    1. I changed setting to continuous from adjusting and had lots less trouble falling asleep. Also fewer wake ups. 2. I recently went on Medicare and my 8 yr old ResMed is starting to get cranky so I asked my supplier for a replacement machine. Seems Medicare now requires a new sleep study IN-LAB (no home study) so now I’m going to have to wait about 2 months to get an in lab study. [Fortunately I have an original Transcend travel machine as a replacement if it dies.] But, hilariously, when I called to schedule they asked “who has been doing your cpap followup?” Say what now? Other than working with my supplies provider to get a good fitting mask and my regular doc occasionally asking how I feel I’m doing on cpap NO ONE has ever followed up with me. Lefty Lanky’s videos have been more educational and helpful than 8 yrs of medical inattention. So BIG THANKS BUDDDAY to Jahsohn for his work here. I can honestly say I sleep better because of Lefty.     Thanks Jason for the consultations and review of my data on Oscar. As we talked about, I just started on CPAP and was not getting the quality of life benefits after being set on APAP 4-20 pressure with an Airsense 11. The consultation and your channel are really making a big difference. The advice I got from my doctors at Kaiser Permanente was so high-level and inadequate...they didn't really look at anything other than the Resmed myAir score and the summary details. Neither of those were giving a realistic picture of how I was actually doing on therapy (not great, before your help). Now after your review and following your suggestions I've had 4 nights in a row with a true AHI averaging less than 1, and just as importantly I'm feeling the benefits of *way* better and more even/uninterrupted sleep. So thanks again. Given the consequences of untreated or ineffectively treated sleep apnea, you are literally a life saver! P.S. Maybe someday I'll thank you in person if I run into you at the premium outlets, but for now this Super Thanks will have to do. :) Cheers.

    I bought a used Airsense 10 off of Craigslist, and it was set for APP from the last person. I had 7 or 8 events per night. After watching your videos I changed it to CPAP with a constant pressure that my at-home sleep study said to use, and I'm having almost 30 events a night. I haven't used Oscar yet, just trying different settings and will check it all out soon. It's a long wait to go meet with the DME, so I'm just experimenting in the meantime.

    Many thanks for this, now I know better what to look for in Oscar (i.e. nicely rounded flow rate peaks), and I guess I'll have to try CPAP. I've been on APAP since forever, haven't tried CPAP even once. Not sure what pressure level to set, but I'll go with your +1 or +2 advice. Practice makes perfect. Or pufferfish. Sooner or later I'll probably get tired of getting it wrong, and pay you for an analysis. 😉

    This video is exactly what i needed. Ive been wondering what a good peak for a breath looks like. Mine have been consistently flat this week on Oscar. And i feel it. Im going to try the cpap setting on my number 13.

    Appreciate this video. I've always thought i had to use apap because it was set up that way. Your videos help me realize i can experiment to find my optimal operating range because i dont think 10 is working for me anymore.

    If you are going to use Auto make sure you have a very small range! I was on auto for months with a small range of 10 to 12. This was really good. Because of Jason I recently switched to a set pressure of 12 and my sleep has been amazing! For me one set pressure has been best! I like EPR set to 2 and EPR off feels weird.

    Thanks for validating my understanding of APAP pitfalls. I think it's useful when starting a new device to zero in on your optimal pressure but then you'll quickly want to switch to CPAP and micro adjust from there. I'm currently down to an AHI of just 0.9 doing it this way (untreated I'm at 58).

    I switched to CPAP MODE after using APAP for quite awhile after you and another person suggested it . I breathe well with it and it has really reduced the AHI.

    You had almost zero obstructive apneas. I have severe apnea without a cpap. When I use the cpap mode on the AirSense 10 AutoSet, my AHI is usually around 10. When I use the apap mode with min pressure 12 and max pressure 20, AHI is usually around 5 or a bit less, and I feel much better in the morning. I think a lot depends on the severity of your sleep apnea. Yours appears to be mild.

    My sweet pressure is 12. I decided to do APAP from 11 to 15. My average for the night was 12 to 13 and my events (which I don’t pay attention to) were reasonable but I started waking up not completely rested. The problem I noticed when Resmed interprets an event to be a clear airway, it does nothing. I then changed it to run APAP from 12 to 15 and have more good nights.

