Primary failure rate for 1680 mandibular buccal shelf miniscrews placed in movable mucosa or attached gingiva
Chris Chang; Sean S-Y Liu; W. Eugene Roberts
ABSTRACT
Objective: Compare the initial failure rate (≺ 4mo) for mandibular buccal shelf (MBS) miniscrews placed in movable mucosa (MM) or attached gingiva (AG).
Materials and Methods:
1680 consecutive stainless steel (SS) 2X12mm MBS miniscrews were placed in a total of 840 patients (405 males 435 females, mean age 16±5 yr). All screws were placed buccal to the lower 1st and 2nd molars roots and the screw heads were at least 5 mm superior the soft tissue. Loads from 8-14 oz (227-397g, 231-405cN) were used to retract mandibular buccal segments for at least 4 months.
Result
Overall, 121 miniscrews out of 1680 (7.2%) failed: 7.31% were in MM and 6.85% were in AG, but that difference was statistically insignificant. Failures were divided into 89 unilateral and 16 that were bilateral (32 screws). Failures on the left side (9.29%) was significantly greater (P≺.001) compared to those on the right (5.12%). Average age for failure patients was 14+3yr.
Conclusion
MBS miniscrews were highly successful (~93%), and there was no significant difference between MM or AG. The right-handed clinician experienced more failures on the left side and in younger patients (~14yr). About 13% of failures were bilateral suggesting an inherent susceptibility in some patients.

Mariano Follana Thank you! My friend!
Amazing knowledge from Dr. Chang, professor how or what can we do to reduce failure on left side of the patience? only position of the body and arm from 12 to 1? greetings from a Peruvian studying in Brazil.
how did you create the morphing movie of the pano showing the impacted bicuspid at about 2:44 minutes into this presentation. i love seeing your work and have 2 cases treatment planned. are you available via email to review my cases? many thanks for all of these videos.. i wish instead of music you could do a audio at some points to give any tips, etc.

Hi Chris, what is the local anaesthetic u provide when doing the Class 3 whole lower arch distalization case, and for the upper arch distalization with infrazygomatic screws. is it just an local infiltration anaesthetic?

+andrew chang Yes, it is. Keep it simple. Thank you, my friend.
Sir, hello , kindly tell that are we to use only local infiltration anesthesia and not nerve block?
Thank you very much for perfect presentation. Can you present some manufacturers of stainless steel BS screws? Thank you very much.

Ok we all know that TADs fail. But I'm confused as to why the failure difference from left to right. Is this due to more people being right handed? I have 10 screws currently placed and 4 are not currently in use (8 buccal 2 palatal). I have an anterior open bite (completed myofunctional therapy for tongue thrust). I just wish my ortho had the screws like this with the big head...hey keep using elastic thread instead of power chain like everyone else does. Also the screws used in this video seemed super long and weren't inserted all the way?
To Dr. John Friend, 1. Please study the article "The primary failure rate for Buccal Shelf Screws" in Angle Orthodontist next month. www.angle.org/doi/abs/10.2319/092714.695.1 2. I like to leave 1/3 of screws outside the soft tissue which is easy for the design of force system.
how can I buy the bone screws? am from Palestine-middle east? Thanks
