The safety tape is supposed to be played on a cassette player which I think is supposed to be found in a cultists house.
13:04 That bone piece at the top of the tower there is part of something you can assemble later with the other parts that go with it. You left without even acknowledging it. 14:47 You put your sword in the furnace and it got smelted.

Altair is most probably from newer version of man from the fog. You have to somehow activate it for him to spawn, and there comes after a sanity, his realm etc.. So i recomend to you version 1.1 of mod if im not mistaken, it has similar mechanics to Calvin's one without unnessesary addons..😉
