This is absolutely a part of the twist. What's brilliant is that the situation is conceptually the same with the tropes inverted. I'm sure they started with the idea of the cliche then took the time to work something new out from there.

I thought it was gonna be the different parts of Steve rogers just all split up in his mind With skinny Steve being his intelligence, big Steve being his caution, dog Steve being his loyalty, freedom Steve his determination for freedom and peace, and buff Steve being in a way the super-serum manifested as a part of his personality, the pure strength and rage that’s needed in battles

Medieval Steve at the end is Yeoman America from Mark Waid and George Perez's awesome run on Avengers in the late 90s. That story had medieval versions of a bunch of Avengers.