    When i change my settings on my own, they get overridden the next day or two by my doctor’s office automatically. How do I explain to my doctor that I know better than she does so she stops changing the settings back?😊

    Thanks for this one Jason. It would be good if you could shrink the pressure graph and include the Flow Limit graph (which we can just see at the bottom of screen) in future videos like this. Resmed use flow limits as one measure driving pressure changes, and this should ideally increase pressure without H or OA events, although as you point out there can be arousals and SDB which do not meet the H/OA criteria. Resmed flow limits are a heuristic (i.e. wild guess) algorithm measuring flatness (or non-nippularity?).

    Wow great video. I honestly thought I was better off on APAP and have been with a tight range for a couple years now, but I just compared my Oscar data after watching this, my CPAP data vs my APAP data, and came to the same comparison, I actually did better on CPAP after looking over my cycles and breathing and events. I noticed the same up and down pattern on APAP and noticed that when on APAP it would keep me on the lower end of my pressure range and even when im struggling, it will stay low or barely move up pressure, then slowly drop again, even though looking at the breathing, it ain't got no nipples, it would start to flatten out, and the pressure would stay at minimum, then I would have an event. Which is wild to think about, I never really took a deep dive into my APAP data, mainly just went off of AHI or how I felt. I will be switching back to CPAP mode tomorrow, also have a V-com on order from CPAPsupplies, as well as other accesories for my machine. I currently had to go back to my Deathstation CPAP because on the Philips machines, you can change the daily time cutoff for your cpap data, and as a truck driver with ever changing schedules, on the Airsense 11, some days would have 2 sessions and then the next day would show no usage, so I dropped out of CPAP compliance for my DOT Medical. No worries though I removed the foam out of my DS1 very early on and did it with the coat hanger method, no cuts. Honestly besides the absolutely shitty company behind it and the death foam, the Dreamstation is still my favorite machine I own. The running on 12v and having the ability to use it on just the 12v cigarette lighter plug is amazing, for being a truck driver, as well as when camping. I own the Deathstation One, The Deathstation 2 (Sent as replacement, Would never buy it), and a Resmed Airsense 11. Still prefer the Dreamstation One.

    Hi Lanky! Really appreciate your content and it has helped me out heaps. Especially the use of a soft cervical collar, I can't thank you enough for that suggestion. Are you able to do a video showing a close up of a persons breathing profile should look like in the flow rate chart, and what different shaped peaks and dips mean? For example, my inspiration has an increase followed by a flatting then followed by the peak before exhalation. What does that shape mean? Why is it not sinusoidal?

    I also think this would be really useful. This is the first time I've heard Jason share examples of bad sleep patterns that don't necessarily count as 'events'. My wave forms look like they've gone through a blender, but I actually have a very low number of classifiable events so I went undiagnosed for years.

    In a lab setting these would count as events. Machines are horrible at identifying subtle events and very often very clear sleep disordered breathing events.

    @@Freecpapadvice I was put on a ResMed 10 back in 2020. I thought it was a CPAP but now confused. What I remember it was set from 4-15 ? I guess it ramped up as I slept. It never felt like normal breathing. Seemed I was breathing against air even when first put on. Never slept more then 2 hours with it on. Worse sleep I ever had. During Covid so never got a lot of one on one care in the office. What I can remember the highest I remember seeing it go up when asleep was 8 and that felt like a wind tunnel. I took it back after 3 weeks and three mask. What was wrong?

    ​@@2007vwbeetleI'm relatively new too (6 months), but it takes time to get used to it. For me, I found that a face mask works better than nose pillows. Also, my air pressure had to be adjusted down. At higher pressures (14-15), my mask would leak no matter how tight it was, and that kept waking me up. Each person is different and you will need to find what works for you. You might want to talk with your doctor about giving it another try. My sleep has greatly improved, and I only have mild sleep apnea.

    just got a ResMed. When breathing goes non-nipple, it starts increasing the pressure. However, when pressure drops (it seems to decrease it slowly for ME) it takes quite awhile for the breaths to flatten. It's certain not proactive thou!

    You can train your brain with a static pressure. You can not train your unconscious brain with variable algorithmic pressure. Seems like it's good at first to get your ranges then once ranges have been discovered beat to switch to a CPAP.

    The for her mode ive read all the different modes when i got my as11 . The for her mode doesnt go past 10 cm h20 . Also takes five events before it will increase the pressure.

