Board Meeting: Fire Aboard Small Passenger Vessel Conception

​The National Transportation Safety Board met on Oct. 20, 2020, at 1:00 p.m., to consider the agency’s investigation of the fatal, Sept. 2, 2019, fire aboard the dive boat Conception off Santa Cruz Island, California.
The 75-foot commercial diving vessel, with 33 passengers and six crew aboard, was anchored in Platts Harbor off Santa Cruz Island when it caught fire. One crewmember and 33 passengers died in the accident.
The NTSB’s five-member board voted on the findings, probable cause and recommendations as well as any changes to the draft final report.
In keeping with established Federal and local social distancing guidelines to prevent the spread of the coronavirus, while also ensuring the NTSB’s compliance with the Government in the Sunshine Act, the board meeting for this investigation will be webcast to the public, with the board members and investigative staff meeting virtually. There will be no physical gathering to facilitate the board meeting.

WHEN: Tuesday, Oct. 20, 2020, 1:00 p.m. (EDT).
HOW: The board meeting will be webcast only, there will not be a public gathering of NTSB investigative staff or board members. A link to the webcast will be available shortly before the start of the meeting at ntsb.windrosemedia.com/.
MEDIA AVAILABILITY: NTSB Chairman Robert Sumwalt is scheduled to hold a virtual media briefing at 5 p.m. (EDT) to answer questions about the earlier board meeting and the NTSB’s investigation of the accident.
The virtual media briefing with Chairman Sumwalt will be conducted using Microsoft Teams Live Event. Journalists who RSVP to ntsbmediarelations@ntsb.gov will receive an email with the attendee link and information about how the briefing will be conducted. A recording of the briefing will be made available on the NTSB’s KZread channel as soon as practicable.

    At 2:39:40 Jennifer Homendy puts her finger on the crux. "During all those inspections, could the CG not note that the emergency exits both access the same area?" Absolutely fundamental design flaw which was ignored by owners and CG for decades.

    Anybody who is or does now work on a fishing vessel needs to watch all 3 hours and 53 minutes of this video. It should scare the crap out of them and make them realize what can happen if the rules are not followed, even if those rules are inadequate. The Coast Guard is worse than the FAA!

    Do we know or has it ever been mentioned who the boat manufacture was that built the Conception Boat. Are they the same company that built the Vision? I think it was. What I do know is it was Built by a company in Long Beach. I have done some research and these two boats are not the only two built from this Company. There are some Fishing Charter boats that are very close Sister ships to the Conception and Vision used out of San Diego as well as other ports in California. They have the same Hatches and stairwells as well as the same deck lay outs as conception and vision but set up for Fishing Boats. I am not sure if this boat manufacture is still operating or is a now defunct company? Also is there any word on when the Final Coast Guard report will be released?

    The lack of a roving watch was the single biggest factor, here. There should be criminal penalties for a captain’s failure to make sure that a proper watch rotation is established. The Coast Guard is overwhelmed by having multiple mandates. There’s no way they can assure that these standards are adhered to.

    Was in the Coast Guard for 8 years and we had an Oiler of the watch and a Boatswain's mate of the watch. I was in Operations/Comms but they are the single most important people on the ship when you are sleeping.

    Insurance company’s Loss Control Program doesn’t have an audit?! Sigh. If they asked questions they might have to provide some structure or insight and that could potentially make them liable to the process. Rule number one is defer all express responsibilities. Rule number two is maintain all implied responsibilities. It is remarkable how thoroughly a group of people can protect themselves while taking rigorous advantage of everyone around them.

    Might have been a diver from a previous trip. People take pictures of everything now. Or possibly from the sister vessel, the NTSB was all over that one post sinking. That's where they were checking out the hatches and stuff from the b-roll on KZread

    At 22:23 - they say pre-accident photos of Conception. Looking at the accident docket and looking at the "5 - Fire and Explosions - Fire and Explosions Factual Report" PDF (dms.ntsb.gov/public/64000-64499/64213/638842.pdf ). On page 36 it says "Figure 35. Left: August 31, 2019 photo of devices plugged in at the port side aft corner of the salon on the Conception (source: FBI)". To be clear I also think its a diver from a previous trip. The diver could have gave the photo to the FBI who then passed it on the the NTSB.

    Important to note that deviance is a dynamic and not a state, so this process can continue indefinitely. We have come to think of ourselves as the sum of our differences.
