It is heard during Atrial Contraction which occurs during the Last Rapid Ventricular Filling phase when the atria has to do extra work incase of Restrictive Cardial Myopathy. S4 is the sound heard when atria is contracting so hard(for that left 20%), that a sound is generated. It usually happens in the case when the contraction of atria is less functional and some exercise has been done by the same person.

In gyton there ia a statement at 3times the normal heart rate systole is about 0.65 of the entire cardiac cycle. I don't understand this one ...if the heart rate is increasing why is the systole duration also increasing