    I never got any settings when I started and have a Vauto EP 10, IP 17, PS 3 . Going to try straight CPAP at 11. If you have a 6 pack of beer on occasion, should you increase the setting by 1 or so in your experience? 🐸🐸🐶🐶

    CPAP is always better!! If your on a apap, get your SD card and import it into Oscar, look at key levels on where the events started than go a bit higher and use CPAP!!! I was on apap my results varied alot each night, since I was on CPAP /fixed pressure, my ahi is stable now between 0.5 and 1.5

    Just got prescribed an APAP. Been using it for 6 days now. They have me on 6-19 and although I feel better than I did before therapy and I think I’m adjusting to using the machine quite quickly, I looked at Oscar and it’s crazy how crappy my sleep really is. Your channel is fantastic and I’m pretty sure I’m going to have one of those consultations with you in the future. The algorithm sucks. I bought the AirSense 10 over the 11 because of the reviews. CPAPsupply 👍🏻. Already raised my pressure to 10-19 but my average a night says 12.4.

    Hi Lanky, I have been constantly changing my pressures in apap setting on my resmed 11 to try and find the right pressure for both comfortable sleeping and keeping my AHI's below 5. I find that Whatever auto 😢max pressure I set my machine at it always sits right at the max line all night as if it needs to go higher whatever the max 9, 10, 11. I was thinking of switching to cpap but don't know at what pressure setting ? I want to start at 8 but worried if I should use 8? What if the pressure wants to rise higher? Is it ok to try it at 8? How will I know if that pressure is not good for me? Help! Thanks

    I tried cpap mode on a DS1 a few years ago because even with self-titration on apap I could not get my ahi under 1. Once on cpap mode @ 16cmh2O with cflex+ on 2 I sleep much better, have an ahi under 1 and my leakrate is lower, apap is garbage.

    Ahh Hi Jashohn!! Hey I am brand new to CPAP. So brand new I haven’t even gotten my machine yet. I got prescribed an APAP as it was found I have “Mild Sleep Apnea” Anyhow. I hear you saying to try this preasure and that preasure, but I’ve also seen where my pressures are prescribed and either should not/could not be changed by me the patient. I am assuming maybe I have been prescribed an APAP so I can be more quickly introduced to my new therapy by letting my APAP do all the work on adjusting my pressures? I think this because once I get my equipment from the supplier, my next appointment is 30 days after to go over how compliant I have been in using the machine. Also it sounds like there is an option to use my APAP in CPAP mode? Is that also something I can change? I am really interested in “nerding out” to this stuff as I am figuratively and literally quit tired of being tired and I’m really hoping CPAP will make a positive change in my life that allows me to be fully rested in the morning so I have the energy to live life to its fullest rather than looking forward to getting to bed in a vain attempt to recharge for the next day. Thanks for the very informative videos. So far I’ve gotten way more insite from you than the doctor. I will also be getting one of the V-COM adapters since you highly recommend it!!

    I'm sure both work.....but everyone is different....at the end of the day its how you feel when you wake up...."if it aint broke don't fix it" if it is, then don't give up until you find your place. In my opinion your mask is the most important part of therapy....if you don't solve that then all the numbers are meaningless except for mask leak. For me I played with several masks until i found the perfect mask for me....unfortunately here in Australia trying several masks can be very costly. My other point would no sleep technicians take a hollistic approach to sleep apnoea and cpap therapy. 😊

    I have read that resmeds algos are awful. Other machines were suggested including the push for the Lowenstein machine? I'm in the process of selecting a cpap machine and I am open/looking for advice on the initial purchase.

    Not a big fan of APAP, constant attempts to get the right pressure lead to bad outcomes. I work in a sleep lab like our host, I am an RRT. Get a in lab study nail your pressure hence a CPAP. I use a BIPAP now as I am a train wreck 22/20 and my numbers are good. I have been on for 25 years...................

    Have you tried APAP with a narrow range like 10-11 or 11-12? What has been your experience? Would if the right range allow for good sleep on your side and back combined?

    I was on a range of 10 to 12 for months and it was good. Having a narrow range was way better for me!! Now I'm set with a solid pressure of 12 and I'm sleeping better than ever!! If I ever go back to auto I would do a range of 11 to 13. So far a solid CPAP pressure has made me sleep the best. I'm 36 and got my CPAP almost a year ago. Most of my family has sleep apnea.

    Hey Jason.....I bought the V-com and honestly not impressed. I found that during ramp time (5min) I felt like I was not getting enough air flow. My history is that I have been using a cpap for 15 years and two years ago got a new machine (Resmed 10). I use my old machine when I travel so I will give that V-com another try on that machine.

