I've had something similar happen to me, basically Rev01 gives you an extra life after achieving 50000 points but only if the region is set to JP. My guess is something got overwritten to the RAM address it saves the next threshhold and gives you a life prematurely.

I'm curious if it affects all Japanese Mega Drive systems or just certain board revisions. I know VA0 for example is different from any other Mega Drive/Genesis released outside Japan.

I'm late, but it might be caused by using an incorrect sound command, just like Sonic 2 does with the Continues code (the one in the sound test), as it causes Oil Ocean to play indefinitely, similar to this.

The Scary thing is green hill is always playing. Sonic Team would enjoy this

(4:08) I don't claim to be any sort of expert on the Genesis/Mega Drive, but I have a theory for you. In vanilla Sonic 1 the one up theme plays on the sound effect channel if I understand correctly. Maybe this glitch is causing Green Hill Zone to be played on the sound effect channel in its place, thereby overriding all the other SFX and music. The song never encounters a flag telling it to stop since it's playing on the wrong channel, so it keeps looping forever.

How did i guess this was going to be Sonic.exe? Also I think that green hill zone music bug is basically a sound driver crash or communication error. It's an issue that mainly only occurs with Sonic 2, 3 and SK and it's when the sound driver enters an error state and stops accepting commands and basically gets stuck playing whatever sound or music track until it's reloaded but it rarely occurs in sonic 1 because normally when Sonic 1's sound driver breaks or glitches out the entire game crashes. You can see an example of this issue in the original sonic 2 when you enter the continue cheat on the level select or by playing certain sounds that are invalid or empty in the sound test.

This game has scared me for 10 years. And it still does.

This has probably been said already, but the music and life bugs likely stem from the disassembly. I dabbled a bit with hacking Sonic 1, and the disassembly I used had code that enabled stuff like the 50,000 point lives only if the region is set to Japan. I think it wouldn't even be an issue if the region-based code was removed or edited.

That intro was hilarious. It sure got me good

Great hack for a Halloween episode! You should do another various hacks episode, like Knuckles' Emerald Hunt, Dr. Robotnik's Creature Capture, ones like those. Otherwise, great episode!

The only thing that scares me in this game is the CREEPY screen that appears when you go to score records, just like we see on this video.

The reason why starlight scrap brain and final are inaccessible are because it only goes up to labyrinth and once you beat act 3 you see the sega screen and the jump scare from trying to access those levels in score records

im honestly kinda sad to see this rom hack not talked about more and shocked to not see any new .exe stuff based on this. for me this was the best sonic.exe thing to come out back in the day as it was a really cool concept, with a great new spice to gameplay. some of the new stuff people have done like eyx and omt/olr are great, but its really missing the gameplay in my horror game. and even today i think this rom hack and sonic.exe spirits of hell are the best .exe content there is. i hope to see more like an ordinary sonic rom hack in the future (if i knew how to mod sonic 3 air or make rom hacks i would do it myself)

I remember watching my friend play this and he was in Labyrinth Zone and Green Hill was playing. He was playing on an emulator, so it isn’t a hardware problem

Actually I tested this on emulation. I set the country to Japan and yes I am receiving the life counter glitch and some random person emulated this when recording a gameplay of the rom hack got the green hill glitch so no it's not a hardware issue.
