, such as ulcerative colitis or Crohn disease, is characterized by blood and pus in the stool and an elevated fecal calprotectin level. Invasive bacteria and parasites also produce inflammation. Infections caused by Clostridium difficile subsequent to antibiotic use have become increasingly common and virulent. Not all chronic diarrhea is strictly watery, malabsorptive, or inflammatory, because some categories overlap.

My diarrhea started about 3 month now. I have since changed my diet and lifestyle to see if there's improvement but it kept going on and worse then I went to the hospital and was diagnosed with typhoid which I treated for a total of 28 days now my typhoid is negative but diarrhea symptoms persist and I feel really tired because doctors here don't know More that testing for just typhoid and recommending diet change which I have done.. is there any advice you can give me. My stool volume is typically low and watery, no blood and no vomiting, my appetite is average and am basically just getting by everyday.. please advise me on what to do..

IBS can only cause weight loss if you have chronic diarrhea. If you have loose stools most of the days of a week then most likely your weight loss is due to IBS. Weightloss can also be due to insufficient dietary intake. Keep taking small healthy carbohydrate meals through out the day.

I have chronic diarrhea since 4 years, no doctor has been able to tell me the cause. The stools are like husk, food not getting digested, pain in the middle upper abdomen, weight loss, no blood in stools. Loperamide, rifaximin, econorm, lacto bacillius don't work at all. Colonscopy, endoscopy, CT scan done, no hard findings. Some say IBD, some say IBS. What to do !? I am totally broken while going through this.

We go to the toilet 15-20 times a day. We have pain and cramps. Any food is like poison. Find us solutions about it. What should patients who have had half a meter of intestine from 2 meters of intestines done by surgery? We would like information on this please. We can't sleep at night. I am so very sorry. Of course I believe in prayer I believe that God wants to heal us and that Jesus paid for that price. But I was wondering if hydrochloric acid which helps the food digest in the stomach and pancreatic enzymes which helps the food digest in the intestines would help your food digest better and help you get stronger and fight. Anyway God bless.