    Is 8 too low? Any higher and I get air going to gut. I have acid reflux as well. At 10cm no sleep. 🎉

    I have an Ibreeze 20, APAP mode for this machine isn’t good. The best mode for APAP was when I kept the range narrow, 10-15. My logic was if I’m keeping my pressure range so narrow, why not just use CPAP? I switched to 12 cm with v-com and it worked better.

    I’ve been fighting with sleepiness for months, can’t seem to get my machine dialed in just right, recently raised min to 11 and max to 14.5, but now it’s getting up to 14.5 and then getting leaks… going to try 12 tonight on cpap not apap.. it looks like on Oscar that I’m usually right around 12 all night … hope it helps! I can’t take much more of this foggy sleepy feeling every day

    Yeah I have struggled with this same thing. The pressure you really need seems to be above the mask's ability to withstand it. You're probably going to need to try some different masks (if you haven't already) until you find one that fits your face better. And try just tightening your mask up a bit. Sometimes very small changes can make the difference. My mask seems just a bit too tight when I first put it on in the evening, but when I wake up it feels fine. I've been PAPing for just a bit over a year now and feel like it's only been the past few months where I finally have worked out an ideal --pressure/mask/position-- combo that's working really well, and my AHI has dropped below 1. I don't know how long you've been on PAP, but keep at it despite the frustrations. It will come together for you eventually. Do you have a doctor or sleep tech you're working with? That's actually looking at your sleep data? If not, you might try something like Jason's service. I'm really not a shill for him, but if you can't seem to get the pieces of the puzzle to fit together, it might be worth having a consult with him to gain some insight. Good luck.

    @@BLenz-114 on cpap for about 7 years now, just sleepy and foggy every day. I’ve tried every mask , even have a bag full of masks that didn’t work lol.. I was ok with a bad night here and there , but now it seems every night is a bad night, just getting very frustrated, so many variables, raise min pressure? Raise max pressure, turn auto off, try a chinstrap (made my jaw hurt) tried a backpack gizmo made to keep you off your back during sleep ..

    @@maxmasterson3804 did you try Bleep? Both Nick and Jason have said the owner was really nice so I emailed them because I wasn't sure the Bleep would work for me (wearing "For her Masks" aka small headgear, and pillows IN XS, and small pillows are too big). The owner wrote back and said he wasn't sure if they would work, but he'd send me the adhesive samples so I can see for myself...they are too big (if I were a small-large they would have worked). I mentioned this because it might be a good solution for you and you can ask for adhesive samples if you have concerns about sizing or allergic reactions. They are covered by insurance as well. Second thing, you might want to get an spo2 monitor to see if there is an issue. I export both by resmed data and my o2 ring data to Oscar and you can see how it lines up, both sets of data are really helpful to figuring out what to do. Even if I have low AHI, if my spo2 is bad, I need to increase the pressure. It's only been a few weeks of futzing with my settings and the last three nights I didn't go below 88% once. Oh last thing, if you aren't using full face masks, MOUTH TAPE and/or cervical collar. What a difference!

    I have been using CPAP for three months now & its working freaking fine (APAP mode). since two week now once I use the CPAP my nose gets blocked & I wake up with severe headache (since air does not flow properly through my nose). I tried changing the filter, sterilizing but all did not worked out Anyone knows why??

    I’m not much with these iPhones and technology. I’ve got a Rez 10 I have 6.5 apnea an hour. I wanna just use CPAP trying to figure out what to set my pressure at.

    This is weird... I have an Airsense 10 on APAP, with a 8-14 range. It tends to start at 8, slowly ramp up to 13, and then basically spends most of the night at a stable 13, with some slight reductions down to ten or so. My pressure graphs look absolutely nothing like yours in the video. There are barely any changes in pressure during the night, and definately not that wild pressure zig-zagging, that you seem to have.

    So I’m curious how things would be for someone who is accustomed to APAP if you tried these experiments with them. I don’t have the 11 but I have the 10 and I’m used to APAP and my pressure is 4-20 (I know that’s terrible in your opinion)

    Nobody should ever be on that wide of a range. It just doesn't work very well. See if you can get your sleep tech to narrow it. All the best in your journey.

    I don’t understand what you are saying. I first had a CPAP by Phillips set to 9. I never got below an AHI of 8 and most ly more. This machine had an option to try an APAP and my AHI immediately droppped to under 5. I bought myself an ResmedSirsense 10 and set it to 11-20. My AHI most nights is under 1 for the last year or so. Confused.

