As a Cat diesel fitter i used to get called in to a mine to do a top tune on any 793 that had a overspeed event. The contractor had installed a indicator liight that showed a overspeed and it was taken out of operations until it was inspected and then the supervisor had the key to reset it, the operator got a boot up the arse but it was all about minimizing failures.3500 eui engines had a run also of cam failures back in the early 2000s and i changed out hundreds of cams but 3400 engines were not bad apart from throwing legs out of bed when they felt like it ! LOL

I have seen in the comments that it was a "Overspeed" that caused the failure. Isn't there any Rev limiter or governor on these engines? Or is it like when you miss a gear shifting and it revs? As you can tell I'm not a Diesel mechanic.

If you’re going down a decline in too high a gear you will get a overspeed, also not getting early on the retarder will also not help slow down before ground speed gets too high. Most overspeeds are down to operator error.
