Mere tooth 6 me Hai file 3 saal se bhe Jyada ho gya h but no problem Bas fill karwana padta h after every 6 month

My file has broken in apical 1/3rd of mb canal of lower molar 2 weeks ago after completing bmp and patient has no symptoms till now and I will complete the obturation tomorrow.

Your videos are amazing 👏 😍 did a perforation (in max 2nd molar in floor)few days back but there were no signs of bleeding or pain during instrumentation after radiograph I got to know about the perforation but the patient was pain free how???the tooth was grossly carious as well with mini structure and blocked canal used rotary for prep what will be the prognosis after obturation? Secondly if MTA isn't available can we use GIC to close the perforation? Plz do rply

If it's orifice level perforation then irrigation well and seal the perforation using composite or GIC, if it's root level perforation mta is best option otherwise do proper BMP and irrigation with activation and obturate using bioceramic sealer

Sir upper 6 me beyond palatal canal me break h but i don't know how bu t now pt have pain and palataly sawalling can i go for extraction or other way im very dispressed if i do extraction than how will I remove that instrument

Agar file tooth ka beyond chala gya h but half part root mein ho ga tho.,we can try for its retrieval...(still its complicated in curved canals)... If there is no broken file portion in root and all the broken file part is beyond the apex,then EXTRACTION will be the only option we left with.

I just fill hypo in the chamber and heat it with gp pen for 10sec, it works well rather than heating outside.

Always be open with your patient to gain there trust, even if you've done any mistake he'll forgive you and trust you more rather than hearing from other dentist about your mistake

So, doesn't it dangerous for a patient's overall well-being to have a broken file in the root canal?

If it is inside tooth canal, issue can be resolved either with bypass or extraction. Yes if it extrudes Apex then you have to surgically remove it.

​@@jungcookie8241I don't think removal of the tooth is a wise first decision, you can first try to remove the tooth by surgical method, elevate the flap and remove the bone where the file is located and close it, coz natural tooth is the best and not even implant can replace it, but if you can't surgically remove the file, then it depends on your expertise how well you can communicate with the patient and remove the tooth without losing the patients faith in you

If this happens again doc then try to bypass the broken file using 6 or 8no file, and do BMP with thorough irrigation, if possible you can use ultrasonic endo tip and use loupes to locate the broken file and remove it, but if it's in apical third then try to remove if you can otherwise just do proper irrigation with activation and obturate with ceraseal and inform the patient about this, it's better to listen from you then from some other doctor that you're previous dentist have broken a file inside the canal, hope this helps

Edta should be used before obturation to dissolve smear layer and during orifice locating(in case of any calcification) only, hypo is your main irrigation agent

You can put hot water in a paper cup and place the syringe with hypo in that cup, it will warm up the hypo
