Lesson 3 - Preventing Root Canals
WHAT IS A ROOT CANAL?
A root canal means removing the nerve and pulp from inside the tooth, disinfecting the roots, and placing a filling material into the root "canals". This process is avoidable, but when the pulp becomes badly inflamed, it will lead to pressure and pain and ultimately need a root canal or alternatively an extraction. The important point is that most, but not all, root canals are preventable if the teeth are maintained and problems are addressed before it is too late.
BIOMIMETIC DENTISTRY CAN AVOID OVER 90% of ROOT CANALS by saving more tooth structure, preserving more of the tooth, and using protocols to treat cavities.

    Thanks sooo much for the information. I’m watching this video with A LOT OF PAIN during the COVID-19 pandemic 😩😭 I was told by my dentist that I need a root canal but I refuse to believe that’s the only option I have. I need something done ASAP 😓

    You are so welcome! I saw your other comment and I hope there has been some relief. Pain doesn't always mean root canal and some people jump to that conclusion without proper testing and diagnosis but with high levels of spontaneous pain it is certainly not a good sign.     Sir, Great explanation. Sir, I have a doubt, exposed root nerve is highly sensitive to any foreign material, what if platelet rich fibrin from the same patient is utilised to cover the exposed nerve? Is there any research done in this direction? If prf along with bone graft material(preferably from the same patient bone if available) is used to seal exposed nerve and closed with caps or by some other means(like composite fillings) natural tooth could be saved. I strongly feel it Does work. Have u done some research in this direction? Please reply me.

    Is there a video where you explain the most difficult decision of keeping the tooth doing a root canal or do an extraction as you mentioned in min. 6:21? I’m right at that point of having to decide what to do with my molar #31 😩

    Great question. A majority of root canals worldwide are done by general dentists but the quality is not universally adequate. A well done root canal does work well. As you know, the goal is to avoid root canals but when you need one. Its still the most natural possibility. Extractions, implants, removable partials are all less natural. With that said, i have a lot of patients who prefer to remove it. I personally would keep. Nearly every well done root canal responds favorably and shows bone healing and resolutions of signs and symptoms which is excellent proof of how well tolerated root canals CAN be but this is no guarantee because of the quality disparity.     I have deep decay on #4 & #29. Though the decay is near the pulp, the pulp and roots are not infected. My options are a root canal or extraction. What're my options with biomimetic dentistry?

    If the pulp is vital and it is not infected, then a root canal should be avoidable. The options are filling, inlay, onlay, or crown depending on the severity of the damage and integrity of remaining tooth structure. There is no reason for a root canal if wha you are saying is correct, though a deep cavity may need root canal treatment if symptoms develop or the pulp degenerates. Best- MN

    So if I was told I need more root canals, can a biomedic dentist possibly save the tooth from needing that?!

    Possibly. Sometimes patients are told they need root canals simply because the cavity is close to the pulp, so they are told a root canal will be necessary even though there is a good chance it can be kept vital with a biomimetic approach. However, that does not mean every root canal can be preserved and there is a clear distinction. Often times, I am able to prevent root canals even when a patient was told it is necessary but it is not 100% and it is not up to me completely, because the condition and the diagnosis are important.     ....If teeth are porse, shouldn't the sealer be porse?....I don't know if I could afford other oppions about my teeth...When one is struggling financially, things are difficult....I probably will just opt to get my teeth pulled tomorrow....Wish I had money and time to investigate this issue more, but the winter is coming and I need to prepare...

    Thoughts on using ozone therapy in conjuction to this procedure?

    In general, I am not in favor of ozone because there are numerous studies that show it can lower the bond strength to resins (composites and adhesives). This is not the typical filling method inside the root, so I am ok with using Ozone in conjunction but I don not believe it can replace other irrigants used during the procedures and there is no indication that it improves the success rate. You would have to use Ozone water to reach into the canal spaces to disinfect. Best- MN

    Nowaday dentists want to do root canal for every little cavity they see BUT the problem is RC is not a definitive solution because it can fail.

    I was a little bit shocked and surprised when you at 07:13 say that the traditional way is to take away that amount of tooth structure (on the right picture).... in which country is that traditional??? Please inform your patients which country to avoid! This sounds insane. I dont perform biomimetic yet, but I will soon, but at no time in my career have I ever, and I cant see that other normal dentists would either, remove all that tooth structure just to put that "small" composite filling. I am sorry, this just sounds insane! I can understand that in any profession there are "cowboys" and unethical people thinking only about how much money they can make, but that does not make it a standard, but then again... I have only worked in one country, and here, and in the countries around me, this is NOT the standard. Unfortunately neither is biomimetic, but hopefully one day it will.

    Are you saying you are a dentist? What country are you in? Im not sure I understand what you mean but it is very standard for something like that to be turned into a crown in traditional approaches. Its a large and deep cavity. It covers around 50% of the occlusal surface. I agree its completely uncalled for but it is 100% common practice unfortunately.
