One of my illithid's eye is horrible. That's because I got it removed from Volo and replaced with the magical eye when I was human yet. This is the level of detail and choice of this game.

Withers said that mindflayers don’t have apostolic souls. They don’t have souls like the others do. Their souls are as alien as they are. So all ilithids do have souls, but they work differently.

The primordial god of death himself Withers tells you mind flayers do not have souls. When you turn into a mind flayer your soul goes to the nether world and what is left of you in the mind flayer are some memories and characteristics.. sometimes.

You don't really get a choice to, SOMEONE has to be a mind flayer in order to continue. Whether that's the Emperor (If you want the bad ending I guess?), Orpheus (If you want another bad ending.) yourself, or Karlach as she'll offer to do it too.

Both. It's in the endgame, after getting all the stones and it affects the Orpheus quest as well.
