the Problem with Wenckebach is that in my standard example in the book the Qrs doesn't nesserilly drop... maybe 6 second strip is not long enough. However it is better identified with irregular rhythm R-R intervals differ in this block. R-R intervals are same in all other blocks.

Thank you for the comment. However your statement and definition are inaccurate, so hopefully I can help clarify. 😊 A Wenckebach is a type of 2nd degree heart block. A 2nd degree heart block occurs when some of the atrial impulses are intermittently blocked from going to the ventricles. Therefore, some of the P waves will not be followed by a QRS (a dropped QRS). In the case of a Wenckebach, the conduction is progressively slowed from the atria to the ventricles until the conduction is blocked and there is a dropped QRS. If all of the impulses are conducted from the atria to the ventricles as you described in the example, then it is not a 2nd degree heart block. Instead, it could be a 1st degree heart block if the conduction is delayed (prolonged PR interval). I am not sure the example you are looking at but maybe the strip is not long enough? Let me know if you have follow up questions and hope that helps! 🙏🏻

How do you know from the ECG that impulses are beating independently? You still see a P wave and a QRS wave.

Great question! I assume you're referring to a third-degree heart block? You are correct that you can still see P waves and QRS complexes in a third-degree heart block. The difference is there is no relationship between these P waves and QRS complexes. In a normal functioning heart, every P wave has a QRS complex shortly after it. In a third-degree heart block, the electrical communication between the atria and ventricles is blocked. So not every P wave will have a QRS complex following it. You may have several P waves between QRS complexes. The P waves are independently spaced out in their own rhythm, and the QRS complexes are independently spaced out in their own rhythm, and there is no relationship between the two.
