Ask the Expert: Dr. Marcus Stoodley - What is the surgeon trying to achieve?

Dr. Stoodley shares the thought processes and factors behind the neurosurgeon's decision when treating Chiari and syringomyelia. Stoodley goes through some of the symptoms or problems they are hoping to alleviate with surgery, regular surgical risks they are trying to avoid, and a few of the different techniques surgeon’s use. Interestingly, Dr. Stoodley also goes through what is known and what is not known about CM/SM and their treatments.
Some questions addressed were:
What determines whether a surgeon performs a posterior fossa decompression with or without duraplasty?
How does a syrinx form in conjunction with Chiari?
Does syrinx size determine severity of symptoms and surgical intervention?
What is a good technical result?
Why might a syrinx re-occur after surgery?
& more!
Because it is still unclear why Chiari and syrinxes occur, it is difficult for a surgeon to know absolutely when surgical intervention will successfully alleviate the symptoms and pain associated with these disorders. “Successful” treatment of syringomyelia, the collapse of the syrinx [from surgical intervention], does not necessarily translate to symptomatic improvement and may result in the patient having worse pain while the spinal cord heals.

That was super interesting! I particularly appreciated the comments on movement/stretching? of the cerebellum being a potential cause of brain fog! And I actually had no idea that why decompression's help a lot of the time was not actually known for sure. That's a bit mind boggling really! Thanks!