    Sorry for my ignorance but 3 marginal doctors later and TWENTY THREE years after first being diagnosed with severe sleep apnea (and STILL untreated and suffering greatly), if one has "mixed" apneas with central sleep apnea, is APAP better or worse for that? Seems I heard, or most likely read somewhere that CPAP should not be used if one has central apneas? Maybe APAP is better, recommended for those with central sleep apnea?? Thank you!

    If you have mixed apnea with a high percentage of them being central apneas then you may be a candidate for ASV (auto servo-ventilation). My training was in radiography, but I have read that CPAP and Bi-Pap don't treat central apnea.

    I’ve asked this question a few times. I want to get off of APAP and on to continuous. I’m gonna mention it to my doctor at the first follow up since i started therapy. If he doesn’t want to help me try continuous, how long should i wait to mess with the settings myself without my insurance stopping payment. In other words, when is the machine usually paid for by insurance?

    I was told they check if you are compliant for three months and if so they'll pay the monthly and you own it in 10 months. But that might be my insurance/DME. I did change my settings from the get-go, then a week in I called the DME and dr and said I can't use Oscar with a Luna I want an airsense, and I don't like the mask, they sent a guy over a week later... With an airsense 10... then I started futzing with the settings every night, and a week later they mailed me an airsense 11 (I called the dme to ask why, and they said, wait we did? Oops ... I called back and they told me the dr requested it.. they told me they'll send a call tag I should send one back even though they don't have a record of actually sending it 😁🤦🏼). Had appt with the dr, they are like whatever you did is fine you're data looks great... Keep on doing, you know, whatever you did. 😒

    Btw, I am juggling the pressure (CPAP 8.4) and bloating stomach (wake up with cramps). So hard ..

    I had CPAP for 15 years and a couple years ago they replaced my machine with an APAP. My CPAP pressure was set to 8 and my average pressure on APAP is between 5.5 and 7.0. I'm getting much better sleep on the APAP. AHI 8hrs. The only time it changed was when I had surgery and had sedation. Average pressure went up. To 8.5, AHI of 12.5 and CAI of 6.2.

    Those are all super low leaks under 10 Liters/Min. How do those impact anything? Your idea of a bad leak is most people's dream numbers! Maybe that's because the CPAP / APAP pressures are pretty low.

    I definitely do better on APAP than CPAP, Oscar proves it for me. Less tired, less cranky, less forgetful .

    This is an interesting concept that I don't disagree with. Would probably make a really good video to explain. In short, you may be trading awakening from mouth leaking for sleep disordered breathing events. A WHOLE lot I don't know about your particular situation, but it's a pattern that I've seen a lot of.

    How does my ResAir11 identify a central apnea? I don’t have a real sleep specialist medical professional but what I do have had to be pressured to see me within 90 days for Medicare coverage. I might need to ask you for consultation. I was started on 4-10 APAP but I hate the low 4, and I was pushing 10 every night so I changed it to 5-12. I really want to find a CPAP setting that works better.

    Fan of the channel but I gotta say your methodology set you up to fail on your APAP test. If you know your optimal pressure is 11 or 12, why would you set your APAP to 8-15? Set it to 11-15, Jahssen! I did this experiment myself about a year ago. Found that my optimal pressure is 8-9, so i set my apap range to 8-12. If i start it at 5 or 6, I'm never getting the pressure I need and it's my own fault, not the APAP. Maybe CPAP is better for you, maybe it's not, but you didn't really prove anything with this one. Would love to see your results with a 11-15 range instead.

    I understand what you're saying, but to me it's a fail no matter how I set it up. The way I did shows what happens to most people: wide range with a low minimum and the pressure will never reach and stay at optimal pressure. Had I started at my optimal pressure that would just allow it to go too high when and if false positive OSA occurs. And to my point: if you know your optimal pressure, why not use it on CPAP? I know mine but wanted to test something that is familiar to most people.

    @@Freecpapadvice an optimal pressure is a range, though. I need another cm or two when I roll onto my back. If I'm on fixed pressure, I'm either setting for back pressure and getting too much pressure when I'm on my side or I'm under pressured when I am on my back. I think APAP with a tight range is going to be better for most people. They just have to get it dialed in, which to your point is a process a lot of people can't or don't bother to do.
