ERCP का पूरा नाम Endoscopic Retrograde Cholangiopancreatography है
यह एक Endoscopic Procedure है जो Obstructive Jaundice का इलाज करने में मदद करता है
0:35 Bile Duct Cancer में ERCP कैसे मददगार हो सकता है?
Jaundice Bile Duct Cancer में एक सामान्य लक्षण है।
इसलिए Cancer के इलाज की शुरुआत से पहले, पहले Jaundice का इलाज करना होता है।
अगर Bile Duct Cancer Liver के बाहर होता है, तो ERCP Jaundice का इलाज करने में काफी मददगार हो सकता है।
Doctors ERCP की सिफारिश करते हैं अगर Patient में Jaundice के कारण High Nutrition Deficiency हो, अगर Jaundice Long-Standing हो, अगर Definitive Treatment में देरी हो सकती है और अगर Blocked System Infected हो।
2:55 ERCP की सिफारिश किन अन्य मामलों में की जाती है?
अगर Patient को Distal Cholangiocarcinoma के लिए Surgery की आवश्यकता है और Bilirubin Level 10 से 15 mg/dl के बीच है, तो ERCP की आवश्यकता नहीं होती।
लेकिन अगर Patient को Hilar Cholangiocarcinoma है, तो Successful Treatment के लिए Liver को हटाने की आवश्यकता हो सकती है।
ऐसे मामलों में, Bilirubin Level को कम रखना होता है। इन मामलों में, अगर Bilirubin Levels 3 से ऊपर हैं, तो ERCP की सिफारिश की जाती है।
Type 1 या Type 2 Hilar Cholangiocarcinoma वाले Patient में, ERCP संभव है।
Type 3 और Type 4 के लिए, Doctors CT Scan और MRCP के बाद, अगर संभव हो, तो ERCP की सिफारिश करते हैं।
4:35 ERCP कैसे काम करता है?
यह एक Endoscopic Procedure है जिसमें एक बड़ा Scope Duodenum तक Inserted किया जाता है।
फिर एक Guide Wire को Blockage के पार Inserted किया जाता है, और Bile Duct तक पहुँचाया जाता है
एक Stent Inserted किया जाता है जो Blockage के लिए एक Bypass के रूप में काम करता है।
5:35 Conclusion
कुछ Bile Duct Cancer के मामलों में ERCP मददगार हो सकता है।
लेकिन Doctors केवल एक व्यापक परीक्षण के बाद ही इसकी सिफारिश करते हैं।

About Dr. Praveen Kammar
Dr Praveen Kammar is a surgical oncologist practicing in the city of Mumbai.
He completed his MCh course in surgical oncology from the reputed Tata Memorial Centre, Mumbai, one of the premier cancer care centers in the country. He also holds a fellowship in Colorectal surgery from Yopnsei Cancer Centre, Seoul, South Korea. Dr Praveen’s main area of focus is colorectal and GI oncology, Gynecological cancers, minimal access surgeries for abdominal and thoracic malignancies, transanal surgeries, and sphincter-saving surgeries for rectal cancer, cytoreductive surgeries, and HIPEC for advanced abdominal cancers.
