In my opinion, Q is not obligated to pick Liz. Yes she can’t eat and it’s a major thing for her, but she‘s done nothing for Q, nothing. So why should he pick her in a game setting.

He didn't need to but I think he probably should've - not taking her (and according to Tiff, denying Kenzie and Maria's requests to give up their spots) just makes him look like a dick, and the whole plan to reuinite the Yanu 3 didn't go anywhere.
